AVALIAÇÃO DA ADESÃO AO TRATAMENTO PROFILÁTICO DE PACIENTES HEMOFÍLICOS TRATADOS NO SISTEMA ÚNICO DE SAÚDE: FUNDAÇÃO HEMOCENTRO DE BRASÍLIA

Introdução: A hemofilia é uma enfermidade congênita ocasionada pela deficiência de fator VIII ou fator IX de coagulação e se caracteriza pela presença de sangramentos, principalmente, articulares ou musculares. O tratamento é feito por meio da reposição intravenosa do fator deficiente e pode ocorrer por demanda, após o sangramento, ou como profilaxia com a reposição contínua do fator. A indicação da profilaxia é o tratamento mais efetivo ao longo da vida, no entanto, por ser um tratamento intravenoso, a adesão é um ponto crítico e pode impactar no sucesso da terapia. Objetivo: Avaliar o nível de adesão ao tratamento profilático dos pacientes hemofílicos tratados na Fundação Hemocentro de Brasília, bem como identificar as barreiras capazes de interferir na adesão e correlacionar o registro de episódios de sangramento com a variável adesão. Materiais e Métodos: Trata-se de um estudo transversal conduzido entre 25 de maio de 2022 e 15 de outubro de 2022 na Fundação Hemocentro de Brasília em pacientes com hemofilia. Foram aplicados dois questionários de autorrelato, sendo o primeiro validado, a ferramenta VERITAS-Pro instrumento que mensura a adesão por meio de domínios que interferem na profilaxia: Rotina, Dosagem, Planejamento, Lembrança, Omissão e Comunicação, bem como de forma global por meio de uma pontuação com corte ≥ 57. O segundo instrumento foi desenvolvido para a pesquisa. Além disso, fez-se análise retrospectiva de dados de prontuário para variáveis clínicas. Resultados: Dos 169 pacientes elegíveis, 122 foram contatados e 101 efetivamente participaram do estudo. Considerando a ferramenta VERITAS-Pro, 78,2% dos pacientes apresentam adesão à profilaxia, sendo a faixa etária de 0-11 anos o grupo com maior adesão 96,4%. A adesão reduziu entre os adolescentes (66,7%) e foi ainda menor para os idosos (33,3%). Considerando a pontuação geral do VERITAS-Pro, a pontuação média foi de 42,5 pontos (Mediana = 39 pontos). As variáveis relatadas com maior frequência foram “Falta de Tempo”, “Dose de 250UI”, “Distância”, “Veia”, “Esquecimento” e “Medo”. Com base nos testes de hipóteses, constatou-se que não há evidências para associar as variáveis estudadas e a adesão. O teste de correlação também apontou ausência de relação entre a variável registro de sangramento e adesão. Considerações Finais: Espera-se que este trabalho possa motivar intervenções e melhorias no contexto de atendimento aos pacientes com hemofilia e que outros estudos possam ser desenvolvidos com amostras maiores e uso de outras metodologias
Abstract
Introduction: Hemophilia is an inherited bleeding disorder caused by a deficiency of blood clotting factor VIII or factor IX and is characterized by bleeding, mainly in joints or muscles. Treatment includes intravenous replacement of the deficient factor and can be on demand, after bleeding, or as prophylaxis with continuous replacement of the factor. Prophylaxis is the most effective treatment throughout life, however, as it is an intravenous treatment, adherence is a critical point and can impact the success of therapy. Objective: Evaluate the level of adherence to prophylactic treatment of patients with hemophilia treated at Blood Center Foundation of Brasilia, as well as to identify the barriers capable of interfering with adherence and to correlate the recording of bleeding episodes with the variable adherence. Materials and Methods: This is a cross-sectional study conducted between May 25, 2022 and October 15, 2022 at Blood Center Foundation of Brasilia in patients with hemophilia. Two self-report questionnaires were applied, the first being validated, the VERITAS-Pro tool, an instrument that measures adherence through domains that interfere with prophylaxis: Routine, Dosage, Planning, Reminder, Omission and Communication, as well as globally through of a cut-off score ≥ 57. The second instrument was developed for the research. Besides, a retrospective analysis of medical records was performed for clinical variables. Results: Of the 169 eligible patients, 122 were contacted and 101 actually participated in the study. Considering the VERITAS-Pro tool, 78.2% of patients adhere to prophylaxis, with the age group 0-11 years being the group with the highest adherence, 96.4%. Adherence reduced among teenagers (66.7%) and was even lower among the elderly (33.3%). Considering the overall VERITAS-Pro score, the average score was 42.5 points (Median = 39 points). The most frequently reported variables were “Lack of Time”, “Dose of 250UI”, “Distance”, “Vein”, “Forgetfulness” and “Fear”. Based on the hypothesis tests, it was found that there is no evidence to associate the studied variables and adherence. The correlation test also showed no relationship between the variable bleeding record and adherence. Final Considerations: It is expected that this study can motivate interventions and improvements in the context of care for patients with hemophilia and that other studies can be developed with larger samples and the use of other methodologies
