O presente trabalho trata do tema da rotatividade de profissionais de saúde na zona rural de Sergipe. A pesquisa surgiu da experiência profissional da pesquisadora como enfermeira da zona rural da cidade de Tobias Barreto entre os anos de 2000 e 2001. O estudo da alta rotatividade percebida nessa localidade e em localidades vizinhas foi motivado pela hipótese de que a rotação constante de profissionais de saúde pudesse
estar trazendo prejuízos diversos à comunidade.
O objetivo deste trabalho foi analisar o fenômeno da rotatividade em cinco
municípios do estado. Aplicamos questionários com o objetivo de coletar as opiniões de todos os atores envolvidos no processo desde agentes de saúde a gestores municipais. Calculamos indicadores de rotatividade baseados no número de desligamentos e contratações num período de dois anos.
Foram encontradas como causas principais de descontentamento profissional e desligamento questões como: grande distância da capital, salário insuficiente, vínculo trabalhista precário e condições inadequadas de trabalho entre outras causas. As taxas de rotatividade encontradas variaram entre cidades, com uma média geral de
permanência média no emprego de 15 meses para enfermeiras e 13,5 meses para médicos.
Com a utilização do Planejamento Estratégico Situacional foi construída uma rede explicativa através dos questionários aplicados aos profissionais de saúde e propostas ações para o enfrentamento do problema. O estudo aponta para a necessidade do aprofundamento da pesquisa sobre rotatividade em saúde na atenção básica rural e para a necessidade de uma política nacional voltada para essa questão.
Abstract
This dissertation abords the theme of Turnover of Health Professionals in Rural areas of Sergipe State. The research came from the experience of the author as a nurse in the rural area of Tobias Barreto city during the years of 2000 and 2001. The high turnover that was perceived in this community and in other communities of the area lead the author to the hypothesis : Constant turnover came from volunteer decision of heath professional and was bringing several lost to the health care of the community.
The objective of this study was to analise the phenomena of turnover in five
cities of Sergipe state. We invite all the actors involved in the issue to answer questionnaires from the communities health agents to the health authority of the city, in order to know their opinion and suggestions. We calculate the turnover taxes based on the numbers of professionals who was admitted or left the work post in a period of two years.
We found as maim causes of high turnover problems such as work post away from the capital, insufficient income, precarious work contract, inapropiate work conditions and others. The retention taxes vary among cities but the average was 15 months for nurses and 13,5 months for physicians.
Using Situational Strategic Planing we build a explanatory net though the
questionnaire answered by the health professional. This study points to the need to further and deeper research about turnover of primary care professional in rural areas and to the need of a national policy for this issue in Brazil.
