A Residência Médica (RM) é considerada, tanto academicamente quanto profissionalmente, método padrão-ouro no ensino de médicos egressos da Universidade. O Programa de Residência em Medicina de Família e Comunidade de Fortaleza, Ceará, teve seus primórdios em 2005, no contexto da implantação da Estratégia Saúde da Família (ESF) e do Sistema Municipal de Saúde-Escola (SMSE) nesta capital. Esse programa educacional é o primeiro do gênero em larga escala, implantado em uma capital, concomitante com a rede de serviços da ESF e com taxa de ocupação maior que a nacional, sendo necessário aferir crítica e reflexivamente como este Programa tem se desenvolvido. Para tanto, esta pesquisa tem como objetivo avaliar o Programa de Residência em Medicina de Família e Comunidade de Fortaleza, a partir da ótica dos residentes. Trata-se de um estudo transversal descritivo com abordagem qualitativa, realizado com a participação de 18 residentes (R2) em cinco Grupos Focais, no segundo semestre de 2020. Os dados foram analisados pela técnica de Análise Temática em que foram identificadas como subcategorias principais: Estratégias de ensino-aprendizagem, Avaliação do processo ensino-aprendizagem, Produção de autonomia, Transferência da formação para a prática, Fragilidades da residência e Potencialidades da residência. Identificou-se que os residentes compreendem a responsabilidade que têm no atendimento médico diário, ainda que como aprendizes. Eles demonstram insegurança como sujeitos da formação (aprendizagem ativa), sentindo-se mais confortáveis quando são colocados no papel passivo. Por outro lado, os discentes do programa reconhecem a metodologia ativa de ensino-aprendizagem como adequada para o ensino de adultos, mas precisam que as estratégias estejam voltadas para a resolução de problemas comuns à prática profissional deles. Dessa forma, é preciso valorizar as experiências e competências adquiridas heterogeneamente pelos residentes para compor momentos de equalização do aprendizado, buscando o protagonismo do aluno ao invés da imposição de conhecimentos.
Abstract
Medical Residency (MR) is considered, both academically and professionally, the gold standard method in teaching physicians who graduated from the University. The Residency Program in Family and Community Medicine in Fortaleza, Ceará, had its beginnings in 2005, in the context of the implementation of the Family Health Strategy (ESF) and the Municipal School Health System (SMSE) in this capital. This educational program is the first of its kind on a large scale, implemented in a capital, concomitant with the ESF service network and with an occupancy rate higher than the national one, making it necessary to critically and reflectively assess how this Program has been developed. Therefore, this research aims to evaluate the Residency Program in Family and Community Medicine in Fortaleza, from the perspective of residents. This is a descriptive cross-sectional study with a qualitative approach, carried out with the participation of 18 residents (R2) in five Focus Groups, in the second half of 2020. The data were analyzed using the Thematic Analysis technique, in which the main subcategories were identified: Teaching-learning methodologies, Evaluation of the teaching-learning process, Production of autonomy, Transfer of training to practice, Residency weaknesses and Residency potentials. It was identified that residents understand the responsibility they have in daily medical care, even as apprentices. They demonstrate insecurity as subjects of training (active learning), feeling more comfortable when placed in the passive role. On the other hand, the program's students recognize the active teaching-learning methodologies as adequate for teaching adults, but they need them to be focused on solving problems common to their professional practice. Thus, it is necessary to value the experiences and skills acquired heterogeneously by residents to create moments of equalization of learning, seeking the student's protagonism instead of imposing knowledge.
