O presente estudo teve por objetivo avaliar a acurácia de indicadores antropométricos de adiposidade na predição de componentes da síndrome metabólica (SM) segundo a influência da proporção individual da ancestralidade genômica africana. O estudo foi conduzido entre participantes da linha de base da coorte de idosos de Bambuí. Para a determinação do melhor ponto de corte da circunferência da cintura, índice de massa corporal, razão cintura quadril e razão cintura estatura segundo a proporção individual de ancestralidade genômica africana e sexo foi utilizado o Índice de Youden. A acurácia foi avaliada por meio da área sob a curva ROC (AUROC), da sensibilidade e da especificidade, além dos valores preditivos positivo e negativo. Os resultados do estudo mostraram que entre os 1383 participantes, a prevalência da SM foi igual a 63,8% entre as mulheres, e semelhante entre os grupos de baixa, intermediária e alta proporção individual ancestralidade genômica africana. Entre os homens, 43,4%, e mais alta entre os indivíduos com baixa proporção individual de ancestralidade africana, 50,3%. Na população geral, no sexo masculino o ponto de corte da circunferência da cintura que melhor prediz 2 componentes da SM (88 cm) foi semelhante àquele recomendado pelo JIS (90 cm), com AUROC = 0,673 e 0,672, respectivamente. Entre as mulheres, o valor correspondente foi mais alto (92 cm) em relação ao recomendado pelo JIS (80 cm), com AUROC = 0,605 e 0,560, respectivamente. Na análise estratificada pela proporção individual de ancestralidade genômica africana, os pontos de corte da circunferência da cintura foram semelhantes entre os grupos de proporção individual da ancestralidade genômica africana, em ambos os sexos, 90 cm para o sexo masculino e 80 cm para o sexo feminino. Foi observado AUROC = 0,839, 0,830 e 0,885 para os grupos de baixa, intermediária e alta proporção individual de ancestralidade genômica africana, respectivamente, no sexo masculino. Os valores correspondentes no sexo feminino foram 0,674, 0,700 e 0,718. Para os índices antropométricos de adiposidade, para o índice de massa corporal no sexo masculino foi observado AUROC = 0,846, 0,800 e 0,805, para os grupos de baixa, intermediária e alta proporção individual de ancestralidade genômica africana. Os valores correspondentes para a razão cintura estatura, e razão cintura quadril foram 0,828, 0,824 e 0,757, e 0,796, 0,715 e 0,785, respectivamente. Os pontos de corte identificados são mais específicos e mostraram pequena variação entre os grupos. O oposto foi observado no sexo feminino. Entre as mulheres, para os três índices nos diferentes grupos, o Índice de Youden e área sob a curva ROC foram mais baixos em comparação ao sexo masculino, entretanto, os pontos de corte foram mais sensíveis. A validade global da circunferência da cintura na discriminação de componentes da Síndrome Metabólica entre idosos foi baixa, e no sexo feminino foi encontrado diferença no ponto de corte em comparação àquele recomendado pelo Joint Interim Statement. Considerando-se a influência da proporção individual da ancestralidade genômica africana, tanto para homens idosos quanto para mulheres idosas os pontos de corte observados para a circunferência da cintura e índices antropométricos de adiposidade são semelhantes entre os grupos, e a validade da medida para a predição de outros componentes da síndrome, foi intermediária. Os pontos de corte identificados para a circunferência da cintura são iguais ao preconizado pelo Joint Interim Statement para adultos.
Abstract
The present study aimed to evaluate the accuracy of anthropometric indicators of adiposity in predicting metabolic syndrome (MS) components according to the influence of the individual proportion of African genomic ancestry. The study was conducted among participants from the baseline of the Bambuí Cohort Study of Aging.The best cut-off point of the waist circumference, body mass index, waist height ratio , waist hip ratio second to the individual proportion of African genomic ancestry and gender for the endpoint was assessed by the Younden Index. The accuracy of those cut-off points was assessed by the area under the ROC curve (AUROC), the sensitivity, the specificity and the positive and negative predictive values were used. The study results showed that among the 1383 study participants, the prevalence of MS was 63,8% among women, and similar among the low, intermediate and high proportion individuals with African ancestry. Among men, 43,4%, and highest among individuals with low individual proportion of African ancestry, 50,3%. In the general population, in males the cutoff point of waist circumference that best predicts 2 components of SM ( 88 cm) was similar to that recommended by JIS (90 cm), with AUROC = 0,673 and 0,672, respectively. Among women, the corresponding value was higher ( 92 cm) than recommended by JIS ( 80 cm), with AUROC = 0,605 and 0,560, respectively. In stratified analysis for individual proportion of African genetic ancestry, the cutting points waist circumference were similar between the individual proportion of groups of African genetic ancestry, in both sexes, 90 cm for men and 80 cm for females. It was observed AUROC = 0,839, 0,830 and 0,885 for low groups., intermediate and high individual proportion of African genetic ancestry, respectively, in males. The corresponding values in females were 0,674, 0,700 and 0,718. For anthropometric indices of adiposity, in males, AUROC = 0,846, 0,800 and 0,805 were observed for the low, intermediate and high individual proportion of African genomic ancestry groups. The corresponding values for waist height ratio, and waist hip ratio were 0,828, 0,824 and 0,757, and 0,796, 0,715 and 0,785, respectively. The cutoff points identified are more specific and showed little variation between groups. The opposite was observed in females. Among women, for the three indices in the different groups, Youden and area under the ROC curve were lower compared to males, however, the cutoffs were more sensitive. Although the overall validity of waist circumference in discriminating Metabolic Syndrome components among the elderly was low, and in females a difference was found in the cutoff compared to that recommended by the Joint Interim Statement. Considering the influence of the individual proportion of African genomic ancestry, for both older men and older women, the cutoffs observed for waist circumference and anthropometric indices of adiposity are similar between groups, and the validity of the measurement for prediction. of other components of the syndrome was intermediate.The cutoff points identified for waist circumference are the same as the Joint Interim Statement for adults.
