Abstract in Portuguese
O Apoio Matricial surge como uma ferramenta de gestão e compartilhamento de saberes na prática do cuidado em saúde. Atualmente apresenta-se como principal vertente do trabalho das equipes do Núcleo de Apoio à Saúde da Família (EqNASF) atuantes na Atenção Básica. O NASF por sua vez, se insere por meio deste apoio na dinâmica de trabalho das unidades básicas de saúde, junto às demandas apresentadas pelas Equipes de Saúde da Família (EqSF), atuando tanto nas ações clínico-assistenciais (ação direta com o usuário), como no suporte técnico-pedagógico (apoio nas atividades de ensino). Esta pesquisa por sua vez configura-se como um estudo de caso sobre as possibilidades das práticas pedagógicas desenvolvidas pelas equipes NASF no município do Rio de Janeiro, especificamente a área programática 3.3 (A.P. 3.3). Configura-se metodologicamente como estudo qualitativo tendo como referência para análise dos dados a Hermenêutica Dialética ao considerar os sentidos da fala e todo contexto nela inserido. Foram realizadas 18 entrevistas individuais com os profissionais que compõe as equipes do NASF e da Saúde da Família (três de cada equipe) do Centro Municipal de Saúde Enfermeira Edma Valadão e a Clínica da Família Souza Marques (ambas unidades da A.P. 3.3). Elencamos com as equipes de saúde da família entrevistar um profissional de nível superior e dois de nível médio. Além disso acompanhamos por meio de observação participante: a) a rotina de apoio matricial da EqNASF nas consultas conjuntas com os profissionais das EqSF; b) a rotina de reuniões de EqSF, que acontecem semanalmente para discussão dos casos; c) e os espaços formais e informais de desenvolvimento de atividades de ensino-aprendizagem apoiados pelo NASF junto à EqSF. Das diversas contribuições que o Ministério da Saúde apresenta por meio do trabalho na lógica matricial, questiono-me então se o NASF tem sido ferramenta de ensino-aprendizagem ou tem sido atropelado pelas necessidades clínicas das EqSF, considerando a dinâmica de trabalho das equipes onde metas e indicadores sinalizam a todo tempo uma saúde em torno da produtividade, num território vulnerável.
Abstract
The Matrix Support comes as a tool of management and sharing of knowledge in the health care practice. It is currently presented as the main part of the work of groups active in the Primary Care of the Núcleo de Apoio à Saúde da Família (EqNASF - Support Nucleus Family´s Health). NASF, in turn, inserts itself through this support into the working dynamics of Primary Health Care Units, together with the demands presented by the Equipes de Saúde da Família (EqSF - Family Health Groups), acting in clinical care actions (user direct care), as well as in technical and pedagogical support (support in teaching activities). This research, therefore, is set as a case study about the possibilities of pedagogical practices developed by NASF groups at Rio de Janeiro city, specifically the programmatic area 3.3 (A.P. 3.3). It is configured methodologically as a qualitative study having as reference for data analysis Dialectic Hermeneutics for considering the meaning of speech and the entire context of it. 18 individual interviews were held with professionals from NASF and Family Health groups (three from each group) at Centro Municipal de Saúde Enfermeira Edma Valadão (Municipal Health Centre Nurse Edma Valadão) and Clínica da Família Souza Marques (Family Clinic Souza Marques) – both A.P. 3.3 units. We chose to be interviewed in the Family Health groups one professional with a higher education degree and two with a secondary education degree. Furthermore, through participant observation we followed: a) the matrix support routine of EqNASF in conjoint consultations with EqSF professionals; b) the routine of EqSF weekly meetings for case discussions; c) and the development of teaching-learning activities in formal and informal spaces by NASF and EqSF together. From the diverse contributions that the Health Ministry presents through the matrix logic work, I ask myself if NASF has been a teaching-learning tool or has been ran over by EqSF clinic necessities, considering the work dynamic of groups in which goals and indicators signal all the time health around productivity on a vulnerable territory.
