O vírus Chikungunya (CHIKV) atualmente é um dos arbovírus de grande relevância médica, associado a casos de morbidade e/ou mortalidade, principalmente no Brasil. Assim, este projeto objetivou a otimização (às condições locais) e implementação do sistema de transcrição reversa de parte do genoma viral, seguido de amplificação em cadeia da polimerase em tempo real (RT-qPCR), com base no protocolo desenvolvido pelo Centers for Disease Control and Prevention (CDC). A RT-qPCR foi avaliada através de uma curva padrão utilizando um transcrito e o limite de detecção (LOD) foi de 39 cópias/μl. O RNA das amostras de soro do estudo foi extraído e a RT-qPCR foi realizada. Das 151 amostras testadas, 46,4% foram negativas e 53,6% foram positivas, com uma maior capacidade de detecção pela RT-qPCR entre 0-4 dias pós infecção, em comparação ao teste de captura da imunoglobulina M (IgM) (MAC-ELISA). Análises da carga viral foram conduzidas: variação entre 101 a 108 cópias/μl e viremia ≥103 cópias/μl foram observadas na maioria das amostras positivas (64,2%). Cargas virais ≥107 cópias/μl foram evidenciadas nas faixas criança e idoso. De 0-2 dias pós infecção, foram também detectadas altas cargas virais (≥107 cópias/μl) nas amostras positivas. O ensaio da RNAse P humana (RNP), controle endógeno, foi otimizado apresentando amplificações em todas as amostras do estudo. A RNP apresentou níveis praticamente constantes entre os dias pós infecção e também ao correlacionar com a carga viral dos indivíduos positivos. Um ensaio multiplex com detecção simultânea dos alvos CHIKV e RNP foi estabelecido e avaliado preliminarmente apresentando um LOD de 390 cópias/μl. Uma concordância percentual de 88,88% em comparação aos resultados da RT-qPCR simples foi observada, sem diferença estatística entre os testes (p=1). Pode-se concluir que ambos os testes (formato simples e multiplex) foram sensíveis, permitindo a detecção e quantificação do CHIKV, com potencial uso na rotina laboratorial.
Abstract
Currently, one of the most relevant arbovirus is the Chikungunya virus (CHIKV) associated with morbidity and/or mortality cases, especially in Brazil. Thus, this project aimed to optimize (under local conditions) and introduce the RT-PCR in real time assay (RT-qPCR), based on the protocol developed by the Centers for Disease Control and Prevention (CDC). RT-qPCR was evaluated by transcript dilutions on a standard curve and the limit of detection (LOD) was 39 copies/μl. The RNA from the serum clinical samples was extracted and the RT-qPCR was performed. Out of 151 samples tested, 46.4% were negative and 53.6% were positive. A higher detection sensitivity by RT-qPCR was observed between 0-4 days post infection, compared to the immunoglobulin M (IgM) capture assay (MAC-ELISA). Analysis with viral load were performed: variation from 101 to 108 copies/μl and viremia of ≥103 copies/μl were observed in most of positive samples (64.2%). Viral loads of ≥107 copies/μl were described on children and elderly people. From 0-2 days post infection, high viral loads (≥107 copies/μl) were detected among positive samples. The human RNase P assay (RNP), endogenous control, was optimized. All samples were detected by the RNP assay, showing similar levels between the days post infection and in comparison, to viral load of positive individuals. A multiplex assay with simultaneous detection of CHIKV and RNP targets was optimized and preliminarily evaluated with a LOD of 390 copies/μl. A percentage of agreement of 88.88% compared to singleplex RT-qPCR results was observed, with no statistical difference between the tests (p=1). It is possible to infer that both tests (singleplex and multiplex) demonstrated sensibility, allowing detection and quantification of CHIKV, with potential use on laboratory routine.
