Resumo
As pessoas infectadas pelo vírus linfotrópico de células T humana do tipo 1 (HTLV-1) apresentam hiporreatMdade e anergia ao teste tuberculínico (PPD), e têm maior prevalência de tuberculose do que a população em geral. Tais fatos conduzem à hipótese de que haja uma associação de risco entre o HTLV-1 e a tuberculose, o que até o momento n foi esclarecido. Avaliar se a infecção pelo HTLV-1 é um fator de risco para o aumento da incidência de tuberculose. Estudo caso controle realizado no período de setembro de 2001 a outubro de 2002, com 753 indivíduos moradores da cidade do Salvador, sendo 375 do grupo de pacientes com tuberculose e 378 do grupo controle, sem tuberculose, selecionados por freqüência de idade e sexo. Todos foram submetidos ao teste de triagem através do ELISA; o Western blot foi utilizado para confirmação. Os pacientes soropositivos para o HIV foram excluídos. A prevalência do HTLV-1 no grupo de pacientes com tuberculose foi de 4,27% (16/375) e nos indivíduos do grupo controle de 1,32% (5/378). No presente estudo, a razão de chance (OR) foi de 3,32 e o intervalo de confiança (IC) de 1,15 — 11,7; quando ajustada, a OR foi de 3,01 e o IC de 1,06 — 8,58. O Risco atribuível populacional neste estudo foi de 2,99%. O presente estudo encontrou uma associação significativa entre o HTLV-1 e a tuberculose, a qual sugere fortemente que o HTLV-1 seja um fator de risco para o aumento da incidência de tuberculose.

Individuals infected with human T cell lymphotropic virus, type I (HTLV-1) present hyporeactivity and anergy to the tuberculin purified protein derivative skin testing (PPD), as well as an increased prevalence of tuberculosis raising hypotheses of a risk association between HTLV-1 and tuberculosis, which has not been clarified up to this point. OBJECTIVES: To evaluate if infection by HTLV-1 could be a risk factor for increased incidence of tuberculosis. MATERIAL AND METHODS: case- control study carried out September 2001 to October 2002, with 753 subjects from the city of Salvador. 375 of the subjects were in the group of patients with tuberculosis and 378 in the control group, paired by age and gender. Plasma were screened for HTLV-1 antibodies by ELISA and the positive sample were tested by a confirmative assay (Western blot). HIV-1 Infected individuals were excluded. RESULTS: the prevalence of HTLV-1 in tuberculosis patients was 4.27% (16/375) and In the control group 1.32% (5/378). One person in each group was not included in this calculation due to seropositivity to HTLV-2. The odds ratio (OR) was 3.32 and the confidence interval (Cl) 1,15 - 11,7. When adjusted for income, the OR was 3.01 and Cl 1,06 - 8,58. The attributed risk in this study was 2.99%. CONCLUSION: Our present results showed an association with HTLV-1 infection and tuberculosis. The HTLV-1 could be a risk factor to increased incidence of tuberculosis.
