A presença de Enterobacterales resistentes aos carbapenêmicos e polimixinas no ecossistema aquático representa um grande desafio para a saúde pública globalmente. As Estações de Tratamento de Esgotos (ETEs) são consideradas como reservatórios desses patógenos, contribuindo com a sua disseminação para o ambiente. Desta forma, para a remoção de bactérias multirresistentes e, consequentemente, redução de riscos ambientais, é necessária a implementação de tecnologias de desinfecção, como os processos oxidativos avançados (POAs) catalisados por resíduos de mineração, fornecendo valor agregado a esse material. O objetivo deste estudo foi avaliar a ocorrência de Enterobacterales resistentes a carbapenêmicos e/ou polimixinas em efluentes de diferentes ETEs, além de investigar o potencial de POAsusando resíduo da mineração na remoção de Klebsiella pneumoniae multidroga resistente edo gene blaKPC no efluente tratado por POA. Para este fim, foram coletadas e caracterizadas seis amostras de efluentes brutos e tratados de três diferentes ETEs localizadas no Rio de Janeiro. A partir de meios seletivos foram isoladas e identificadas, pelo sistema MALDI-TOF, 98 Enterobacterales. O perfil de susceptibilidade aos antimicrobianos foi avaliado pelo método disco-difusão e concentração inibitória mínima por microdiluição em caldo. Das 98 cepas, 3,1% (3/98) foram suscetíveis a todos os antimicrobianos, 18,4% (18/98) apresentaram o perfil nãoMDR (“multidrug-resistant”), 54,1% (53/98) o perfil MDR; 23,5% (23/98) XDR (“extensively drug-resistant”), e 1% (1/98) PDR (“pandrug-resistent”) A presença dos genes blaNDM, blaKPC, blaOXA-48-like, blaIMP, blaVIM, blaGES, blaBKC e mcr-1, 2, 3, 4, 5 foi investigada pela PCR. Entre os genes investigados, o gene mcr-1 foi encontrado apenas em uma Escherichia coli, sendo este o primeiro relato deste gene em efluentes no Brasil. Além disso, foram detectados os genes blaNDM (51,7%; 30/58); blaKPC (24,1%; 14/58); blaOXA-48-like (22,4%; 13/58); e blaIMP (5,2%; 3/58). Avaliamos também a relação clonal de K. pneumoniae por MLST. A análise da relação clonal de K. pneumoniae revelou cinco “sequence types” (STs): ST11, ST37, ST147, ST281, ST244, sendo o primeiro relato da presença dos STs 244 e 281 em efluentes. Uma cepa K. pneumoniae ST11 carreando gene blaKPC foi selecionada para os testes com os processos oxidativos combinados com resíduo de mineração. O processo combinado se mostrou eficiente para eliminação de K. pneumoniae e redução do número de cópias gene blaKPC em efluentes tratados. Os resultados deste estudo destacam a importância do monitoramento da resistência antimicrobiana em ETEs, além da necessidade de implementação de tecnologias alternativas para desinfecção de efluentes, a fim de contribuir para a redução da propagação da resistência bacteriana no ambiente.
Abstract
The presence of carbapenem and polymyxin-resistant Enterobacterales in the aquatic ecosystem represents a major challenge for global public health. Wastewater Treatment Plants (WWTPs) are considered reservoirs of these pathogens, contributing to their dissemination in the environment. In this way, for the removal of multidrug-resistant bacteria and consequently, reduction of environmental risks, the implementation of disinfection technologies, such as catalyzed advanced oxidative processes (AOPs) using mining waste, is necessary, providing added value to this material. The aim of this study was to assess the occurrence of carbapenem and/or polymyxin-resistant Enterobacterales in effluents from different WWTPs, as well as to investigate the potential of combined advanced oxidative processes using mining waste in the removal of multidrug-resistant Klebsiella pneumoniae carrying the blaKPC gene and this gene in the effluent treated by AOPs. For this purpose, six samples of untreated and treated wastewater from three different WWTPs located in Rio de Janeiro were collected and characterized. From selective media, 98 Enterobacterales isolates were obtained and identified using the MALDI-TOF system. The antimicrobial susceptibility profile was evaluated by the disk diffusion method and minimum inhibitory concentration by broth microdilution. Out of the 98 strains, 3.1% (3/98) were susceptible to all antimicrobials, 18.4% (18/98) showed the non-MDR (multidrug-resistant) profile, 54.1% (53/98) showed the MDR profile, 23.5% (23/98) showed the XDR (extensively drug-resistant) profile, and 1% (1/98) showed the PDR (pandrugresistant) profile The presence of the blaNDM, blaKPC, blaOXA-48, blaIMP, blaVIM, blaGES, blaBKC and mcr-1, 2, 3, 4, 5 genes was investigated by PCR. Among the investigated genes, the mcr-1 gene was found only in one Escherichia coli, representing the first report of this gene in effluents in Brazil. Additionally, the blaNDM (51.7%; 30/58), blaKPC (24.1%; 14/58), blaOXA-48- like (22.4%; 13/58), and blaIMP (5.2%; 3/58) genes were detected. We also evaluated the clonal relationship of K. pneumoniae by MLST. Analysis of the clonal relationship of K. pneumoniae isolates revealed five sequence types (STs): ST11, ST37, ST147, ST281, ST244, with the first report of the presence of STs 244 and 281 in wastewater. A K. pneumoniae ST11 strain carrying the, blaKPC gene was selected for tests using combined oxidative processes with mining residues. The combined process proved to be efficient in eliminating K. pneumoniae and reducing the number of, blaKPC gene copies in the treated effluents. The results of this study highlight the importance of monitoring antimicrobial resistance in WWTPs, as well as the need for the implementation of alternative technologies for wastewater disinfection, in order to contribute to the reduction of bacterial resistance propagation in the environment.
