O Praziquantel (PZQ) é um anti-helmíntico amplamente utilizado para tratamento da esquistossomose. Esta doença afeta mais de 200 milhões de pessoas ao redor do mundo, principalmente países em desenvolvimento, incluindo 90 milhões de crianças na idade escolar e pré-escolar. PZQ está presente na lista de medicamentos infantis essenciais da OMS (Organização Mundial da Saúde). Por este motivo, várias companhias farmacêuticas formaram um consórcio, entre elas Farmanguinhos, para o desenvolvimento de uma nova formulação dirigida a este grupo particular de pacientes. Para conceder o registro de novas formulações e o seu direito de comercialização, a agência regulatória brasileira, ANVISA, exige que se faça a avaliação da qualidade do medicamento, sendo relevante o conhecimento de sua estabilidade química e física até o fim de seu prazo de validade. O monitoramento dos produtos de degradação está incluído nesta avaliação, para garantir a segurança, biodisponibilidade e eficiência da droga. Logo, o estudo de degradação forçada é realizado de modo a submeter o princípio ativo e produto final a condições de estresse extremo para se formar vários produtos de degradação com o intuito de se desenvolver um método analítico indicador de estabilidade (MIE), o qual será utilizado para o controle daquelas possíveis impurezas na rotina dos estudos de estabilidade. O objetivo deste trabalho foi compilar dados na literatura de metodologias analíticas e condições de degradação do PZQ a fim de apresentar uma proposta para condução do estudo de degradação forçada do fármaco, placebo e produto e o desenvolvimento do MIE.
Abstract
Praziquantel (PZQ) is an anthelmintic widely used for the treatment of
schistosomiasis. This disease affects more than 200 million people worldwide,
mainly developing countries, comprising 90 million pre-school and school-age
children. PZQ is included in the WHO (World Health Organization) List of
Essential Medicines for Children. For this reason, a Consortium of
Pharmaceutical Companies was formed, including Farmanguinhos, to develop a
new formulation of PZQ directed to this particular group of patients.
To grant the register of a new drug product (DP) and the right of
commercialization, the Brazilian regulatory agency, ANVISA, demands the
evaluation of quality and the knowledge of the chemical and physical stability of
the new DP until the end of its shelf life period. Monitoring of degradation products
is included in that evaluation in order to guarantee the new DP safety,
bioavailability and efficiency. Thus, the forced degradation study is performed in
order to expose the product to extreme stress conditions to produce a number of
degradation products that will be used in the development of an analytical stability
indicating method (SIM), which is employed to investigate those impurities in the
routine of stability studies. The goal of this work was to compile data in the
literature for analytical methodologies and degradation conditions of PZQ in order
to present a proposal to conduct the degradation study of the drug substance,
placebo and drug product to develop the SIM method.
