Resumen en Portugués | No contexto do diagnóstico da Tuberculose, a agilidade na liberação do resultado do teste de sensibilidade é de extrema importância para que se inicie o tratamento imediato e adequado, melhorando as condições físicas e a qualidade de vida do paciente, aumentando as chances de cura e quebra da cadeia de transmissão. O objetivo foi analisar as funcionalidades do Sistema Gerenciador de Ambiente Laboratorial no diagnóstico da Tuberculose Resistente ás Drogas (TBDR), no Estado do Rio de Janeiro, no período de 2013 a 2015. Realizou-se um estudo descritivo transversal, com uso de dados secundários do Gerenciador de Ambiente Laboratorial (GAL) e do Sistema de Informação de Tratamentos Especiais da Tuberculose (SITE TB), possibilitando avaliar a completitude das variáveis do GAL e sua efetiva função como ferramenta para diagnóstico da tuberculose resistente. O tempo médio entre a data de processamento e liberação do teste para diagnóstico da TBDR foi 25 dias para teste de sensibilidade (Método das Proporções), 20 dias para teste de sensibilidade (MGIT) e 4 dias para Teste Rápido Molecular (Gene Xpert). Todavia, o tempo médio entre a liberação do exame e o início do tratamento foi de 88 dias ou 12,6 semanas. Considerando um intervalo de confiança de 95%, a análise do desempenho do Gene Xpert em comparação ao MGIT revelou sensibilidade de 90% (IC 95% 81,9-99,3) e especificidade de 96,6% (IC 95% 94,8-97,9). Atualmente, a utilização de testes rápido para o diagnóstico, incluindo o Gene Xpert® MTB/Rif, favorece a identificação de resistência medicamentosa em até duas horas, trazendo a possibilidade de início de tratamento em menor tempo. Concluímos que o Gene Xpert® MTB/Rif teve um bom desempenho na detecção do bacilo e na confirmação de resistência a rifampicina. Todavia, no período em estudo, o tempo médio entre a liberação do teste rápido molecular e o início do tratamento foi muito superior ao desejado. Considerando todas as técnicas de detecção, o tempo transcorrido entre a liberação do exame no GAL e o início do tratamento foi ainda pior e bem superior ao esperado. Foi possível identificar ainda limitações do GAL. A maior parte dos testes de sensibilidade, tanto os provenientes do método das proporções, quanto do MGIT, apresentaram tempo entre processamento-liberação do resultado inferior ao mínimo necessário, sendo considerados inválidos. A fim de deflagrar ações oportunas de vigilância em relação aos casos TBDR, diminuindo o tempo entre a liberação do resultado do exame e o início do tratamento, acreditamos que o GAL seria mais eficiente se fosse interligado ao SITE TB. | pt_BR |

Resumen en Inglés | In the context of the diagnosis of Tuberculosis, the agility in releasing the sensitivity test result is of extreme importance in order to initiate immediate and appropriate treatment, improving the patient's physical conditions and quality of life, increasing the chances of cure and breakage Of the chain of transmission. The objective was to analyze the functionalities of the Laboratory Management System in the diagnosis of Tuberculosis Resistant to Drugs (TBDR), in the State of Rio de Janeiro, from 2013 to 2015. A descriptive cross-sectional study was carried out using secondary data The Laboratory Environment Manager (GAL) and the TB Treatment Information System (SITE TB), making it possible to evaluate the completeness of the LAG variables and its effective function as a tool for the diagnosis of resistant tuberculosis. The mean time between the processing and release date of the TBDR diagnostic test was 25 days for sensitivity test (Proportion Method), 20 days for sensitivity test (MGIT) and 4 days for Rapid Molecular Test (Gene Xpert). However, the mean time between the release of the test and the start of treatment was 88 days or 12.6 weeks. Considering a 95% confidence interval, analysis of Gene Xpert performance in comparison to MGIT revealed 90% sensitivity (95% CI 81.9-99.3) and specificity of 96.6% (95% CI 94, 8- 97.9). The use of rapid diagnostic tests, including Gene Xpert® MTB / Rif, favors the identification of drug resistance in up to two hours, bringing the possibility of starting treatment in a shorter time. We concluded that Gene Xpert ® MTB / Rif performed well in detecting bacillus and confirming resistance to rifampicin. However, during the study period the mean time between the release of the molecular rapid test and the start of treatment was much higher than desired. Considering all the techniques of detection, the time elapsed between the release of the examination in the LAG and the beginning of the treatment was even worse and much higher than expected. It was also possible to identify limitations of the LAG. Most of the sensitivity tests, both from the proportions method and from the MGIT, showed time between processing-release of the result below the minimum necessary, and were considered invalid. In order to trigger timely surveillance actions for TBDR cases, reducing the time between the release of the test result and the start of treatment, we believe that the LAG would be more efficient if it were linked to the TB SITE. | pt_BR |
