A segurança e a eficácia da imunoterapia antienvenenamento ofídico estão intimamente relacionadas à pureza e a uma cuidadosa determinação da potência do soro (atividade biológica). A metodologia atualmente utilizada apresenta importantes limitações, as quais este trabalho objetiva contribuir. A mesma é adotada como oficial pela Farmacopéia Brasileira, tendo sido preconizada em 1996 pela Portaria nº 174/MS. Observações preliminares deste estudo identificaram, principalmente, um alto índice de invalidação de ensaios e uma baixa repetibilidade e reprodutibilidade, representado por diferenças significativas no ensaio de potência nos resultados obtidos pelo INCQS e pelos laboratórios produtores, enfatizando a necessidade de aperfeiçoar a metodologia analítica para a determinação da potência do soro antibotrópico. De acordo com as recomendações do Workshop on the Standardization and Control of Antivenoms , coordenado pela Quality Assurance and Safety of Biologicals Unit da OMS em 2001, o estabelecimento de venenos e antivenenos de referência é essencial para a padronização destes ensaios e para permitir a comparação lote a lote, assim como a comparação entre laboratórios. Neste trabalho, apresentamos os resultados de um estudo interlaboratorial para o estabelecimento de veneno e antiveneno botrópico de referência, visando o aperfeiçoamento do controle da qualidade dos soros antibotrópicos, com ênfase na metodologia analítica para a determinação da potência, com a participação de todos os laboratórios brasileiros produtores de soro antibotrópico. Este estudo foi realizado em duas etapas. A primeira compreendeu a reavaliação da potência do lote nº 05 do Veneno Botrópico de Referência BRA/BOT/05, a avaliação da potência do candidato a Soro Antibotrópico de Referência Nacional BRA/ANTIBOT/01, e a avaliação da aplicabilidade de um controle da dose desafio de veneno referente a cinco vezes o valor de dose letal média (5 DL50).
Abstract
Safety and efficacy of antivenom immunotherapy are closely related with purity and an accurate potency determination (biological activity) of the antivenom. The murine lethality assay, still in use, present several limitations and is not a fully validated methodology. Preliminary data shows a high incidence of invalid assays and a low repeatability and reproducibility of potency assays, evidenced by significant differences between INCQS and manufacturers, indicating the need for improvement of the analytical methodology. According to the recommendations of the “Workshop on the Standardization and Control of Antivenoms”, sponsored by the Quality Assurance and Safety of Biologicals Unit of WHO at 2001, the establishment of local venom reference preparations and standard antivenom preparations are essential to standardize these assays and to allow batch to batch comparisons as well as comparisons between different laboratories. In this work, we present the results of an interlaboratorial study - with the participation of all Brazilian manufacturers - for the establishment of the lot 05 of the Brazilian Bothropic Reference Venom and lot 01 of the Brazilian Reference Antivenom, looking forward the improvement of quality control of Bothrops antivenom. This study was done in two steps. The first one consisted in the potency evaluation of the lot 05 of the Brazilian Bothropic Reference Venom, the potency evaluation of the lot 01 of the Brazilian Bothropic Reference Antivenom and the evaluation of the use of a control of the challenge dose of five times the median lethal dose (5 LD50). The second step consisted on the comparison between the absolute and the relative (parallel line assay) potency evaluation of commercial Bothrops antivenom, using the Brazilian Bothropic Reference Antivenom, in a blind assay. This monograph will focus only in the establishment of Brazilian Reference Bothropic Venom and Antivenom. These results allowed us to evaluate the intra-assay; inter-assay and inter-laboratorial precision, the incidence of invalid assays, and showed that each laboratory interpret differently the Bothrops Antivenom monograph of Brazilian Pharmacopoeia.
