Nos últimos anos observamos a emergência das febres Zika e chikungunya no Brasil. Essas arboviroses se expandiram rapidamente em todo território nacional num contexto de simultaneidade com a dengue, altamente prevalente e endêmica há décadas, configurando um cenário de tríplice epidemia. Somente em 2016, as três doenças causaram cerca de 2 milhões de casos, que aliados aos relatos de complicações neurológicas configuraram um grande problema para saúde pública nacional. Com o recente avanço de técnicas estatísticas de análise espacial, o estudo da difusão espacial demonstra-se uma importante ferramenta para o conhecimento e formulação de estratégias para o controle das doenças, na medida em que
permite compreender como e onde estas se propagam através do espaço e no decorrer do tempo. Existem alguns estudos sobre o padrão de difusão da dengue no Brasil, porém pouco
se sabe sobre as febres chikungunya e Zika, e em especial, a condição de tríplice epidemia. Nesse sentido, esse estudo teve como objetivos identificar e comparar o padrão de difusão espacial das epidemias de febre Zika, dengue e febre Chikungunya entre 2015-2016 no
município de Salvador-Bahia. Trata-se de uma pesquisa com dois desenhos de estudo, um seccional, para os dados de pontos, e um ecológico, para os dados de área. A área de estudo
compreendeu o município de Salvador, na Bahia, e o período de estudo abrangeu as ondas epidêmicas das três arboviroses, em 2015 e 2016. Foram utilizados dados do Sistema de Informação de Agravos de Notificação (SINAN), os quais foram georreferenciados para
aplicação de quatro técnicas de análise de difusão espacial utilizadas anteriormente em outros estudos; Avaliação por inspeção visual de mapas sequenciais (temáticos e Kernel), Análise da autocorrelação global (correlogramas espaciais) e local (mudança no LISA) e Análise do vizinho mais próximo. Os resultados demonstraram que o padrão de difusão das epidemias de febre Zika em Salvador foi o mesmo das epidemias de dengue e febre chikungunya, nos dois anos de estudo; difusão por expansão. As três doenças tiveram como principal foco de
transmissão a área que se estende do oeste do município à região central, caracterizada por intenso fluxo de transportes e alta densidade populacional. Esse conhecimento de como e onde as arboviroses se difundiram em Salvador, na situação de tríplice epidemia, consiste
numa importante evidência tanto no monitoramento de casos, quanto no direcionamento de intervenções para o controle das doenças.
Abstract
In recent years we have observed the emergence of fevers Zika and chikungunya in Brazil. These arboviruses expanded rapidly throughout the country, in a context of simultaneity with dengue fever, which was highly prevalent and endemic for decades in the country, creating a scenario of triple epidemics. Only in 2016, these arboviruses caused about 2 million cases, which allied with reports of neurological complications have been a major public health problem. With the recent advancement of statistical techniques of spatial analysis, the study of spatial diffusion is an important tool for the knowledge and formulation of strategies for disease control, since it allows understanding how and where they propagate through space and in the course of time. There are some studies on the pattern of diffusion of dengue in Brazil, but little is known about the fevers chikungunya and Zika, and especially, the condition of triple epidemic. In this sense, this study aimed to identify and compare the spatial diffusion pattern of Zika fever, dengue fever and Chikungunya fever between 2015-2016 in the city of Salvador-Bahia. It is a research with two study designs, one sectional, for the point data, and one ecological, for the area data. The study area comprised the municipality of Salvador, Bahia, and the study period covered the epidemic waves of the three arboviruses in 2015 and 2016. Data from the Sistema de Informação de Agravos de Notificação (SINAN) were used, which were georeferenced to the application of four spatial diffusion analysis techniques previously used in other studies; Evaluation by visual inspection of sequential maps (choropleth and kernel), Analysis of global autocorrelation (spatial correlograms) and local (change in LISA), and Analysis of the nearest neighbor. The results showed that the diffusion pattern of the Zika fever epidemics in Salvador was the same as that of the dengue epidemics and chikungunya fever in the two years of study, occurring predominantly by expansion. The three diseases had as main source of transmission the region that extends from the west of the municipality to the central region, territorial range of great transport connections and high population density. This knowledge of how and where the arboviruses spread in the territory of Salvador in the situation of triple epidemic presents itself as an important evidence for epidemiological surveillance, both in the monitoring of cases and in the targeting of interventions for disease control.
