O cenário da prevenção da infecção por HIV, hoje, é marcado pela discussão e implementação de diversas tecnologias, sendo algumas relacionadas ao uso de antirretrovirais. A orientação e a disponibilização dessas tecnologias dependem, em grande parte, do protagonismo médico. Pretende-se, neste trabalho, entender as percepções, conhecimentos e práticas dos médicos residentes em infectologia do serviço especializado do IPEC-FIOCRUZ sobre as estratégias de prevenção da infecção pelo HIV. Para isso, a etnografia foi a metodologia de aproximação com os residentes do IPEC, através da observação participante e entrevistas. O conhecimento sobre as alternativas de prevenção da infecção por HIV por parte dos residentes se mostrou pouco consistente e sua prática revelou-se basicamente prescritiva. A tradição de um modelo de formação médica que organiza seus saberes-fazeres de modo fragmentado, que não privilegia o profissional pensante nem sua responsabilidade social evidenciam o pouco preparo desses jovens residentes em infectologia. A maneira objetiva de cuidar, voltada para o diagnóstico e tratamento de doenças, é resultado da forma acrítica com que se trata o conhecimento científico, da ausência de uma reflexão política da sociedade e de um juízo filosófico
Abstract
Today, the setting of HIV prevention is marked by the discussion and implementation of different technologies, some related to antiretroviral drugs. The orientation and the availability of these technologies depend on the medical role. This work intends to understand the perceptions, knowledge and practices related to strategies to prevent HIV infection by the residents in a specialized centre of infectious diseases, IPEC-FIOCRUZ. So, ethnography was the approach methodology, using participant observation and interviews with the residents. The knowledge of the residents about the options of HIV infection prevention proved inconsistent, and their practice, largely prescriptive. The tradition of a medical training model that organizes its knowledge and practice on a piecemeal way, and does not favour the reflection of the professional or social responsibility, points out to little skills among these young residents. The objective way to care, focused on the diagnosis and treatment of diseases, is a result of the uncritical way by which scientific knowledge is constructed, of the lack of a political reflection of society and of a philosophical judgment
