A prevalência da doença renal crônica tem crescido e o acesso ao transplante renal ainda é um
grande problema no Brasil, apesar do sistema de saúde público universal. Este trabalho visou
estudar uma iniciativa voltada para a melhoria da qualidade do cuidado ao paciente renal
crônico. Partiu da premissa que o conhecimento aprofundado de como uma intervenção
funciona, e sob que condições contextuais, é fundamental para que se construa conhecimento
na área da melhoria da qualidade do cuidado de saúde e se aponte caminhos para a
disseminação de intervenções bem-sucedidas. Buscou descrever a iniciativa a partir da
identificação dos seus componentes ativos e mecanismos de mudança envolvidos, o contexto
da sua implementação e os seus efeitos. A iniciativa foi implementada em um centro
transplantador de Minas Gerais em 2012, sendo o estudo desenvolvido retrospectivo, com um
componente qualitativo e outro quantitativo. Para a identificação dos mecanismos de mudança
e fatores contextuais envolvidos, foi realizada análise temática de entrevistas com os
implementadores da iniciativa e demais profissionais da organização onde se insere o centro
transplantador. Para observar os efeitos da intervenção foram realizadas análises gráficas de
séries temporais de indicadores nos cinco anos anteriores e posteriores à intervenção. Foi
observado que após a implementação da iniciativa obteve-se ampliação do acesso ao
transplante renal e maior efetividade do tratamento. Os mecanismos de mudança identificados
foram a articulação entre diferentes serviços, a racionalidade para a condução de melhores
práticas e a adoção do melhor resultado para o paciente como um valor. Diferentes fatores
contextuais foram identificados como cruciais à implementação, interferindo como
facilitadores ou barreiras, influenciados ou não por um cenário mais geral de busca do
hospital pela acreditação. As lições aprendidas podem propiciar tanto a disseminação de
iniciativas semelhantes de modo factível e sustentável, como subsídios para intervenções mais
gerais. Merece destaque a constatação de que mudanças positivas significativas podem ser
promovidas em microssistemas do cuidado inseridos em ambientes menos favoráveis.
Resumen en ingles
The predominance of the renal chronic disease has been growing, being still the access to the
renal transplant a great problem in Brazil, in spite of the universal public system of health.
This work aimed to study an initiative turned to the improvement of the equity of the access
to the renal transplant and effectiveness of the treatment. It came from the premise that the
deepened knowledge of as an intervention works, and under which context conditions, it is
fundamental so that knowledge is built in the area of the improvement of the quality of the
care of health and if it points to ways for the dissemination of successful interventions. It
looked to identify the active beginnings and mechanisms of change wrapped in the initiative,
the context of his implementation and his effects. The initiative was implemented in a
transplant center of Minas Gerais in December of 2012, being the developed retrospective
study, with a quantitative qualitative and the other one quantitative. For the identification of
the mechanisms of change and factors context wrapped, thematic analysis of interviews was
carried out with the implementing of the initiative and too professional of the organization
where it is inserted the center transplant. To observe the effects of the intervention were
carried out time series analysis of indicators with graphical presentation of data in five
previous and subsequent years to the intervention. It was noticed that after the implementation
of the initiative enlargement of the access was obtained to the renal transplant and bigger
effectiveness of the treatment. The mechanisms of change identified went to articulation
between different services, the rationality for the driving of better practices and the adoption
of the best result for the patient like a value. Different context factors were identified how
crucial to the implementation, interfering how facilitators or barriers, when they were
influenced or not for a scenery more general of search of the hospital it shears trust. Lessons
learnt can favor both the dissemination of similar initiatives of possible and sustainable way,
and subsidies for more general interventions. It deserves distinction to observation of what
positive significant changes they can be promoted in micro systems of the care inserted in less
favorable more spacious environments.
