O Prontuário Eletrônico do Cidadão (PEC) integra o e-SUS, tendo se destacado por informatizar, qualificar e unificar as informações nas unidades de saúde, possibilitando melhores planejamentos e acompanhamentos. Em Itacoatiara-AM, em 2019 o PEC estava disponibilizado em todas as UBS da zona urbana. A pesquisa objetivou avaliar o grau de implantação do PEC nas UBS-ESF da zona urbana de Itacoatiara-AM. E, com base nos resultados, foi elaborado um Proposta de Intervenção para melhoria desteserviço no município. Foi realizada pesquisa avaliativa, do tipo estudo de caso único.As evidências foram obtidas por meio: (i) observação direta da estrutura para as atividades relativas ao PEC nas referidas UBS- ESF; e (ii) questionário semiestruturado com profissionais de saúde usuários do PEC (gestores, médicos, dentistas e enfermeiros) nessas UBS-ESF. Participaram deste estudo vinte e dois profissionais (três gestores, três médicos, cinco dentistas e onze enfermeiros) de seis UBS-ESF. Quanto à estrutura, o grau de implantação do PEC obteve classificação “Parcialmente Implantada” em apenas duas das seis unidades de saúde avaliadas (UBS-ESF 2 e 3). As outras quatro UBS-ESF foram classificadas na condição de “Implantação Incipiente”. A falta de acesso à internet banda larga de alto desempenho, e a falta de suporte técnico disponível para as equipes, contribuíram muito para este resultado. O grau de satisfação dos profissionais com o PEC obteve 72,7% da Pontuação Máxima (PM) sendo classificado na condição de “Parcialmente Satisfeito”. A utilização do PEC foi considerada “Parcialmente Implantada” com 79,9% da PM. Todavia, três subitens de alta relevância obtiveram boas pontuações, atingindo a condição de “Implantado”. São eles: o item 2.2, que investiga se o PEC é usado diariamente (81,8% da PM); o item 2.9, que indaga se os dados dos pacientes no PEC são visualizados com facilidade (89,4% da PM); e o item 2.12, que pergunta se o PEC melhorou o registro de dados do paciente. O Suporte Técnico foi o mais deficiente, chegando a alcançar apenas 39,1% da PM, sendo classificado na condição de “Implantação Incipiente”.Quanto à habilidade dos profissionais para utilizar o PEC, 19/22 participantes (86,4%) afirmaram que se sentem aptos, classificando esse componente na condição de “Implantado”. E, sobre os efeitos da pandemia do COVID- 19 na utilizaçãodo PEC, na opinião de 54,6% dos profissionais houve perdas. Com base nas recomendações do Ministério da Saúde (Manual de Implantação do e-SUS AB), em Itacoatiara-AM a implantação do PEC não está finalizada, estando no 5º cenário de implantação. Ou seja, nem todo o potencial desse software está sendo aproveitado. Para alcançar o 6º cenário (ideal), maiores esforços devemser empreendidos para implantar a estrutura necessária ao processo de informatização, e disponibilizar suporte técnico para a utilização do PEC nas UBS- ESF do município .
Abstract
The Citizen's Electronic Health Record (PEC) is part of the e-SUS, having stood outfor computerizing, qualifying, and unifying information in health units, enabling better planning and monitoring. In Itacoatiara-AM, in 2019 the PEC was available in all UBS in the urban area. The research aimed to evaluate the degree of implementation of the PEC in the UBS-ESF in the urban area of Itacoatiara-AM. And, based on the results, an Intervention Proposal was prepared to improve this service in the municipality. Evaluative research was carried out, using a single case study. The evidence was obtained through: (i) direct observation of the structure for activities related to the PEC in the aforementioned UBS-ESF; and (ii) semi-structured questionnaire with health professionals who use the PEC (managers, doctors, dentists and nurses) in these UBS- ESF. Twenty-two professionals (three managers, three doctors, five dentists and eleven nurses) from six UBS-ESF participated in this study. Regarding the structure, the degree of implementation of the PEC was classifiedas "Partially Implemented" in only two of the six health units evaluated (UBS-ESF 2 and 3). The other four UBS-ESF were classified as "Incipient Implementation". The lack of access to high-performance broadband internet, and the lack of technical support available to the teams, contributed greatly to this result. The degree of satisfaction of professionals with the PEC obtained 72.7% of the Maximum Score (MP), being classified as "Partially Satisfied". The use of the PEC was considered "Partially Implemented" with 79.9% of the PM. However, three highly relevant sub-items obtained good scores, reaching the "Implemented" status. They are: item 2.2, which investigates whether the PEC is used daily (81.8% of PM); item 2.9, which asks whether patient data in the PEC is easily viewed (89.4% of PM); and item 2.12, which asks whether the PEC improved patient data recording. Technical Support was the most deficient, reaching only 39.1% of MP, being classified as "Incipient Implementation". Regarding the professionals' ability to use the PEC, 19/22 participants (86.4%) stated that they feel capable, classifying this component as "Implemented". And, regarding the effects of the COVID-19 pandemic on the use of the PEC, in the opinion of 54.6% of professionals there were losses. Based on recommendations from the Ministry of Health (e-SUS AB Implementation Manual), in Itacoatiara-AM the implementation of the PEC has not been completed, being in the 5th implementation scenario. In other words, not all the potential of this software is being used. To achieve the 6th scenario (ideal), greater efforts must be made to implement the necessary structure for the computerization process, and provide technical support for the use of the PEC in the municipality's UBS-ESF .
