A infecção do trato urinário (ITU) em gestantes é comum e acomete cerca de 10% das grávidas devido a mudanças anatômicas, fisiológicas e hormonais. O agente etiológico mais frequente é Escherichia coli, que apresenta algumas cepas com propriedade de adesão às células uroepiteliais facilitada nas gestantes pelo hiperestrogenismo. A urocultura é método diagnóstico considerado padrão-ouro, e para auxiliar na escolha terapêutica realiza-se o teste de sensibilidade antimicrobiana (TSA). A escolha terapêutica deve considerar a sensibilidade da bactéria, esquema posológico, toxicidade, custo, facilidade de aquisição e ainda a resistência bacteriana que ameaça a eficácia terapêutica. Essa escolha para as gestantes deve considerar também a capacidade de atravessar a barreira placentária e potencial risco de toxicidade ao feto. Este estudo propõe uma padronização de antimicrobianos para a realização do TSA direcionado a gestantes A metodologia adotada foi a descrição do perfil de patógenos, susceptibilidade e uso de antimicrobianos em 153 gestantes com ITU assistidas no Centro de Saúde Escola Germano Sinval Faria e Clínica da Família Victor Valla entre 2012 a 2015; busca sistemática na literatura para identificação dos antimicrobianos de melhor relação risco/benefício; e com base nesses resultados a elaboração do protocolo com a padronização de antimicrobianos para a realização do TSA. A literatura relata a Nitrofurantoínas como melhor risco/benefício no tratamento de ITU em gestantes. A Escherichia coli foi o principal uropatógeno encontrado em 64,3% das amostras. A idade média da população foi de 24 anos, com maior freqüência de ITU no 1o e 2o trimestre de gestação. A Tetraciclina, Penicilina, Ampicilina e Cefalotina foram os antimicrobianos onde observou-se maior resistência e a Cefalexina, Nitrofurantoína e Amoxicilina foram os mais prescritos para essa população. Foram padronizados 35 antimicrobianos categorizados em grupos por ordem prioritária de teste de acordo com os micro-organismos específicos. O protocolo de padronização de antimicrobianos para tratamento de ITU, direcionados para gestantes contribui para aprimorar a escolha terapêutica e favorece o uso racional de medicamentos.
Abstract
Urinary tract infection (UTI) in pregnant women is common and affects about 10% of pregnant women due to anatomical, physiological and hormonal changes. The most frequent etiological agent is Escherichia coli, which presents some strains with adhesion properties to uroepithelial cells facilitated in pregnant women by hyperestrogenism. The uroculture is the diagnostic method considered gold standard, and an antimicrobial susceptibility test (AST) is used to aid in the choice of therapy. The therapeutic choice should consider the sensitivity of the bacteria, dosage regimen, toxicity, costs, access and bacterial resistance. For pregnant women, this choice should also consider the ability to cross the placental barrier and potential risk of toxicity to the fetus. This study proposes a standardization of antimicrobials for AST directed to pregnant women The methodology adopted was the description of the profile of pathogens, susceptibility and antimicrobial use in 153 pregnant women with UTI assisted at the Germano Sinval Faria School Health Center and the Victor Valla Family Clinic from 2012 to 2015; a systematic search in the literature for the identification of the best risk / benefit antimicrobials; and the elaboration of the protocol with the antimicrobials standardized for the accomplishment of AST. The literature reports Nitrofurantoins as the best risk / benefit in the treatment of UTI in pregnant women. Escherichia coli was the main uropathogen, found in 64.3% of the samples. The mean age of the population was 24 years, with a higher frequency of UTI in the 1st and 2nd trimesters of gestation. Tetracycline, Penicillin, Ampicillin and Cephalotin were the most resistant antimicrobial agents observed and Cephalexin, Nitrofurantoin and Amoxicillin were the most prescribed for this population. Thirty five antimicrobials were categorized into groups by priority order of test according to the specific microorganisms. The antimicrobial standardization protocol for the treatment of UTI, directed to pregnant women, can contributes to improve the therapeutic choice and the rational use of medicines.
