Esta pesquisa buscou compreender o modo como os pacientes adultos jovens com câncer dão sentido ao seu processo de adoecimento e tratamento apoiados nas relações de cuidado no contexto de internação hospitalar, com ênfase na sua relação com os profissionais de enfermagem. Falar sobre o cuidado em saúde em seu aspecto relacional implica reconhecer suas dimensões intangíveis, em especial o vínculo, autonomia e criatividade possíveis para esses pacientes. Indagou-se se e como essas dimensões poderiam se fazer presentes nas experiências dos pacientes e quais seriam os sentidos (re)construídos acerca dessas experiências. A pesquisa se fundamentou nos referenciais da Psicossociologia Francesa de base psicanalítica e sua realização se deu através da construção e análise de narrativas sobre as vivências dos pacientes durante os períodos de internação, sendo estas construídas em conjunto pelos pacientes e pesquisadora. Os resultados foram analisados conforme quatro categorias que emergiram das narrativas: \201CConstruções sobre castelos de areia \2013 o lugar do vínculo nas relações de cuidado e novas formas de saúde\201D, \201CSherazade, super-heróis, princesa-guerreira e maratonista \2013 autonomia, criatividade e (re)invenções da saúde após o câncer\201D, \201CA face \2018amiga\2019 da morte \2013 possibilidades de falar sobre o inominável e pensar sobre o inimaginável\201D e \201C Interpelar, refletir e implicar-se: a pesquisadora no processo de construção das narrativas\201D. As dimensões intangíveis mostraram-se imbricadas umas às outras nas experiências de adoecimento e tratamento dos pacientes; no caso do vínculo, foi apontado sua importância enquanto confiança e pelas funções de holding (sustento) e containing (continência). A autonomia e criatividade foram discutidas através das representações singulares e metáforas que esses pacientes utilizaram para falar sobre seu processo de saúde-doença. O tema da morte foi dividido em três momentos que articularam passado, presente e futuro dos pacientes: o diagnóstico, o tratamento no hospital e as situações nas quais presenciaram a morte de amigos e colegas de quarto durante a internação. A última categoria tratou da participação implicada da pesquisadora na construção dos sentidos, tanto na escrita das narrativas quanto no momento de análise clínica destas. Os pacientes puderam expressar que a partir dessas relações de cuidado foram possíveis construções singulares para \201Catravessar\201D o momento de adoecimento e tratamento, mesmo frente à ruptura, o desencaixe e a desconstrução trazidos pelo câncer.
Abstract
This research aimed to understand how young patients undergoing cancer treatment give meaning to their process of illness and treatment, supported by care relationships in the context of hospitalization, with an emphasis on their relationship with nursing professionals. Talking about health care in its relational aspect implies recognizing its intangible dimensions, especially bond, autonomy and creativity. It was asked whether and how these dimensions could be present in the patients\2019 experiences and what the (re)constructed meanings about these experiences would be. The research was based on the references of French Psychosociology based on psychoanalysis and it was conducted throught construction and analysis of narratives about patients\2019 experiences during hospitalization, which were built together by patients and researcher. The results were analyzed according to four categories that emerged from the narratives: \201CBuildings on sandcastles \2013 the place of bonding in care relationships and new ways of health\201D, \201CSherazade, superheroes, warrior-princess and marathon runner \2013 autonomy, creativity and (re)inventions of health after cancer\201D, \201CThe \2018friendly\2019 face of death \2013 possibilities to talk about the unnameable and think about the unimaginable\201D, and \201C Interpellate, reflect and imply: the researcher in the process of building narratives\201D. The intangible dimensions proved to be imbricated to each other in the experiences of illness and treatment of patients; in the case of bond, its importance was pointed out as trust and by the functions of holding and containing. Autonomy and creativity were discussed through the singular representations and metaphors that these patients chose to talk about their health-disease process. The subject of death was divided into three moments that articulated the patients\2019 past, present and future: diagnosis, treatment in the hospital and the situations in which they witnessed the death of friends and roommates during hospitalization. The last category dealt with the researcher\2019s implicated participation in the construction of meanings, both in the writing narratives and in the moment of their clinical analysis. Patients were able to express that, based on these care relationships, singular constructions were possible to \201Cget through\201D the moment of illness and treatment, even in the face of rupture, disengagement and deconstruction brought about by cancer.
