Esta pesquisa teve como objetivo analisar a política e a configuração da atenção à saúde bucal, com ênfase a partir dos anos 2000, nos municípios cearenses de Sobral, Juazeiro do Norte e Fortaleza. A investigação baseou-se no referencial de análise de políticas públicas e em contribuições da abordagem do institucionalismo histórico. As principais categorias e variáveis consideradas foram: a trajetória de implementação da política de saúde bucal, a relação entre o contexto nacional, estadual e municipal da política, a caracterização da configuração dos sistemas de atenção à saúde bucal, a configuração público-privada de atenção à saúde bucal, os condicionantes e a institucionalidade da política de saúde bucal nos municípios. A pesquisa envolveu revisão bibliográfica, análise documental, análise de dados secundários e entrevistas com os dirigentes da política de saúde bucal em nível estadual e municipal, bem como, com dirigentes dos Centros de Especialidades Odontológicas. Antes dos anos 2000, a política de saúde bucal no estado do Ceará e nos municípios estudados era marcada pelo predomínio de ações e serviços odontológicos assistenciais curativos, pontuais e mais direcionados para grupos específicos como os escolares, ainda que já existissem alguns centros de atenção odontológica ligados à esfera estadual. A partir dos anos 2000, a política de saúde bucal no estado do Ceará expressou mudanças relacionadas às inflexões na política nacional. Houve expressiva expansão de serviços odontológicos públicos no conjunto do estado e nos três municípios estudados, porém sob contextos e com diferentes momentos de implementação em cada município. Além da forte influência federal, outros fatores que condicionaram as políticas municipais foram: a atuação da esfera estadual, o comprometimento e peso político de atores envolvidos com a saúde bucal e o lugar ocupado por essa política na agenda governamental local.
A articulação dessas variáveis levou a diferenças nos incentivos e nas condições de organização da atenção em saúde bucal entre os municípios e ao longo do tempo, com implicações para a institucionalidade dessa política. O estudo também permitiu identificar alguns limites ao fortalecimento da política pública na área, concernentes, por exemplo, às relações público-privadas, à integração entre serviços e à gestão do trabalho em saúde bucal, a serem exploradas em estudos futuros.
Abstract
The aim of this study was to analyze the policy and configuration of oral health care, focusing particularly on the 2000s onwards, in the municipalities of Sobral, Juazeiro do Norte and Fortaleza, in the state of Ceará. The investigation was based on the theoretical contributions of public policies analysis and of the historical institutionalism approach.
The main categories and variables considered were: the history of the implementation of oral health policy, the relationship between the national, state and municipal contexts of the policy, description of how oral health care systems are configured, the public-private arrangement for oral health care, and the conditioning factors and institutionalization of oral health policy in the municipalities. The research involved bibliographic review,
documental analysis, secondary data analysis and interviews with oral health policy managers at a state and municipal level, as well as with managers of Dental Specialty Clinics. Up to the 2000s, oral health policy in Ceará and the municipalities studied was marked by a predominance of isolated and curative dental care services and actions directed at specific groups such as schoolchildren, although there were already some dental care centers linked to the state administration. As from the 2000s, oral health policy in Ceará has displayed changes related to the shifts in national policy. There has been a significant expansion in public dental services in the state and in the three municipalities studied, however this has been implemented in different contexts and at different times in each municipality. In addition to strong federal influence, other factors that have conditioned municipal policy include: the role of the state administration, the
commitment and political weight of the actors involved with oral health care and the priority given to this policy on the local government agenda. The combination of these variables led to differences between the municipalities in the incentives and conditions for the organization of oral health care and, over the course of time, to implications for the institutionalization of this policy. The investigation also resulted in the identification of some limits to the strengthening of public policy in the area, regarding, for instance,
public-private sector relations, integration between services and work management in oral health care, which issues are to be explored in future studies.
