O LDGCB é o mais comum subtipo histológico de LNH, respondendo por 30-40% dos diagnósticos. Mais de SO% dos linfomas do anel de Waldeyer acometem as tonsilas palatinas e o LDGCB é o principal diagnóstico. Atualmente, os LDGCB estão sendo classificados segundo o perfil de expressão de determinadas proteínas que conferem melhor ou pior prognóstico. A infecção pelo EBV como causa destes linfomas também vem sendo estudada, principalmente na região da cabeça e pescoço, pois a orofaringe é a porta de entrada do vírus no organismo. Estudar as características clínicas, laboratoriais e o perfil de expressão imuno-histoquímica para alguns marcadores (CD10, BcI-2, BcI-6, MUM1, pS3, CDS e CD34) nos LDGCB das tonsilas palatinas, correlacionando-os com o prognóstico, e estabelecer a associação destes linfomas com a infecção pelo EBV, comparativamente com linfomas nodais. Foram estudados retrospectivamente todos os pacientes com LDGCB das tonsilas palatinas diagnosticados no Hospital Aristides Maltez, em Salvador - Bahia, Brasil, no período de janeiro de 1999 a dezembro de 2006. Foi também estudado um grupo de 19 pacientes com LDGCB nodal diagnosticado no mesmo período. Os pacientes foram submetidos à revisão histopatológica e foi feita lHa complementar para CD1 O, BcI-2, BcI-6, MUM1, pS3, CDS e CD34. Foi também realizada pesquisa da infecção pelo EBV pela técnica da hibridização in situo A análise de SG e SLE foi feita pelo método de Kaplan-Meier e a comparação destas sobrevidas pelo log-rank test. De um total de S67 diagnósticos de LNH, o LDGCB representou 44,6% dos casos (2S3 casos) e os LDGCB das tonsilas palatinas representaram 4,2% do total de diagnósticos (24 casos). Os pacientes com linfoma tonsilar apresentaram LDH significantemente mais baixa que pacientes com linfoma nodal (p= 0,02). Pacientes com IPI O e 1 apresentaram SG e SLE maior (se considerados todos juntos) que pacientes com IPI ~ 2 (p= 0,04 e O,OOS, respectivamente) e pacientes com linfoma nodal e IPI O e 1 apresentaram SLE maior (p= 0,01). Pacientes com expressão de Bcl-2 e linfoma tonsilar apresentaram SLE significantemente menor (p= 0,02). Pacientes com linfócitos < 1.000/mm3 ao diagnóstico apresentaram SG menor (p= 0,02), se considerados todos juntos. Pacientes com linfoma tonsilar e linfócitos < 1 .000/mm3 apresentaram SG e SLE menores (p= < 0,0001 e 0,003, respectivamente). Não houve diferença significativa na sobrevida entre pacientes dos dois grupos (tonsilar e nodal) e o perfil de CG e não-CG, assim como em relação à expressão de pS3 e CD34. A pesquisa de CDS e da infecção pelo EBV resultaram negativas. OS parâmetros que influenciaram a SG e a SLE nos pacientes com linfoma tonsilar foram a contagem linfocitária ao diagnóstico e a expressão de BcI-2. Nos pacientes com linfoma nodal o IPI influenciou a SLE. Não foi demonstrada infecção pelo EBV nos pacientes estudados.
Abstract
Diffuse large B-cell lymphoma (DLBCL) is the most histological subtype of non-Hodgkin lymphoma (NHL), corresponding for 30- 40% of the diagnosis. More than 50% of the lymphomas in Waldeyer’s ring occur in the palatine tonsils and DLBCL is the main diagnosis. Presently, DLBCL are classified according to the expression profiles of certain proteins that are related to better or worse prognosis. Epstein-Barr virus (EBV) infection as a cause of this lymphomas has being studied, especially in head and neck region, once oropharynx is the main access of this virus Into the organism. [OBJECTIVE] Study the clinical and laboratories characteristic, and the immunohistochemical expression profile for same markers (CD10, Bcl-2, Bcl-6, MUM1, p53, CD5, and CD34) in the DLBCL of the palatine tonsils relating than with the prognosis, and establish an association of this lymphomas with the EBV infection, comparing with nodal lymphomas. [PATIENTS AND METHODS] All patients included at this study were diagnosis with DLBCL of the palatine tonsils and were followed at the Aristides Maltez Hospital during January 1999 to December 2006 was studied retrospectively. A group of 19 patients diagnosed with nodal DLBCL was studied at the same period. The patients were submitted to histopathological review and complementary immunohistochemical was done for CD10, Bcl-2, Bcl-6, MUM1, p53, CD5, and CD34. Search for EBV infection was also done through in situ hybridization technique. Overall survival (OS) and event free survival (EPS) analysis was done through Kaplan-Meier method and comparison of these survivals through log-rank test. [RESULTS] Out of 567 diagnosis of NHL total, DLBCL represented 44.6% of cases (253 cases) and DLBCL of the palatine tonsils 4.2% of the total diagnosis (24 cases). Patients with tonsils lymphoma had significantly lower LDH (lactate dehydrogenase) value than patients with nodal lymphoma (p= 0.02). Patients with 0 and 1 IPI (International Prognostic Index) had higher OS and EPS (if all were considered together) than patients with IPI ^
2 (p= 0.04 and 0.005, respectively), and patients with nodal lymphoma and IPI 0 and 1 had higher EPS (p= 0.01). Patients with tonsil lymphoma who expressed Bcl-2 had significantly lower EPS (p= 0.02). Patients with lymphocyte count < 1.000/mm3 had lower OS (p= 0.02) if considered all together. Patients with tonsils lymphoma and lymphocyte count < 1.000/mm^ had lower OS and EPS (p= < 0.0001 and 0.003 respectively). There was no significant difference in the survival among patients from both groups (tonsil and nodal) and the GC (germinal center) and non-GC profile, as well as in relation to p53 and CD34 expression. CD5 and EBV infection research was negative. [CONCLUSIONS] Parameters than influenced OS and EPS in the patients with tonsils lymphoma were lymphocyte count at the time of the diagnosis and Bcl-2 expression. IPI influenced EPS in the patients with nodal lymphoma. EBV infection was not found in the studied patients.
