A assistência à saúde e a garantia de proteção social universal e integral a população constitui-se no desafio dos sistemas de saúde em qualquer parte do mundo. No Brasil, a criação do SUS trouxe importante conquistas para a população, contudo, a organização da atenção básica a partir da estratégia da saúde da família, e a substituição da fragmentação da média e alta complexidade por linhas de cuidado organizada em redes, com pontos de dispersão formados pelas unidades de saúde da família e de pontos de convergência que propicie a integralidade, é o desafio da gestão nas três esferas de governo. Na atenção de média e alta complexidade a rede hospitalar pública é ponto de conflito e de superação com novos modelos de gestão. Diante desta realidade apresentada, este estudo tem como objetivo geral analisar o modelo de gestão da rede de atenção hospitalar no município de Petrolina. Este município é de referência regional como sede de região assistencial. Para apreensão do objeto de estudo utilizou-se como estratégia de pesquisa um estudo descritivo a partir dos dados e informação de documentos, e observação dos autores envolvidos na pesquisa. Em decorrência desta análise podemos observar que quanto ao porte dos estabelecimentos hospitalares em sua maioria são de pequeno porte e que existe um déficit de leitos hospitalares para atender o município e região. A organização da rede não tem como referência linhas de cuidados horizontal que privilegie a Atenção Básica. Existem serviços essenciais que não são ofertados no município, como neurologia e ortopedia de alta complexidade e alguns tratamentos em oncologia. Apesar da mudança já ocorrida do modelo de gestão hospitalar com a criação da Fundação Estatal e terceirização do HDM, não foi possível avaliar seus resultados em virtude do seu pouco tempo de funcionamento. Por fim, consideramos a importância da continuidade deste estudo na organização de uma rede interestadual unindo as regiões de Juazeiro-BA e Petrolina-PE visto a sua proximidade
Abstract
Health care and guaranteed universal and comprehensive social protection for the
population represent a challenge to public health systems all over the world. In Brazil,
the establishment of the SUS (Unified Health Care System) has brought about important
achievements for the population. However, a major management challenge to all three
spheres of government is the organization of basic attention based on the strategy of
family health care, as well as the substitution of the fragmentary medium and high
complexity service by a care model organized in network, where the family health care
units form points of distribution, and points of convergence that propitiate integrality.
For medium and high complexity care, the public hospital network has some points of
conflict and there is an attempt to overcome them with new management models.
Considering this reality, the present paper’s general purpose is to analyze the
management model for the public hospital care in the municipality of Petrolina,
Pernambuco state. This municipality is a regional reference and it is headquarters to
regional health care. As a research strategy to approach the purpose of the present paper,
a descriptive study was carried out, based on data and information gathered from
documents, as well as the authors’ personal observation. As a result of this analysis it
was possible to establish that most of the hospitals are of small size and that there is a
lack of hospital beds attending the municipality and the region. The organization of the
network is not based on a reference of horizontal care lines that favor Basic Health
Care. There are several essential services not offered in the municipality, as for instance
neurology and high complexity orthopedics, as well as some cancer treatments. In spite
of the change in the hospital management model that occurred with the creation of the
State Foundation and the outsourcing of HDM, it has not been possible to evaluate any
results due to the short period of operation. Finally, the paper considers the importance
of continuity for the present study for the organization of an interstate network uniting
the region of the two neighboring cities of Juazeiro, Bahia state, and Petrolina,
Pernambuco state.
