Objetivos: Conhecer atitudes e percepções relacionadas à disfunção erétil (DE) em médicos que atendem pacientes ambulatoriais no Brasil. Métodos: Elaboramos um questionário para coletar dados sobre atitudes relacionadas à DE, o qual foi distribuído aleatoriamente a 6.000 médicos de 24 Estados, representando todas as regiões do Brasil. Após preencher esta parte do questionário, cada um dos médicos entrevistou 20 pacientes do sexo masculino, numa consulta médica de rotina e os interrogou sobre a presença de DE e seus correlatos. Posteriormente, esses médicos completaram o preenchimento da segunda parte do questionário, a qual avaliava a experiência de participação no estudo. Resultados: 4.377 (73%) médicos responderam ao questionário por completo, 81,3% homens e 18,7% mulheres, com idade média de 44,5 ± 9,8 anos. A maioria eram clínicos gerais (41%) e cardiologistas (19%). Apenas 10,7% dos médicos relataram que sempre perguntavam aos pacientes sobre DE e 23,1% nunca ou raramente o faziam. Os médicos consideraram que a maioria dos pacientes teve boa (58%) ou muito boa (21%) receptividade às perguntas sobre DE. Após entrevistar a amostra de pacientes, os médicos de nosso estudo concordaram com as afirmações: "DE é mais comum do que eu pensava" (74%); "conversar sobre DE foi mais fácil do que eu imaginava" (79%); e "perguntas sobre sexualidade deveriam fazer parte da anamnese de todos os pacientes" (89%). A prevalência de DE nos 71.503 pacientes entrevistados foi 53,5%. Conclusões: DE é uma condição comum, acometendo aproximadamente metade dos homens atendidos em consultórios médicos no Brasil. Apesar disso, apenas 11,1% dos médicos e 8,7% das médicas, sistematicamente, perguntam aos seus pacientes sobre possíveis problemas sexuais durante a consulta médica. Essa investigação sistemática de disfunções sexuais deveria fazer parte da avaliação médica de rotina. A maioria dos homens no Brasil concorda com essa prática e espera que seu médico discuta problemas sexuais com eles.
Abstract
Objectives: To determine attitudes and perception regarding erectile dysfunction (ED) among physicians attending outpatients in Brazil. Methods: We designed a questionnaire to collect data on attitude towards ED and delivered it to a random sample of 6,000 physicians from 24 states representing all regions in Brazil. After self-completing this part of the questionnaire, each doctor was asked to interview 20 consecutive male patients attending a routine office consultation, and query them about presence of ED and its correlates. Then, the physicians completed the second part of the questionnaire, assessing how their experience was. Results: Overall 4,377 (73%) doctors returned a completed questionnaire, 81.3% males and 18.7% females, with a mean age of 44.5 (±9.8) years. The majority were general practitioners (41%) or cardiologists (19%). Only 10.7% of the doctors reported they always asked their patients about erectile difficulties and 23.1% never or rarely do that. Physicians felt that the majority of the patients received the survey on ED well (58%) or very well (21%). After surveying their sample of patients about ED, Doctors in our sample strongly agreed or agreed with the following statements: "ED is more prevalent than I thought" (74%); "talking about ED was easier than I anticipated" (79%); and "questions on sexual dysfunction should always be asked to patients" (89%). Among the 71,503 patients interviewed, the prevalence of ED was 53.5%. Conclusions: ED is a common problem, affecting about half of the patients attending a medical consultation in Brazil. Despite that, only 11.1% and 8.7% of male and female doctors, respectively, always ask their patients about sexual problems during a routine consultation. The systematic assessment of sexual problems should be part of a routine office consultation. Most men in Brazil agree with this practice and expect their doctors to discuss about sexual problems with them.
