O HTLV, sigla em inglês para o vírus que infecta células T humanas (Human T-cell Lymphotropic Virus), é um retrovírus que afeta as células de defesa do organismo, os linfócitos T. Sua forma de transmissão é por via hematogênica, sexual ou vertical. A maioria das pessoas infectadas são portadores assintomáticos, porém algumas podem vir a desenvolver quadros neurológicos degenerativos graves como a paraparesia espástica tropical, além de leucemia e linfoma. O Brasil é considerado o país como o maior número absoluto de indivíduos infectados, com estimativa de prevalência de até 2,5 milhões de soropositivos. Estudos realizados com doadores de sangue mostram que a infecção pelo HTLV está presente em todo país, ainda que de forma heterogênea. Nesta pesquisa buscamos conhecer e analisar o processo de formulação de agenda e implementação de políticas e ações referentes ao HTLV nos estados da Bahia e Minas Gerais, dois estados pioneiros no desenvolvimento de políticas públicas voltadas para pessoas infectadas pelo vírus. Utilizamos o Modelo Múltiplos Fluxos para compreensão de como e por que a temática foi inserida na agenda governamental dando origem a políticas públicas referentes ao HTLV nesses estados. Constatamos que vários fatores contribuíram para inclusão do tema na agenda governamental no estado da Bahia tais como, magnitude e gravidade de casos, mobilização da comunidade científica para o desenvolvimento de pesquisas, conjuntura política favorável com gestores e parlamentares sensíveis à situação e militância de indivíduos infectados. Houve consenso em relação à necessidade de envolvimento de diferentes atores para estruturação de políticas de prevenção e controle dessa IST. No estado de Minas Gerais, apenas alguns problemas identificados em seu fluxo entraram na agenda. Observamos que as decisões aprovadas favoreceram a ampliação de ações referentes ao HTLV, porém, apesar de defesa de pesquisadores quanto à necessidade de estruturação da assistência e investimentos na área, não houve consenso em todo momento para maiores avanços. As dificuldades de implementação de política setorial e a inexistência de grupos de pressão organizados colaboraram para que essas ações não fossem uma prioridade para as secretarias de saúde de Minas Gerais. Este trabalho pioneiro de análise da inclusão do tema em agendas governamentais visou compreender este processo, contribuindo para o debate e conferindo maior visibilidade sobre a implementação de políticas referentes ao HTLV. Constatamos que na atual conjuntura, indivíduos infectados por HTLV e outros atores envolvidos com o tema ainda possuem um longo caminho a percorrer na mobilização para investimentos na área, para a formulação e implementação de políticas públicas e conquista de direitos sociais.

Brazil is considered the country with the highest absolute number of infected individuals, with an estimated prevalence of up to 2.5 million HTLV-positive individuals. Studies carried out with blood donors show that HTLV infection is present throughout the country, albeit heterogeneously. The objective of this research is to analyze the process of formulating an agenda and implementing policies and actions related to HTLV in the states of Bahia and Minas Gerais, which are two of the pioneer states in the development of public policies aimed at people infected by the virus. The Multiple Streams Model was used to understand how and why the theme was inserted in the governmental agenda giving rise to public policies regarding HTLV in these states. Several factors contributed to the inclusion of the topic on the governmental agenda in the state of Bahia, such as the magnitude and severity of cases, mobilization of the scientific community for the development of research, favorable political conditions with managers and parliamentarians sensitive to the situation and the militancy of individuals infected. There was consensus regarding the need for involvement of different actors to structure policies for the prevention and control of this STI. In the state of Minas Gerais, only a few problems identified entered the agenda. We observed that the approved decisions favored the expansion of actions related to HTLV, however, despite the defense of researchers regarding the need for structuring assistance and investments in the area, there was no consensus at all times for further advances. The difficulties in implementing sectorial policy and the lack of organized pressure groups, contributed to the fact that these actions were not a priority for the health departments of Minas Gerais. This pioneering analysis aimed to understand that process, provide greater visibility and contribute to the debate on the implementation of policies related to HTLV. In this context, individuals infected with HTLV and other actors involved with the theme still have a long way to go in mobilizing investments in the area, in the formulation and implementation of public policies and the conquest of social rights.
