O antimoniato de meglumina (AM) é um dos principais tratamento da leishmaniose tegumentar americana (LTA) e recentemente foi associado a toxicidade coclear (TC). Objetivou-se validar o uso do autorrelato de perda auditiva e zumbido e o uso de emissões otoacústicas evocadas, produto de distorção (EOAPD) e emissões otoacústicas evocadas transitórias (EOAT) no diagnóstico de TC por AM. Este estudo foi realizado em dois artigos. Artigo 1: Estudo prospectivo longitudinal com 102 pacientes com diagnóstico parasitológico de LTA, tratados com diferentes esquemas de AM. Presença de toxicidade auditiva clínica foi definida como o surgimento ou agravamento de perda auditiva e/ou zumbido autorreferidos durante o acompanhamento. Foram calculadas medidas de sensibilidade, especificidade e valor preditivo positivo e negativo do autorrelato de perda auditiva e zumbido do paciente em relação ao resultado do exame audiométrico (considerado padrão-ouro). A idade dos pacientes avaliados variou de 15 a 81 anos, com mediana de 41 anos, sendo a maioria do sexo masculino (73,5%). Setenta e cinco pacientes (73,5%) apresentavam leishmaniose cutânea e 27 (26,5%) leishmaniose mucosa. Oitenta e seis pacientes (84,3%) receberam tratamento intramuscular (IM) e 16 (15,7%) foram tratados com AM intralesional. Durante o tratamento, 53 (52%) pacientes apresentaram TC confirmada por audiometria tonal limiar (ATL) e audiometria de altas frequências (AAF), dos quais 60% receberam dose de 20 mg Sb5+/kg/dia (p=0,015) e 96.2% foram tratados com AM IM (p=0,001). O zumbido tem maior especificidade e valor preditivo positivo do que a perda auditiva, com baixo número de falsos positivos, mas com alto valor de falsos negativos. Artigo 2: Estudo prospectivo longitudinal com 23 pacientes com diagnóstico parasitológico de LTA com monitorização auditiva por meio de EOAPD e EOAT. Os pacientes foram submetidos a PTA, HFA e EOAE antes do início do tratamento, a cada dez dias durante o tratamento e ao final do tratamento. Dezoito pacientes (78,3%) eram do sexo masculino, com idade entre 16 e 70 anos, com mediana de 44 anos. Durante o tratamento com AM, observou-se que 10 (43,5%) pacientes apresentaram TC confirmada por ATL e AAF. A redução de 0,15 na amplitude de resposta na frequência de 1000Hz nas EOAEPD apresentou sensibilidade de 70% e a redução de 0,35 na amplitude de resposta na faixa de frequência de 2500-35000Hz nas EOAT apresentou sensibilidade de 90% na detecção precoce após 10 dias do tratamento da toxicidade coclear. O autorrelato de perda auditiva ou, mais especificamente, de zumbido, pode ser uma ferramenta útil e de fácil aplicação no diagnóstico de toxicidade coclear ao AM, indicando a necessidade de direcionar o paciente para avaliações e acompanhamento audiológicos, principalmente naqueles pacientes tratados com 20 mg Sb5+/kg/dia via IM. Qualquer redução na amplitude das EOAE na frequência de 1000 Hz nas EOAPD e na faixa de 2500 e 3500 Hz nas EOAT pode ser considerada toxicidade coclear ao AM nos primeiros dez dias de tratamento, mostrando a boa aplicabilidade de utilização destes exames no seu diagnóstico precoce, em especial as EOAET que apresentam maior sensibilidade (90%) que as EOAEPD (70%)
Abstract
Meglumine Antimoniate (MA) is one of the treatments of American Tegumentary Leishmaniasis (ATL) and has recently been associated with cochlear toxicity (CT). The aim was to validate the use of self-reported hearing loss and tinnitus and the use of distortion product evoked otoacoustic emissions (DPOAE) and transient evoked otoacoustic emissions (TEOAE) in the diagnosis of AM’s CT. This study was carried out in two articles. Article 1: Prospective longitudinal study with 102 patients with a parasitological diagnosis of ATL, treated with different MA regimens. Presence of clinical hearing toxicity was defined as the onset or worsening of self-reported hearing loss and/or tinnitus during follow-up. Measures of sensitivity, specificity, and positive and negative predictive value of the patient's self-report of hearing loss and tinnitus were calculated in relation to the result of the audiometric exam (considered the gold standard). The age of the evaluated patients ranged from 15 to 81 years, with a median of 41 years, most of whom were male (73.5%). Seventy-five patients (73.5%) had cutaneous leishmaniasis and 27 (26.5%) mucosal leishmaniasis. Eighty-six patients (84.3%) received intramuscular (IM) treatment and 16 (15.7%) were treated with intralesional AM. During treatment, 53 (52%) patients had CT confirmed by pure tone audiometry (TTA) and high frequency audiometry (HFA), of which 60% received a dose of 20 mg Sb5+/kg/day (p=0.015) and 96.2% were treated with AM IM (p=0.001). Tinnitus has greater specificity and positive predictive value than hearing loss, with a low number of false positives but a high value of false negatives. Article 2: Prospective longitudinal study with 23 patients with parasitological diagnosis of ATL with auditory monitoring through DPOAE and TEOAE. Patients were submitted to PTA, HFA and EOAE before the starting of the treatment, every ten days during treatment and at the end of treatment. Eighteen patients (78.3%) were male, aged between 16 and 70 years, with a median age of 44 years. During MA treatment, it was observed that 10 (43.5%) patients had CT confirmed by ATL and AAF. The reduction of 0.15 in the response amplitude in the frequency range of 1000Hz in TEOAE showed sensitivity of 70% and the reduction of 0.35 in the amplitude of response in the frequency range of 2500-35000Hz in TEOAE showed sensitivity of 90% in early detection after 10 days of cochlear toxicity treatment. The self-report of hearing loss or, more specifically, tinnitus, can be a useful and easy-to-apply tool in the diagnosis of cochlear toxicity to MA, indicating the need to direct the patient to audiological evaluations and follow-up, especially in those patients treated with 20 mg Sb5+/kg/day IM. Any reduction in the amplitude of EOAE in the frequency of 1000 Hz in DPOAE and in the range of 2500 and 3500 Hz in TEOAE can be considered cochlear toxicity to MA in the first ten days of treatment, showing the good applicability of using these tests in its early diagnosis, especially TEOAE, which are more sensitive (90%) than DPOAE (70%)
