Abstract in Portuguese | A tendinopatia é uma doença degenerativa do tendão, causada por altos níveis de estresse mecânico ou acúmulo de microtraumas, que provocam a desorganização do tecido tendíneo. É uma doença multifatorial, que acomete 2 - 9% da população em geral, muito frequente em indivíduos que trabalham em ambientes suscetíveis a riscos ergonômicos, sendo considerada um distúrbio osteomuscular relacionado ao trabalho (Dort). Em atletas profissionais, que são altamente expostos a movimentos contínuos, a tendinopatia pode ocorrer em até 50% dos indivíduos. A principal queixa dos pacientes é a dor crônica no tendão afetado, que é resultado dos processos inflamatórios nas fases iniciais do desenvolvimento da lesão. Estudos indicam que as células T regulatórias (Treg), influenciadas pela proteína forkhead-box tipo3 (FOXP3) e pelo fator de transcrição homólogo aos receptores FcyIII clássicos (FcRL3), modulam a resposta imunológica para reparar a lesão no tendão. Além disso, uma maior expressão gênica do fator de necrose tumoral tipo alfa (TNF-α) também é encontrada em tendões degenerados. Sendo assim, este trabalho teve por finalidade avaliar a influência de polimorfismos dos genes FcRL3, FOXP3 e TNF-α no desenvolvimento da tendinopatia. Trata-se de um estudo analítico de caso-controle envolvendo 270 atletas brasileiros, sendo divididos em casos diagnosticados de tendinopatia e controles. Os polimorfismos de único nucleotídeo (SNPs) FcRL3 -169T>C, FOXP3 -2383C>T e TNF-α (-1031 T>C, -857C>T, -308G>A) foram determinados pela reação em cadeia da polimerase em tempo real (PCR), utilizando sistema TaqMan. As razões de chances brutas e ajustadas (OR) com os seus intervalos de confiança (IC) de 95% foram estimadas usando o método de regressão logística multivariada não-condicional. A idade (P < 0,001), o sexo (P < 0,001), o índice de massa corporal (P = 0,04), o acompanhamento nutricional (P = 0,001), o tempo de prática no esporte (P = 0,001) e a presença de dor no tendão (P < 0,001) foram identificados como fatores de risco para a tendinopatia em atletas. O genótipo e o alelo variante do polimorfismo FcRL3 -169T>C (OR: 2,02, IC 95%: 1,00 - 4,09 e OR: 1,44, IC 95%: 1,02 - 2,04, respectivamente) e o haplótipo TNF-α TCA (-1031 T>C, - 857C>T, -308G>A) (OR: 2,65, IC 95%: 1,32 - 5,30) foram associados positivamente com o desenvolvimento da tendinopatia em atletas. Estes resultados evidenciam que polimorfismos dos genes envolvidos com a atividade das células Treg e a expressão de TNF-α estão associados com a suscetibilidade da tendinopatia e podem contribuir para a identificação de novos alvos terapêuticos ou programas de prevenção da doença. | en_US |

Abstract | Tendinopathy is a degenerative disease of the tendon, caused by high levels of mechanical stress or accumulation of microtraumas that cause disorganization of the tendon tissue. It is a multifactorial disease, which affects 2 - 9% of the general population, being frequent in individuals who work in environments susceptible to ergonomic risks, being considered a work-related musculoskeletal disorder (Dort). In professional athletes, who are highly exposed to overuse movements, tendinopathy can occur in up to 50% of individuals. The main complaint of patients with the lesion is chronic pain in the affected tendon, which is the result of inflammatory processes in the early stages of lesion development. Studies shown that regulatory T cells (Treg), influenced by the type 3 forkhead-box protein (FOXP3) and by the transcription factor homologous to classic FcyIII receptors (FcRL3), modulate the immune response to repair a tendon injury. Moreover, the high gene expression of tumor necrosis factor type alpha (TNF-α) is found in degenerated tendons. Thus, this study aimed to evaluate the influence of polymorphisms of the FcRL3, FOXP3 and TNF-α genes in the development of tendinopathy. This is an analytical case-control study involving 270 Brazilian athletes, divided into diagnosed cases of tendinopathy and controls. Single nucleotide polymorphisms (SNPs) FcRL3 -169T>C, FOXP3 -2383C>T and TNF-α (-1031 T>C, -857C>T, -308G>A) were determined by the polymerase chain reaction in time (PCR), using the TaqMan system. The odds ratios (OR) with their 95% confidence intervals (CI) were estimated using the non-conditional multivariate logistic regression method. The age (P<0.001), sex (P<0.001), body mass index (P=0.04), nutritional monitoring (P=0.001) and practice time in sport (P=0.001) and the presence of tendon pain (P<0.001) were identified as risk factors for tendinopathy in athletes. The variant genotype and allele of the FcRL3 -169T>C polymorphism (OR: 2.02, 95% CI: 1.00- 4.09 and OR: 1.44, 95% CI: 1.02-2.04, respectively) and the TNF-α TCA (-1031 T>C, - 857C>T, -308G>A) haplotype (OR: 2.65, 95% CI: 1.32-5.30) were positively associated with development tendinopathy in athletes. These results show that polymorphisms of the genes involved with the activity of Treg cells and the expression of TNF-α are associated with the susceptibility of tendinopathy and may contribute to the identification of new therapeutic targets or programs for disease prevention. | en_US |
