POLIMORFISMOS DOS GENES HLA-G, IL-10, TNF E PERFIL DE CITOCINAS EM PACIENTES COM LÚPUS ERITEMATOSO SISTÊMICO DE INÍCIO NA INFÂNCIA

Resumo
Lúpus eritematoso sistêmico de início na infância (LESi) é uma doença autoimune crônica que apresenta fenótipo clínico agressivo. Citocinas pró-inflamatórias participam da patogênese da doença. Neste contexto, determinamos citocinas Th1 (IL-2, IFN-gama, TNF), Th2 (IL-4), Th17 (IL-17A, IL-6) e Treg (IL-10) em pacientes com LESi e grupo controle, assim como avaliamos polimorfismos na região promotora dos genes IL-10, TNF e na região 3' não traduzida do gene HLA-G. Também determinamos concentração plasmática da proteína HLA-G. Conduzimos estudo caso-controle com 51 pacientes com LESi; 10 pacientes participaram de estudo longitudinal; amostra de sangue foi coletada no mesmo paciente com doença ativa e após com doença inativa ou vice-versa. Pacientes apresentaram aumento de IL-6 (P=0.0002) e IL-10 (P=0.0002) quando comparado com grupo controle. Pacientes com doença ativa apresentaram níveis aumentados de IL-6 e IL-10 do que pacientes com doença inativa (P=0.001 e P=0.014, respectivamente) e grupo controle (P<0.0001 e P<0.0001, respectivamente). No estudo longitudinal, níveis aumentados de IL-6 (P=0.022), IL-10 (P=0.013) e IL-17A (P=0.041) foram observados durante doença ativa em relação à doença inativa. Nenhuma diferença significativa nas frequências genéticas nos genes estudados foram encontradas entre pacientes e controle. No entanto, o genótipo 14 pb Del/Del do HLA-G apresentou associação significativa com nefrite lúpica (OR=8.00, IC 95 por cento=1.57-40.65, P=0.006) e o alelo - 863 A do TNF com serosite (OR=4.17, IC 95 por cento=1.24-13.98, P=0.027). Em relação à expressão do HLA-G, não foi observada diferença entre os grupos. Ainda assim, pacientes com manifestações hematológicas apresentaram menor concentração do HLA-G quando comparado com pacientes sem manifestações hematológicas (P=0.013). IL-6, IL-10 e IL-17A apresentam papel importante na patogênese do LESi; já o HLA-G pode estar envolvido na patogênese da doença. Polimorfismos nos genes estudados não apresentaram associação com susceptibilidade ao LESi (AU)

Childhood-onset systemic lupus erythematosus (cSLE) is a chronic autoimmune disease which exhibits an aggressive clinical phenotype. Pro-inflammatory cytokines are involved in disease pathogenesis. In this context, we determined plasma levels of Th1 (IL-2, IFN-γ, TNF), Th2 (IL-4), Th17 (IL-17A, IL-6), and Treg (IL-10) cytokines in cSLE patients and healthy controls, and we evaluated polymorphisms at promoter region of the IL-10 and TNF genes and at the 3’ untranslated region of HLA-G gene.
We also determined plasmatic concentration of HLA-G protein. We conducted a case-control study with 51 cSLE patients; 10 cSLE patients participated in a longitudinal follow-up study; blood sample was collected from the same patient during active and inactive disease or vice-versa. cSLE patients had higher IL-6 (P=0.0002) and IL-10 (P=0.0002) levels than healthy controls. Patients with active disease had higher IL-6 and IL-10 levels than patients with inactive disease (P=0.001 and P=0.014, respectively) and the control group (P<0.0001 and P<0.0001, respectively). In the longitudinal study, IL-6 (P=0.022), IL-10 (P=0.013), and IL-17A
(P=0.041) levels were significantly higher during active than inactive disease. No significant difference in genetic frequencies of the studied genes was observed between patients and control group. However, the 14 bp Del/Del genotype (OR=8.00, 95% CI=1.57–40.65, P=0.006) showed an association with lupus nephritis and the TNF -863 A allele with serositis (OR=4.17, CI 95%=1.24-13.98, P=0.027). Regarding HLA-G expression, no significant difference was observed between the groups. However, patients with hematological manifestations showed a lower plasma concentration of HLA-G than patients with no hematological manifestations (P=0.013). IL-6, IL-10 and IL-17A play an important role in cSLE pathogenesis; on the other hand, HLA-G may be involved in disease pathogenesis. Polymorphisms in
the studied genes showed no association with cSLE susceptibility.
