Este estudo teve o propósito de investigar os possíveis efeitos adversos
associados à gravidez na adolescência no Município do Rio de Janeiro - MRJ.
A tese foi apresentada sob a forma de artigos. No primeiro deles,
observou-se que entre 1980 e 1995, ao contrário das taxas de fecundidade que
vêm diminuindo nas mulheres com 20 anos e mais, houve um crescimento das
taxas de fecundidade nas adolescentes, em especial no grupo de 10 a 14 anos.
Em uma amostra de nascimentos provenientes do SINASC/RJ, entre 1996 e
1998, os fatores determinantes do baixo peso ao nascer (BPN) foram
comparados em dois grupos de mães, categorizados por idade de 15 a 19 e de
20 a 24 anos. Foram estimadas as associações entre as variáveis pela razão
dos produtos cruzados - Odds Ratio (OR) e respectivos intervalos de confiança.
Utilizou-se também procedimentos de regressão logística. A proporção de BPN
foi significativamente maior entre as mães adolescentes do que nas de 20-24
anos. O pré-natal não foi realizado em 13% das adolescentes, enquanto 10%
do outro grupo não recebeu atendimento. Quando realizado o pré-natal, as
adolescentes tiveram menos consultas. As mais velhas apresentaram melhor
grau de instrução, em comparação às mais jovens. Entre as adolescentes, o
percentual de recém-nascidos prematuros foi significativamente maior que no
outro grupo. Diferenças foram observadas por tipo de maternidade
(públicas/privadas), com predomínio de uso das públicas pelas adolescentes. A
análise de regressão logística mostrou que existe um efeito da idade materna
na explicação do BPN, mesmo quando controlado pelas outras variáveis.
No segundo artigo, são comparadas as características socioeconômicas,
a assistência pré-natal e o estilo de vida de três grupos de puérperas, um
composto por adolescentes (<20 anos) e os demais por mulheres de 20-34
anos, categorizadas segundo experiência (ou não) de gravidez na
adolescência. Foram entrevistadas 3508 puérperas no pós-parto em
maternidades municipais e federais do MRJ. A análise estatística consistiu em
utilizar testes qui-quadrado (χ2
) para testar hipóteses de homogeneidade de
proporções. Ao comparar os três grupos, observou-se uma situação mais
desfavorável entre as mães de 20-34 anos com história de gravidez na
adolescência. Estas têm pior nível de instrução, apresentam com maior
freqüência hábitos de fumo e uso de drogas ilícitas durante a gestação e
realizam menor número de consultas de atendimento pré-natal.
Na última parte do estudo, apresentada no terceiro artigo, objetivou-se
caracterizar o perfil das gestantes com atendimento pré-natal precário,
segundo variáveis sociodemográficas, história reprodutiva da mãe, apoio
familiar, satisfação com a gestação e comportamentos de risco durante a
gravidez. Foram entrevistadas 1801 adolescentes no pós-parto imediato de
maternidades públicas e conveniadas com o SUS no MRJ. A variável
dependente foi o número de consultas no pré-natal (0-3; 4-6; 7 e mais
consultas). A análise estatística foi dirigida a testar a hipótese de
homogeneidade de proporções mediante análises bi e multivariada, com o uso
de regressão logística múltipla. Foram encontradas maiores proporções de
mães com número insuficiente de consultas (0-3), estatisticamente
significativas, nos grupos de mães: de baixo nível de instrução; residentes em
domicílios sem água encanada; que não vivem com o pai do bebê; com
nascidos vivos anteriores; que não ficaram satisfeitas com a gestação; que não
tiveram apoio do pai do bebê; que tentaram interromper a gestação e as que
fumaram, beberam e/ou usaram drogas durante a gestação.
A assistência pré-natal se apresentou neste estudo como uma política
compensatória eficiente para a prevenção da prematuridade e do BPN,
sobretudo entre as puérperas adolescentes. Aponta-se que as mães com
piores condições de vida e comportamentos de risco na gravidez foram as que
mais ficaram à margem da assistência pré-natal.
Diante do papel relevante do atendimento pré-natal, é sugerida
fortemente a ampliação da sua cobertura efetiva em todos os grupos sociais,
sobretudo, aqueles marginalizados pela família e/ou sociedade. Cabe ressaltar
a importância dos serviços de saúde na captação dessas gestantes através de
programas de busca ativa, enfatizando o papel fundamental dos profissionais
que atuam nos Programas de Saúde da Família - PSF, ainda que sua atuação
esteja restrita a poucas comunidades do Município do Rio de Janeiro.
Abstract
The study aimed at the investigation the possible adverse effects
associated to pregnancy in adolescence in the Rio de Janeiro County – RJ.
The dissertation was presented in three scientific papers. In the first of
them, the temporal analysis of the fertility rates from 1980 to 1995 showed an
increase among adolescents (10-19 years old), especially in the age group from
10 to 14, in contrast with a decrease in the fecundity rates among nonadolescent mothers. Following this argument, a stratified sample of live births
from the Information System of Live Births in the Municipality of Rio de Janeiro
from 1996 to 1998 was drawn. The risk factors of low birth weight (LBW) were
compared in two groups of mothers categorised by age group (15-19 and 20-24
years old). For the statistical analysis, odd ratios and correspondent confidence
intervals were estimated, besides logistic regression procedures for the
multivariate analysis. The LBW was significantly greater among adolescent
mothers than among mothers aged 20-24. Prenatal care has not been attended
by 13% of adolescents. A lower proportion (10%) was found among the 20-24
year-old group. Further, when the adolescents received prenatal care, they had
fewer appointments. Also, the group of 20-24 years old presented a higher level
of education. In relation to the adverse effects on the baby, among the
adolescents the proportion of premature babies was significantly higher than in
the other group. As for the public and private maternity hospitals, the
adolescents attended the public hospitals more often than the older mothers.
The logistic regression analysis showed a significant effect of the mother age on
LBW even when controlled for the other variables considered in the study.
In the second paper, the socio-economic characteristics, prenatal care as well
as the lifestyle were compared in the following three groups of puerperia
women: one composed of adolescents (10-19 years old) and the others of 20-
34-year-old women, classified according to past experience of pregnancy during
adolescence. A sample of 3,508 parturient women, interviewed just after
delivery, was selected from public hospitals in the City of Rio de Janeiro, Brazil.
To verify the hypothesis of homogeneity of proportions, chi-square tests (χ2
)
were used. Comparing the three groups, the most disadvantaged conditions
were found among the 20-34-year-old mothers having a history of pregnancy in
adolescence. Those women showed the lowest level of schooling, more often
habits of smoking and use of illegal drugs during pregnancy and the lowest
number of prenatal care appointments.
In the last part of this study, presented in the third paper, the profile of pregnant
women having precarious prenatal assistance was characterized according to:
socio-demographical variables; mother’s reproductive history; family support;
fulfilment with pregnancy; as well as risky behaviour during pregnancy. A
thousand eight hundred and one adolescents were interviewed right after
delivery in public maternity hospitals (or when delivery was paid by the Public
Health System (SUS) in the City of Rio de Janeiro). The dependent variable
was the number of antenatal appointments (0-3; 4-6; 7 and more). The
statistical analysis focused on testing the hypothesis of homogeneity of
proportions, including bi and multivariate analysis by multiple logistic regression.
Significantly higher proportions of insufficient number of appointments (0-3)
were found in the following groups: low educational level mothers; women that
live in precarious sanitary conditions; women that do not live with the child’s
father; mothers that have tried to interrupt pregnancy; and the ones who have
smoked, drank and/or used drugs during pregnancy.
The results strongly suggest that prenatal care is an effective
compensatory policy to prevent prematurity and LBW, particularly among
adolescent mothers. The study highlights the fact that the mothers having the
worst living conditions as well as the most risky behaviours were the ones that
showed no attendance to prenatal care.
The findings supports the recommendation that prenatal care be
provided for all social groups, specially for those women that are marginalized
from the family and/or society. It is worthy to note the importance of public
health policies focused on active surveillance of pregnant women that live in
deprived conditions. In this context, it is emphasised the fundamental role of the
professionals working for Family Health Programmes (FHP), despite of the fact
that the program is available only in a few low-income communities in the City
of Rio de Janeiro.
