A poliomielite é uma doença viral de natureza infectocontagiosa, clinicamente caracterizada pelo desenvolvimento da paralisia flácida aguda (PFA). A Iniciativa Global para Erradicação da Poliomielite, lançada em 1988, tem como meta central a eliminação da pólio do mundo por meio da completa erradicação e contenção de todos os poliovírus (PV) selvagens e relacionados à vacina. A vigilância laboratorial é um componente fundamental dentro do programa de erradicação por fornecer, através do diagnóstico virológico, informações que possibilitam o monitoramento contínuo da circulação de PV globalmente, o que direciona estratégias e tomada de decisões. O objetivo principal deste trabalho é analisar a diversidade de PV de origem vacinal circulantes no Brasil entre 2008 e 2015, isolados no Laboratório de Enterovírus da Fundação Oswaldo Cruz por meio da vigilância laboratorial dos casos de PFA e vigilância ambiental. Para tanto, foi realizada uma análise descritiva dos isolados entre 2008 a 2015, considerando variáveis como procedência geográfica, período de isolamento e sorotipos virais. Entre 2008 e 2015, foram analisadas 3242 amostras de fezes provenientes de casos de PFA, das quais 2.5% foram positivas para PV. Todos os PV isolados foram classificados como \201CSabin-like\201D e um isolado Sabin tipo 3 (LEV45507) foi identificado como recombinante natural intertípico Sabin 3/Sabin 2 na região do capsídeo. Quanto às 134 amostras de ambiente, 77.6% foram positivas para PV
Um isolado PVDV tipo 2 altamente mutado (8.6% de divergência em VP1) foi isolado a partir de amostra do ambiente (LEV44624). Os genomas completos dos vírus LEV45507 e LEV44624 foram sequenciados para caracterização molecular. LEV45507 apresentou um único sítio de recombinação, localizado na porção 3\2019 do gene VP1. Dois determinantes de atenuação do genoma de Sabin 3 apresentaram reversão para o fenótipo de neurovirulência (nt 472 em 5' NC e nt 2493 em VP1), no entanto testes de caracterização fenotípica não identificaram alterações no perfil atenuado do vírus. Por sua vez, LEV44624 apresentou mutações em dois determinantes de atenuação de cepas Sabin 2 (nt 481 de 5\2019 NC e aa 143 de VP1), além características fenotípicas que indicam reversão para o fenótipo neutrotrópico. Foram descritos diversos padrões de recombinação com outros Enterovirus C ao longo do genoma de LEV44624. Baseado no relógio molecular dos PV, o tempo de circulação deste vírus foi estimado em ~8.5 anos. Não foi identificada relação filogenética estreita entre LEV44624 e qualquer outro vírus (PV ou Enterovírus não-pólio) isolado no Brasil, indicando que se trata de um vírus importado. As sequências genômicas dos vírus LEV45507 e LEV44624 foram depositados no GenBank (KU763188 e KU372652, respectivamente). A vigilância laboratorial dos PV, executada de forma sistemática, é fundamental para diagnosticar vírus circulantes e identificar casos de importação
Abstract
Poliomyelitis is a viral infectious disease, clinically characterized by the development of acute flaccid paralysis (AFP). The Global Polio Eradication Initiative, launched in 1988, aims primarily the elimination of polio worldwide through the complete eradication and containment of all wild and vaccine-related poliovirus. The laboratory surveillance is a key component within the eradication program, because it generates, through virological diagnosis, information that enables continuous monitoring of PV circulation globally, guiding strategies and decision-making. The main objective of this study is to analyze the diversity of vaccine-related PV circulating in Brazil between 2008 and 2015, isolated in the facilities of the Enterovirus Laboratory (EL) at Oswaldo Cruz Foundation by laboratory surveillance of AFP cases and environmental surveillance. Therefore, a descriptive analysis of isolates from 2008 to 2015 was carried out, considering variables such as geographic origin, period of isolation and viral serotypes. From 2008 to 2015, 3242 stool samples from AFP were examined, and 2.5% were positive for PV. All isolates were classified as Sabin-like and one Sabin type 3 (LEV45507) was identified as an intertypic Sabin 3/Sabin 2 capsid recombinant
Regarding the 134 environment samples, 77.6% were positive for PV, and one highly mutated PVDV2 (8.6% divergent in VP1) was isolated (LEV44624). The complete genomes of LEV44624 and LEV45507 were sequenced for molecular characterization. LEV45507 presented a single recombination site placed in the 3\2019 end of VP1 gene. Two determinants of attenuation for Sabin 3 genome had reverted to wildtype (nt 472 in 5' UTR and nt 2493 in VP1) however the isolate retained the attenuated phenotype. LEV44624 presented mutations in two major determinants of attenuation for Sabin 2 strains (nt 481 5' UTR and aa 143 in VP1) and phenotypic characteristics show reversion to the neurotropic phenotype. Different recombination patterns with other Enterovirus C virus were found throughout the genome of LEV44624. Based on poliovirus molecular clock, its circulation time since the initial OPV dose was estimated to be ~8.5 years. No close phylogenetic relationship was described between this isolate and any polio or non-polio enteroviruses isolated in Brazil, indicating that this is an imported virus. The genome sequences of LEV44624 and LEV45507 were deposited in GenBank (KU763188 and KU372652, respectively).
