Estudou-sea prática psiquiátrica na Bahia, com ênfase no asilo S. Joäo de Deus/Hospital Juliano Moreira, de 1874 a 1947, com base no referencial da psiquiatria como prática contraditória entre as finalidades de tratamento e controle social. Descreve as crises e reformas do sistema psiquiátrico na Bahia e identifica os sujeitos relevantes. Compreende como o tecno-saber psiquiátrico foi usado para caucionar práticas de violência institucional. Descreve e analisa as condiçöes de vida dos internados. Foram utilizados procedimentos da pesquisa histórica na análise documental dos testemunhos voluntários e involuntários; a organizaçäo das fontes e interpretaçäo discursiva dos internados, agentes intelectuais e subalternos. Na primeira parte foi construído um referencial analítico para a psiquiatria manicomial baiana. Constata uma história quase pendular dos movimentos psiquiátricos. O manicômio, apesar de sucessivas reformas, passou a ser questionado. Na segunda parte, foram analisados os cuidados dispensados aos loucos na Santa Casa de Misericórdia. Os saberes e técnicas psiquiátricos de entäo assimilavam propostas do alienismo francês e do organicismo alemäo. As condiçöes de vida e saúde dos alienados foram analisadas, comparando-se dados de mortalidade com os da situaçäo financeira do asilo. Destaca o estudo de Nina Rodrigues sobre os surtos de beribéri no asilo, suspeitando da etiologia nutricional para a doença. Na terceira e última parte estuda o hospício sob a tutela do Estado. De 1912 a 1930, apesar da construçäo de 9 pavilhöes, o asilo permanecia superlotado. De 1931 a 1947, predominantemente autoritário, a crise financeira repercutiu no hospital. Os diretores e demais agentes intelectuais tinham formaçäo organicista e eram apologéticos da higiene mental. A noçäo de degeneraçäo serviu para caucionar práticas moralistas, xenófabas e machistas, destaca a impaludaçäo cerebral, insulino, malario e convulsoterapia química e, principalmente, eletrochoqueterapia. Em todo o período estudado, o manicômio foi o elemento central do sistema de cuidado psiquiátrico da Bahia.

The psychiatric practice in the State of Bahia was studied, focusing on the São João de Deus Asylum/Juliano Moreira Hospital. Psychiatry landmarks were taken as contradictory practices which oscillate between treatment and social control. Objectives were: a) To describe crisis and reforms of the psychiatric system in Bahia, identifying relevant subjects; b) To understand how psychiatric techno-knowledge has been used to support practices of institutional violence; c) To describe and to analyse the living conditions of the patients. Historical research procedures were
used for documental analyses of voluntary and involuntary testimonies: source organization (reports, official letters, surveys) and discourse interpretation of the interns, intelectual agents and their subordinates. In the first part, an analytical referential for Bahian madhouse psychiatry
was developed. Along this period, psychiatric movements alternated from indoors reforms to external, expansionist movements. The madhouse, central element of the psychiatric strategy, despite its consecutive reforms, became to be questioned. Three different conjunctures of the psychiatric practice in Bahia were analysed, structural processes and relevant specific elements being identified. In the second part, the analysis focused on the care provided to mentally diseased patients at the Santa Casa de Misericórdia Hospital. By this time, psychiatric knowledge and practices assimilated proposals from the French alienism and from the German organicism. The living and health conditions of the patients were analysed, by comparing mortality data with those from the asylum financial situation. Dr. Nina Rodrigues study, which arose the hipothesis of a nutritional etiology for the asylum outbreaks of béri-béri, is emphasized. The third and last part studied the asylum in the State ward. From 1912 to 1930,
inspite of nine pavilions had been built, the asylum remained overcrowed. During the period from 1931 to 1945, predominantly authoritarian, the severe financial crisis repercuted on the hospital life. The asylum director and other intelectual agents had organicist backgrounds and were apologetic of the Mental Hygine. The notion of degeneration gave support to moralist, xenophobic and machist practices, like cerebral impaludation, insulintherapy, malariotherapy and chemical convulsotherapy and, mainly, electrical shock therapy. Several forms of violence against the patient were identified. Along all the studied period, the asylum remained as the central element in the psychiatric care system in the State of Bahia.
