O texto destaca a importância de questionar a realidade e ser autocrítico em relação a conceitos e ações como uma forma de liberdade indispensável. Reflete sobre a celebração dos 59 anos da Escola Nacional de Saúde Pública e dos 25 anos da Constituição que instituiu o SUS, destacando o papel do sanitarista Sergio Arouca. Aborda discussões sobre a Reforma Sanitária, a privatização do SUS e a ausência dos movimentos sociais na formulação de políticas públicas. Diversos especialistas criticam a redução de recursos para a saúde desde 1988, resultando em um modelo considerado "anti-SUS". A falta de financiamento, integralidade e a privatização de serviços são apontadas como desafios. Há chamados para a democratização na gestão das políticas públicas, a responsabilização do Estado pelo crescimento do setor privado da saúde e a necessidade de um sistema que reduza as desigualdades.
