INTRODUÇÃO: Os Portadores de coagulopatias congênitas apresentam o risco elevado de contrair a infecção pelo vírus da hepatite C (VHC) devido ao uso de sangue, seus componentes e derivados, tendo sido minimizado pelas políticas do sangue. Em um estudo realizado na Fundação de Hematologia e Hemoterapia da Bahia (HEMOBA) há 16 anos com hemofílicos, a soroprevalência do VHC foi de 42% (IC 95% 36%-48%), sendo que o genótipo 1 do VHC (74%) foi o mais prevalente, seguido pelos genótipos 3 (22%) e 2 (4%) (Silva et al., 2005). O objetivo deste estudo foi determinar a prevalência atual de infecção pelo VHC em hemofílicos e portadores de outras coagulopatias congênitas. MATERIAIS E MÉTODOS: Este estudo possui dois componentes, um de corte transversal, para determinar a prevalência das infecções provocas pelo VHC, e outro tipo caso-controle, para analisar os fatores de risco e associação com gravidade da doença de base. Entre fevereiro de 2014 e fevereiro de 2016, foram incluídos no estudo 172 pacientes com diagnóstico laboratorial de hemofilia ou outras coagulopatias congênitas atendidos na HEMOBA, mediante assinatura do TCLE, entrevista e coleta de amostras de sangue. As amostras foram encaminhadas ao Laboratório Central de Saúde Pública (LACEN-BA) para realização imunoensaio de 4. geração (Architect, Abbott), exames moleculares
para detecção qualitativa, quantificação e genotipagem do VHC-RNA, pelo método Real Time VHC (Abbott). RESULTADOS: A maioria dos participantes foi do sexo masculino 94%; residentes em cidades do interior da Bahia (69%), pardos ou negros (77%) e estado civil solteiro (69%). A média de idade foi de 26,2 anos e desvio padrão de (15,9) anos (mín. 2 anos – máx. 71 anos). A coagulopatia congênita mais frequente foi a hemofilia A (81%). A maioria dos pacientes (91%) já realizaram transfusão de sangue, seus componentes e derivados, e destes, 31% o fizeram antes de 1993. A soroprevalência do VHC foi de 21% (IC 95% 15%-28%), com dois casos soropositivos nascidos após 1993. A viremia foi detectada em 70% dos soropositivos. A maioria dos soropositivos (80%), tinham conhecimento prévio da infecção, mas apenas 24% destes tinham histórico de tratamento antiviral. O genótipo 1 foi o mais frequente (77%), seguido do genótipo 3 (9%) e do genótipo 2 (5%). O genótipo foi indeterminado em 9% dos pacientes. CONCLUSÃO: Apesar da importante redução na prevalência de infecção do VHC, essa prevalência do VHC ainda é muito alta quando comparada
com a prevalência na população geral da Bahia (1,5%). O tratamento com as novas DAAs poderá contribuir para minimizar o risco residual de transmissão e o débito social pelas infecções ocorridas no passado nesta população.
Abstract
INTRODUCTION: Carriers of congenital bleeding disorders are at high risk of being infected by hepatitis C virus (HCV) due to the use of blood and/or derivatives, which was minimized by the blood policies. In a study conducted in the Hematology and Hematology Foundation of Bahia (HEMOBA) with hemophiliacs 16 years ago, HCV seroprevalence was 42% (95% CI 36%-48%), and the HCV genotype 1 (74%) was the most prevalent, followed by genotype 3 (22%) and 2 (4%) (Silva et al., 2005). The aim of this study was to determine the HCV prevalence of infection among hemophiliacs and carriers of other congenital bleeding disorders. MATERIAL AND METHODS: This study has two components: one transversal study to determine the prevalence of HCV infection, and another case-control study to analyze the association of risk factors and severity of base disease. Between February 2014 and February 2016, 172 patients with congenital bleeding disorders treated at the HEMOBA were included in the study after signing the informed consent, being interviewed and collection of blood samples. The samples were sent to the Central Public Health Laboratory (LACEN-BA) to perform fourth-generation immunoassay (Architect, Abbott), molecular tests for the qualitative detection, quantification and genotyping of HCV-RNA by Real Time HCV method (Abbott). RESULTS: As expected most of the participants were male (94%); living in cities in the countryside of Bahia (69%), been african-americans (77%) and single (69%). The mean age was 26.2 years old and standard deviation of 15.9 years (ranging from 2 to 71 years). The most frequent congenital coagulopathy was hemophilia A (81%). Most patients were transfused with blood and/or blood derivatives (91%), and some did it before 1993 (31%). HCV seroprevalence was 21% (95% CI 15%-28%), with two HCV-positive cases born after 1993. Viremia was detected in 70% of the HCVseropositive. Most seropositive cases (80%) had prior knowledge of the HCV infection, but only 24% of them had history of antiviral therapy. Genotype 1 was the most frequent (77%), followed by genotype 3 (9%) and genotype 2 (5%). Genotype was undetermined in 9% of patients. CONCLUSION: Despite the significant reduction in the HCV prevalence of infection in carriers of congenital bleeding disorders, is high compared with the prevalence in the general population of Bahia. The treatment with the new DAAs may contribute to minimize the residual risk of transmission and the social debit for the infections occurred in the past in this population group.
