O grupo de vírus linfotrópicos para células T humanas (HTLV) apresenta quatro integrantes: HTLV-1, 2, 3 e 4. Contudo, somente HTLV-1 e 2 apresentam relevância epidemiológica, com prevalência média de 0,46% em doadores de sangue no Brasil. Conforme o Ministério da Saúde, o diagnóstico de HTLV-1/2 é realizado por triagem com ELISA, seguida de confirmação pelos testes de Western blot (WB), imunoensaio em linha (INNO-LIA) ou reação em cadeia da polimerase (PCR). O teste de INNO-LIA possui uma alta sensibilidade, resolvendo até 98% dos casos, embora tenha um alto custo, enquanto o WB pode apresentar resultados indeterminados em 30-60% dos casos. Em contrapartida à sensibilidade inferior, a PCR possui alta especificidade em comparação a estes métodos. Em virtude disso, os testes de amplificação isotérmica de ácidos nucléicos mediada por alça (LAMP) para HTLV-1/2 estabelecidos por nosso grupo poderiam ser incorporados à rotina diagnóstica como alternativa à PCR com análise de polimorfismo do tamanho dos fragmentos de restrição (PCR-RFLP) ou à PCR em tempo real (qPCR). Portanto, este estudo teve como objetivo otimizar os testes de LAMP e determinar sua acurácia, sensibilidade e concordância frente aos resultados do testes confirmatórios sorológicos (WB e INNO-LIA) e moleculares (qPCR e PCR-RFLP) com amostras de indivíduos monoinfectados por HTLV-1 e 2 (n=144) ou coinfectados por HIV (n=92). Devido ao baixo desempenho de LAMP para HTLV-1 inicialmente, foram desenhados novos iniciadores e ajustadas as concentrações de betaína e magnésio. Após, este teste foi realizado na população coinfectada e mostrou uma acurácia de 90,24% em comparação com a qPCR, de 88,89% com a PCR-RFLP, e de 86,59% com o WB e INNO-LIA. Em relação ao resultado do diagnóstico definitivo, o teste de LAMP para HTLV-1 apresentou sensibilidade de 79,49%, resultando em valores similares à qPCR e à PCR-RFLP, que apresentaram taxas de 74,39% de 81,58%, respectivamente. LAMP para HTLV-2 obteve acurácia de 92,69% na comparação com a qPCR, de 96,30% com a PCR-RFLP, de 82,93% com o WB, e de 81,71% com o INNO-LIA. Na comparação com o diagnóstico final, LAMP para HTLV-2 obteve sensibilidade de 51,43%, contra 62,86% na qPCR e 54,29% na PCR-RFLP. De acordo com os critérios do índice Kappa de Cohen, o teste de LAMP para HTLV-1 teve concordância considerada substancial com todas as demais técnicas, enquanto para HTLV-2 foi quase perfeita na comparação com a PCR-RFLP, substancial com a qPCR, e moderada com as técnicas sorológicas. As taxas de acurácia dos testes de LAMP em pacientes monoinfectados por HTLV1/2 foram de 89,26%, 89,58% e 87,50% nas comparações com o WB, a qPCR e a PCR-RFLP, respectivamente. Em relação ao diagnóstico final destas amostras, o teste de LAMP mostrou maior sensibilidade para HTLV-1 (92,00%) e HTLV-2 (68,42%) na comparação com a qPCR (86,40% e 36,84%, respectivamente). De modo geral, os testes de LAMP para HTLV-1 e 2 tiveram performance semelhante às outras técnicas moleculares. Por isso, estas técnicas representam uma alternativa vantajosa no diagnóstico destas infecções, pois possuem custo reduzido, necessitam de estrutura laboratorial simples e apresentam menor de tempo de execução
Abstract
The group of human T-cell lymphotropic viruses (HTLV) has four members: HTLV-1, 2, 3 and 4. However, only HTLV-1 and 2 have epidemiological relevance, with a mean prevalence of 0.46% in blood donors in Brazil. As Brazilian Ministry of Health recommends, the diagnosis of HTLV-1/2 is performed by screening with ELISA, followed by confirmation with Western blot (WB), in-line immunoassay (INNO-LIA) or polymerase chain reaction (PCR). The INNOLIA has high sensitivity and is capable to solve almost 98% of the cases, although it is expensive. In turn, WB test can present indeterminate results in 30-60% of the cases. Although PCR displays lower sensibility, its specificity is higher than those assays. Therefore, the loopmediated isothermal nucleic acid amplification (LAMP) tests for HTLV-1/2 established by our research group could be used in the laboratorial diagnostic routine as an alternative to PCR with restriction fragment length polymorphism analysis (PCR-RFLP) or real-time PCR (qPCR). Thus, the aim of this project was to optimize the LAMP test for HTLV-1/2 and to determine the accuracy, sensitivity and agreement score (Cohen's Kappa) of LAMP against serological (WB and INNO-LIA) and molecular (PCR-RFLP and qPCR) assays. For this, we used DNA samples from HTLV-1 and 2 monoinfected (n=144) or coinfected with HIV (n=92). Due to a low performance of LAMP for HTLV-1, a new set of primers was designed and the reaction conditions were adjusted. DNA samples from HIV-coinfected HTLV-1/2 carriers were tested again by the LAMP test for HTLV-1, which showed an accuracy of 90.24% compared to the qPCR, 88.89% against the PCR-RFLP, 86.59% compared with WB INNO-LIA. Based on the definitive diagnostic results, HTLV-1 LAMP test showed a sensitivity of 79.49%, which were similar to the rates of 74.39% and 81.58% respectively observed for qPCR and PCR-RFLP. LAMP for HTLV-2 had an accuracy of 92.69% in comparison to qPCR, 96.30% with PCRRFLP, 82.93% against WB and 81.71% compared with INNO-LIA. In the analysis against the definitive diagnostic, LAMP test for HTLV-2 presented a sensitivity of 51.43% against, while the qPCR and the PCR-RFLP showed rates of 62.86% and 54.29%, respectively. The agreement score between the HTLV-1 LAMP test and the other techniques (WB, INNO-LIA, qPCR and PCR-RFLP) was considered strong, while the analysis of the LAMP test for HTLV-2 showed an agreement almost perfect in the comparison with the PCR-RFLP, strong with the qPCR and moderate compared to serological tests. The accuracy rates of the LAMP tests in samples from HTLV-1/2 monoinfected patients were 89.26%, 89.58% and 87.50% in the comparisons with WB, qPCR and PCR-RFLP, respectively. Considering the definitive diagnostic of these patients, LAMP test showed higher sensitivity for HTLV-1 (92.00%) and HTLV-2 (68.42%) compared to the qPCR (86.40% and 36.84%, respectively). In general, LAMP tests for HTLV1/2 showed similar results to the others molecular techniques in both studied populations. Therefore, LAMP for HTLV-1/2 may represent an advantageous alternative for the diagnosis, since it has lower cost, requires a less complex laboratory structure and presents a shorter runtime
