Estudos de estimativa dos custos de provisão do serviço de assistência à saúde dos portadores de diabetes e hipertensão no âmbito da média complexidade são escassos. Com o objetivo de analisar os custos diretos sanitários em uma unidade de referência para portadores de diabetes e hipertensão arterial, este estudo se constituiu em 3 etapas: 1- Levantamento da produção ambulatorial e adequação dos valores pagos pelo SUS; 2- Apuração dos custos diretos sanitários da unidade e; 3- Estimativa do custo médio por procedimento. Utilizou-se o banco de dados do Sistema de Informação Ambulatorial da Secretaria Estadual de Saúde, o Centro de Custo da Secretaria de Saúde do Recife e o sistema de dispensação de medicamentos do serviço de saúde. Para a estimativa do custo médio calculou-se o Custo de Funcionamento e de Pessoal por procedimento. Os procedimentos com finalidade diagnóstica representaram cerca de 57 por cento do total da produção da unidade, enquanto os cirúrgicos apenas 4 por cento da produção. Os grupos de despesas que apresentaram os maiores custos diretos foram medicamentos, pessoal e serviços de terceiros, os quais corresponderam a 36 por cento, 20 por cento e 18 por cento dos custos diretos sanitários, respectivamente. O custo médio do exame de eletrocardiograma e da consulta de nutrição apresentou as maiores diferenças quando comparados com o valor pago pelo SUS: 197 por cento e 83 por cento, respectivamente. O grupo de medicamentos representou maior parcela na composição dos custos diretos sanitários. Todos os procedimentos apresentaram valores estimados em R$ superiores aos valores pagos pelo SUS sendo o Custo de Pessoal o fator de maior relevância (AU)

Studies of estimated costs of provision of services to the health of people with
diabetes and hypertension within the average complexity are scarce. To assess the
direct health costs in a unit of reference for people with diabetes and hypertension.
The study consisted of 3 steps: removal of production and the adequacy of outpatient
amounts paid by SUS, determination of the direct costs of the health unit and
estimate the average cost per procedure. We used the database of the Information
System Outpatient Department of State Health, the Center for Cost of the Secretary
of Health of the reef system and the dispensing of medicines the health service. To
estimate the average cost is the estimated cost of operation and procedure for staff.
The procedures for diagnostic purposes represented about 57% of the total
production of the unit, while only 4% of surgical production. The groups of
expenditure that showed the highest direct costs were drugs, personnel and services
of third parties, which corresponded to 36%, 20% and 18% of direct health costs,
respectively. The average cost of the examination of electrocardiogram and
consultation of nutrition had the greatest differences when compared to the amount
paid by SUS: 197% and 83% respectively. The group of drugs accounted for largest
share in the composition of direct health costs. All procedures had estimated in R $
above the amounts paid by the SUS and the cost of staff the most relevant factor.
