Este trabalho teve como objetivo analisar a governança da Atenção Primária, com foco nos aspectos administrativos e relacionais do processo de contratualização, envolvendo a Secretaria Municipal de Saúde e as Organizações Sociais. Buscou-se compreender o contexto de implantação do Programa Saúde Presente e das OS; as estruturas de comando e coordenação; o controle, prestação de contas e sistema de incentivos; o processo de decisão, pactuação e participação; além das possibilidades e limites para a atuação das OS. Utilizando-se das contribuições do neoinstitucionalismo como referencial metodológico, trata-se de uma pesquisa exploratória, que se caracteriza como um estudo de caso da governança da APS, que além da revisão bibliográfica, se utilizou da análise de documentos e de entrevistas com representantes institucionais do Nível Central e de cinco CAP da SMS e cinco OS diretamente envolvidos na contratualização da APS no período de 2014 a 2016. Inicialmente, identificou-se que a baixa cobertura da ESF e os limites impostos pela LRF foram justificativas apresentadas pela SMS para a contratualização e que o Programa Saúde Presente foi a estratégia utilizada para reformar a APS, consistindo na reorganização do sistema de saúde municipal, com ênfase na ESF. Pôde-se identificar que a SUBPAV da SMS, possui uma estrutura robusta, reproduzida pelas CAP. Essas, por sua vez, organizam o processo de trabalho de forma diferente em cada AP. O mesmo é percebido de forma mais acentuada nas OS, que possuem estruturas gerenciais distintas. Os mecanismos de controle, prestação de contas e sistema de incentivo, na prática, eram desenvolvidos tal qual a regra contratual, além disso, a reunião pré-CTA, não prevista formalmente, foi um achado da pesquisa. Percebeu-se uma discreta atuação do Conselho Municipal de Saúde e uma atuação mais presente dos Conselhos Distritais. O fomento para a realização dos Colegiados Gestores Locais variou entre as CAP e OS. A ampliação da cobertura da ESF foi a principal potencialidade identificada pelos entrevistados e o principal limite estava relacionado à indefinição das funções e responsabilidades entre CAP e OS.

This study aimed to analyze the governance of Primary Care, focusing on the administrative and relational aspects of the contract process, involving the Municipal Health Secretariat and Social Organizations. We sought to understand the context of implementation of the Present Health Program and OS; the command and coordination structures; control, accountability and incentive system; the process of decision, agreement and participation; beyond the possibilities and limits for the action of OS. Using the contributions of neoinstitutionalism as a methodological reference, it is an exploratory research, which is characterized as a case study of PHC governance, which, in addition to the bibliographic review, used document analysis and interviews with institutional representatives of the Central Level and of five CAPs of the SMS and five OSs directly involved in the contracting of the PHC in the period from 2014 to 2016. Initially, it was identified that the low coverage of the FHT and the limits imposed by the LRF were justifications presented by the SMS for the contract and that the Health Present Program was the strategy used to reform PHC, consisting of the reorganization of the municipal health system, with emphasis on FHS. It was possible to identify that the SMS SUBPAV has a robust structure, reproduced by CAP. These, in turn, organize the work process differently in each AP. The same is perceived more sharply in OSs, which have different managerial structures. The mechanisms of control, accountability, and incentive system were, in practice, developed as the contractual rule, in addition, the pre-CTA meeting, not formally foreseen, was a finding of the research. It was noticed a discrete performance of the Municipal Health Council and a more present action of the District Councils. The promotion of Local Management Colleges varied between CAP and OS. The expansion of ESF coverage was the main potential identified by the interviewees and the main limit was related to the lack of definition of roles and responsibilities between CAP and OS.
