Resumo
No momento em que se discute a expansão da Atenção Primária a Saúde (APS) no
Brasil e na cidade do Rio, a avaliação do modelo centrado na Estratégia de Saúde
da Família (ESF) como protagonista do acesso e qualidade dos serviços ganha
importância. Adota-se como hipótese que o Programa Saúde Presente/Rio tenha
provocado mudanças no sentido da reorientação do modelo na atenção básica à
Saúde na Área Programática 5.3. Houve modificações nas condições de estrutura e
processos (de gestão, de organização do trabalho, de logística), alcançando-se
alguns resultados de curto/médio prazo na atenção à população, considerando os
referenciais teóricos sobre pilares da qualidade dos serviços de saúde propostos por
Donabedian (1966). O objetivo foi descrever os aspectos da expansão à luz dos
relatórios gerenciais e documentos oficiais da Secretaria Municipal de Saúde,
identificando fortalezas e fragilidades nesse processo. Os resultados foram
sistematizados considerando as dimensões das mudanças ocorridas na estrutura,
nos processos e em alguns resultados selecionados. Quanto à estrutura, verificou-se
a ampliação das unidades e quantidade de equipes de Saúde da Família.
Atualmente são 26 unidades (apenas uma não dispõe de equipes), atingindo 97% da
população com atendimento pelo Saúde da Família. Em relação aos processos,
investiu-se em capacitações, implantação da Carteira de Serviços, Protocolo de
Enfermagem, Prontuário Eletrônico e Academia Carioca, consequentemente
ampliando-se o acesso aos serviços, com elevação dos procedimentos e da
cobertura vacinal, e a participação mais ativa das lideranças locais. Ressalta-se que
ainda há fragilidades, como a articulação das Redes de Atenção à Saúde, porém
todo esse processo é contínuo e vai ao encontro da Política Nacional de Atenção
Básica, na perspectiva da integralidade, coordenação do cuidado e superação de
paradigmas no campo da saúde, possibilitando uma mudança na concepção do
processo saúde-doença. Assim, está-se trilhando caminhos rumo a uma concepção
mais ampla de saúde, permitindo inovar e atingir uma qualidade, que evolui a cada
dia.

At the time the expansion of the Primary Health Care (PHC in Brazil and in Rio) is
being debated, the evaluation model based on Health Strategy (FHS) earns
importance as protagonist of access and quality of services. We took into account the
hypothesis that the Saúde Presente/Rio Program has caused changes in the
reorientation of primary care in the Health Program Area 5.3. There were changes in
the conditions of structure and processes of management, work organization, and
logistics, achieving some medium and long term results concerning to rendering of
services to community, considering the theoretical background in the light of the
quality of the health care service proposed by Donabedian (1966). Our intent was
to describe the expansion aspects in the light of the management reports and official
documents of the Rio de Janeiro Municipal Health Office trying to identify the
strengths and weaknesses in this process. The results were systemized taking
account the scale of the changes verified in the structure, in the process and in some
selected results. Concerning to the infrastructure, it was observed an expansion of
the health care units and an increase in the family health teams. Currently there are
26 units (just one of them doesn’t have teams) covering 97% of the population
registered in the Family Heath Program. Regarding to the processes, it was invested
in professional qualification, implementation of portfolio of services, nursing protocol,
electronic medical records and Academia Carioca , resulting in the expansion of the
access to the services, with an increase of the medical procedures and the
vaccination coverage, along with the participation of the local leaderships. There is
still some weaknesses especially with respect to the articulation of the Redes de
Atenção à Saúde (Health Care Service Network), nevertheless this is a continuous
process that meets the expectations of the National Policy of Basic Health Care
(Política Nacional de Atenção Básica), from the perspective of the completeness and
coordination of the health care, thus enabling a changing in the way of understanding
the health-disease process. So we are heading in the direction of a broaden concept
of health, allowing innovation and the achievement of a quality that evolves
continually.
