Introduction: In Uruguay the first clinical case of AIDS was detected in 1983 and the first autochthonous case in 1986. Despite efforts and public policies, approximately 1000 new infections are registered per year and the mortality rate has not shown the expected decrease as a result of the policy of universal access to antiretroviral treatment. The objective of this study was to analyze the time between the time of HIV diagnosis and the presence of undetectable viral load and related aspects in the cascade of continuous HIV care in patients diagnosed in 2018. Material and methods: um was carried out retrospective cohort study with 240 people diagnosed with HIV in 2018 in Uruguay. A 12-month follow-up was performed and the time between diagnosis and the start of ART was analyzed; between diagnosis and achievement of undetectable viral load. For the analysis, the free software R version 3.0.3, and Epi-Info 7.2 were used. The opportunity indicator was calculated, the care cascade was constructed and the survival curves were elaborated for each variable and subsequently a COX regression analysis was carried out. The ethical aspects were considered. Results: Of the 240 participants, 54.6% corresponded to the male population, 72% to people between 18 and 44 years old. 50% made their first ART withdrawal in pharmacies at 74 days and more. When analyzing the time to achieve undetectable load among those who withdraw ART, 50% achieved this value at 247 days. When analyzing the pillars of the care cascade, a higher percentage of ART withdrawals was observed among the female population, residents of the country's capital and in the group aged 42 to 71 years. However, the highest percentage of people with undetectable viral load was observed in women, in residents of the interior, and the youngest located in the range of 18 to 28 years, and those caught late. In the COX model considering the most clinically relevant covariates, the number of \201Ccontacts with the system\201D revealed the inverse association, patients with 21 to 30 contacts in 12 months showed a negative rate of 81% lower than patients with 20 or less contacts, controlling for the other variables. Conclusion: The cascade of the continuum of HIV care is a tool for monitoring continuity of care and its use contributes to improving the management of services and care processes to guarantee continuity. The low percentage of undetectable viral load in relation to the total that started treatment in Uruguay, substantially translates aspects related to scarce controls carried out by the Ministry of Health in 2018 and 2019.

Introducción: En Uruguay el primer caso clínico de sida se detectó en 1983 y el primer caso autóctono en 1986. A pesar de los esfuerzos y políticas públicas, se registran aproximadamente 1000 infecciones nuevas por año y la tasa de mortalidad no ha mostrado el descenso esperado como resultado de la política de acceso universal al tratamiento antirretroviral. El objetivo de este estudio fue analizar el tiempo entre el momento del diagnóstico de VIH y la presencia de carga viral indetectable y aspectos relacionados en la cascada de la atención continua del VIH en pacientes diagnosticados en el año 2018. Material y métodos: Se realizó um estudio de cohorte retrospectiva con 240 personas diagnosticadas de VIH en el año 2018 en Uruguay. Se realizó un seguimento por 12 meses y se analizó el tiempo entre el diagnostico y el inicio de TARV; entre el diagnostico y el logro de carga viral indetectable. Para el analisis se utilizo el software libre R version 3.0.3, y Epi-Info 7.2. Se calculó el indicador oportunidad, se construyó la cascada de atención y se elaboró las curvas de sobrevida para cada variable y posteriormente se realizo un analisís de regresión de COX. Se contemplaron los aspectos éticos. Resultados: De los 240 participantes, el 54.6% correspondía a población masculina, el 72% a personas entre 18 y 44 años. El 50% realizó su primer retiro de TARV en farmacias a los 74 días y mas. Al analizar el tiempo para lograr carga indetectable entre los que retiran TARV, el 50% logró este valor a los 247días. Al analizar los pilares de la cascada de atencion, se observo um mayor porcentaje de retiros de TARV entre la población femenina, residentes de la capital del país y en el grupo de 42 a 71 años. Sin embargo el mayor porcentaje de personas com carga viral indetectable se observó en mujeres, em residentes del interior, y los mas jovenes ubicados en el rango de 18 a 28 años, y los captados tardiamente. En el modelo de COX considerando las covariables de mayor relevancia clínica, el número de \201Ccontactos con el sistema\201D reveló la asociación inversa, los pacientes con 21 a 30 contactos en 12 meses mostraron una velocidad de negativización de 81 % menor que los pacientes con 20 o menos contactos , controlando para las otras variables. Conclusión: La cascada del continuo de la atención del VIH constituye una herramienta para el monitoreo de la continuidad asistencial y su utilización contribuye a mejorar la gestión de los servicios y los procesos de atención para garantizar la continuidade. El bajo porcentaje de carga viral indetectable en relación al total que inició tratamiento en Uruguay, traduce sustancialmente aspectos vinculados a escasos controles realizados por el Ministerio de Salud em 2018 y 2019.
