O objetivo deste estudo foi investigar a síndrome da lipodistrofia (SLD) em crianças e adolescentes infectados com HIV tratados com Highly Active Antiretroviral Therapy-HAART (terapia antiretroviral de alta potência). Foram estudadas 27 crianças e 19 adolescentes, em uso de HAART, atendidas no ambulatório de Doenças Infecciosas em Pediatria do Instituto Fernandes Figueira/Fundação Oswaldo Cruz. Foram realizadas dosagens de colesteral total (Col-T), lipoproteína de alta densidade (HDL-c), lipoproteína de baixa densidade(LDL-c), triglicerídeos(TG), teste de tolerância à glicose (TTOG) e avaliados a alteração de distribuição de gordura corporal e estado nutricional (EN).O estado nutricional foi avaliado por parâmetro antropométrico de acordo com o critério da WHO, 1995.Foram investigados os fatores de risco predisponentes para a dislipidemia: consumo alimentar pelo questionário de freqüência semiquantitativa, história familiar de risco cardiovascular e pratica de atividade física.O nível de significância estatística foi o p-valor igual 0,05. Os resultados mostraram maior prevalência de alteração lipídica do que de conformação corporal e maior alteração nos níveis de HDL-c e TG nas crianças e HDL-c nos adolescentes. O EN foi adequado na maioria da população do estudo.A carga viral, tempo de exposição e o tipo de HAART não foram estatisticamente significantes em relação aos lipídos séricos. O TTOG foi normal em toda população estudada. As alterações metabólicas e a alteração de distribuição de gordura corporal foram mais encontradas naqueles com categoria clínica moderada e grave e independente da categoria imunológica. A maior ingestão de gordura saturada e colesteral foi estatisticamente significante para os níveis de LDL-c que foi mais elevado nos adolescentes. A prática de atividade física foi estatisticamente significante quando associada ao nível de HDL-c nos adolescentes com níveis alterados para os mais sedentários. O estudo mostrou que crianças e adolescentes apresentaram dislipidemia e alteração da distribuição da gordura corporal; por isso o acompanhamento dessas alterações é necessário, devido as suas consequências na qualidade de vida e sobrevida destes pacientes.
