Introdução: Cardiopatias congênitas (CC) são anormalidades da estrutura e/ou função cardíaca e surgem durante desenvolvimento embrionário do coração. Podem ser isoladas ou associadas com outras malformações em outros sistemas, inclusive como parte de síndromes genéticas. Caracterizam-se como o tipo mais comum de malformações congênitas e apresentam importante morbimortalidade neonatal. Objetivo: Descrever o perfil clínico-epidemiológico de pacientes com cardiopatia congênita nascidos no IFF/FIOCRUZ; determinar a prevalência desta patologia, dos tipos mais encontrados e da presença de outras anomalias congênitas (AC) e síndromes genéticas concomitantes; avaliar a presença de fatores de risco, o diagnóstico fetal e o desfecho desses pacientes. Método: Estudo retrospectivo e transversal utilizando os dados do Estudo Colaborativo Latino-Americano de Malformações Congênitas (ECLAMC) nos anos de 2018 e 2019. Resultados: Foram analisados os casos de pacientes nascidos neste período no Instituto Fernandes Figueira- FIOCRUZ. Foram 2.036 nascimentos, com um total de 443 casos de AC, sendo 105 diagnósticos de cardiopatia congênita. Destes, 64 foram nascidos vivos e 41 natimortos. Houve maior incidência em gestantes multíparas, nos fetos de sexo feminino e a maioria dos pacientes apresentou outros defeitos congênitos associados e/ou síndromes genéticas. Entre as cardiopatias mais encontradas, destacam-se a comunicação interventricular (CIV), defeito do septo atrioventricular (DSAVT) e a coarctação de aorta. Houve alta taxa de diagnóstico pré-natal, com realização de ecocardiografia fetal em 87/105 e 79% de acerto completo do diagnóstico e 13% de acertos incompletos. Conclusões: a incidência de cardiopatia congênita, bem como a sua associaçao com outras AC e síndromes genéticas foi consideravelmente maior na população estudada, em relação ao descrito na literatura. A ocorrência de diferentes defeitos congênitos tem impacto no prognóstico, pois nesses casos, os pacientes apresentaram tempos maiores de internação e alta mortalidade; a minoria recebeu alta hospitalar. Desta forma, o diagnóstico precoce, melhora a sobrevida e qualidade de vida desta população.
Abstract
Introduction: Congenital heart defects (CHD) are abnormalities in the structure and/or function of the heart that arise during embryonic development. They can occur in isolation or be associated with other malformations in other systems, including as part of genetic syndromes. They are characterized as the most common type of congenital malformations and stand for significant neonatal morbidity and mortality. Objective: To describe the clinical-epidemiological profile of patients with congenital heart disease born at IFF/FIOCRUZ; to determine the prevalence of this pathology, the most common sorts found, and the presence of other congenital anomalies (CA) and concurrent genetic syndromes; to evaluate the presence of risk factors, fetal diagnosis, and the outcome of these patients. Method: Retrospective and cross-sectional study using data from the Latin American Collaborative Study of Congenital Malformations (ECLAMC) in the years 2018 and 2019. Results: Cases of patients born during this period at the Fernandes Figueira Institute-FIOCRUZ were analyzed. There were 2,036 births, with a total of 443 cases of CA, 105 of which were diagnosed with congenital heart disease. Of these, 64 were live births and 41 stillbirths. There was a higher incidence in multiparous pregnant women, female fetuses, and the majority of patients presented other associated congenital defects and/or genetic syndromes. Among the most common heart defects found are ventricular septal defect (VSD), atrioventricular septal defect (AVSD), and aortic coarctation. There was a high rate of prenatal diagnosis, with fetal echocardiography performed in 87/105 cases and 79% accuracy in complete diagnosis and 13% of partial diagnosis. Conclusions: The incidence of congenital heart disease, as well as its association with other CA and genetic syndromes, was considerably higher in the studied population compared to that described in the literature. The occurrence of different congenital defects has an impact on prognosis, as in these cases, patients had longer hospital stays and high mortality rates; the minority were discharged from the hospital. Thus, early diagnosis improves the survival and quality of life of this population.
