Abstract in Portuguese
A doença de Chagas representa um sério problema para a saúde pública, causando grandes impactos econômicos e sociais. Os estudos referentes à essa patologia apresentam várias lacunas incluindo a necessária identificação de novas terapias mais seletivas (em especial para portadores crônicos) e de ferramentas laboratoriais a serem utilizadas para diagnóstico e monitoramento de falha terapêutica e progressão das patologias clínicas, das quais a mais evidente é a cardiomiopatia chagásica crônica. Nesse contexto, o objetivo desse trabalho foi monitorar no período de 11 anos (1999-2010/2011), a evolução da parasitemia, dos níveis séricos e dos sinais clínicos em pacientes portadores da infecção crônica pelo Trypanosoma cruzi, residentes no Distrito Sanitário de Rio Verde (Mato Grosso do Sul). Para tais fins, realizamos (i) ensaios moleculares de PCR qualitativa e PCR quantitativa (qPCR) para acompanhamento da parasitemia e carga parasitária, (ii) testes sorológicos de imunofluorescência indireta (IFI), ELISA recombinante e hemaglutinação (HAI) para acompanhamento dos níveis séricos de Ig G, (iii) além da avaliação clínica pela anamnese e pelo eletrocardiograma de repouso (ECG) para avaliação da evolução da cardiopatia
Na primeira parte do estudo (1999) 110 portadores crônicos de doença de chagas foram recrutados e submetidos aos exames/técnicas mencionados anteriormente. Destes, devido a recusa/contra indicação médica, somente 63 foram tratados por 60 dias com benznidazol (Bz) na dose de 5mg/kg/dia (grupo Bz), restando 47 indivíduos que constituíram o grupo sem terapia (grupo não tratado - NT). Na segunda parte do estudo (2010/2011), após ativo recrutamento desta coorte inicial, 50 portadores consentiram em dar continuidade ao estudo, sendo 28 indivíduos tratados e 22 não tratados. O estudo longitudinal global (n=50) revelou queda de 75% na positividade da parasitemia detectada pela PCR qualitativa. Quando discriminamos esta redução entre os grupos \201Ctratado\201D e \201Cnão tratado\201D não observamos diferenças na evolução da carga parasitária. A quantificação da carga parasitária por qPCR dos indivíduos que mantiveram-se positivos em 2010/11 para PCR convencional também não detectou diferenças entre ambos os grupos (Bz e NT). A avaliação sorológica por IFI mostrou uma redução significativa dos títulos de Ig G anti-T. cruzi no conjunto dos portadores avaliados (n=50) no período, sendo a redução somente significativa no grupo de pacientes tratados pelo Bz
Porém, os ensaios de ELISA e de HAI demonstraram aumento dos níveis de Ig G anti-T. cruzi no curso do acompanhamento dos portadores sendo apenas significativo para o grupo tratado com Bz. Com relação a evolução clínica, observamos que a ausência de óbitos em portadores com ECG normal em 1999 foi indicativa de bom prognóstico. Por outro lado, houve menor progressão de cardiopatia no grupo Bz em comparação ao não tratado, indicativo de efeito protetor da terapia etiológica. Nossos dados sugerem que o uso de Bz possa contribuir para inibir/retardar a progressão da cardiopatia crônica induzida pela infecção por T. cruzi resultando em aumento de sobrevida e melhoria da qualidade de vida dos portadores, ainda que os indicadores sorológicos e moleculares aplicados no presente trabalho não tenham revelado a erradicação do parasitismo pelo benznidazol
Abstract
Chagas disease represents a serious threat to publi
c health, causing major
economic and social impacts.
Studies regarding this pathology present several
shortcomings including the necessary identification
of new therapies more selective
(especially for chronic patients) and laboratorial
tools to be used for diagnosis and
follow-up of treatment failure and to monitor the p
rogression of clinical pathologies,
being chronic Chagas cardiomyopathy the most eviden
t. In this context, the aim of
the present work was to monitor in a 11-year period
(1999-2010/2011), the evolution
of parasitaemia, serum levels and clinical signs in
patients with chronic infection with
Trypanosoma cruzi
, residents in Rio Verde Sanitary District (Mato Gr
osso do Sul).
For such purposes, we performed (i) molecular assay
s for qualitative PCR and
quantitative PCR (qPCR) to monitor the parasitaemia
and parasite load, (ii)
serological tests including indirect immunofluoresc
ence (IIF), recombinant ELISA and
hemagglutination (HAI) for monitoring the levels of
IgG in sera and, (iii) clinical
assessment by history and resting electrocardiogram
(ECG) to evaluate the
progression of heart disease. In the first part of
the study (1999) 110 patients with
chronic Chagas disease were recruited and subjected
to the tests/techniques
mentioned above. Of these, due to refusal or agains
t medical advice, only 63 were
treated for 60 days with benznidazol (Bz) at a dose
of 5mg/kg/day (Bz group) and 47
subjects constituted the group that do not receive
treatment (untreated group - NT).
In the second part of the study (2010/2011), after
active recruitment of this initial xi
cohort, 50 patients consented to continue the study
, being 28 treated subjects and 22
that not undergo therapy with Bz. The global longit
udinal study (n = 50) showed a
reduction of 75% in the positivity of parasitaemia
detected by qualitative PCR. When
this reduction was discriminated between "treated"
and "untreated" groups, no
differences in the evolution of parasite load were
observed. The quantification of
parasite load by qPCR from individuals who remained
positive in 2010/11 for
conventional PCR also showed no differences between
both groups (Bz and NT).
The serological evaluation by IIF showed a signific
ant reduction in the IgG anti-
T.
cruzi
titers in the totality of patients evaluated (n =
50) in the period, with significant
reduction observed only in the Bz treated group. Ho
wever, ELISA assays and HAI
demonstrated increased levels of IgG anti-
T. cruzi
in the course of monitoring
patients, being significant only for the Bz treated
group. With regard to the clinical
outcome, we observed that the absence of deaths in
patients with normal ECG in
1999 was an indicative of good prognosis. On the ot
her hand, there was less
progression of heart disease in the Bz treated grou
p compared to the untreated
patients, suggesting a protective effect of the eti
ologic therapy. Our data suggest that
the use of Bz may contribute to inhibit/slow the pr
ogression of chronic heart disease
induced by
T. cruzi
infection resulting in increased survival and impr
oved quality of
life for the patients, although the serological and
molecular indicators used in the
present work have not revealed the eradication of p
arasitism by benznidazol.
