AVALIAÇÃO DA EXPRESSÃO DE CITOCINAS E DA ENZIMA ÓXIDO NÍTRICO SINTASE INDUZIDA - INOS - NO MÚSCULO CARDÍACO DE MACACOS RHESUS (MACACA MULATTA) EXPERIMENTALMENTE INFECTADOS PELO TRYPANOSOMA CRUZI: ASSOCIAÇÃO ENTRE A EVOLUÇÃO CLÍNICA E A EXTENSÃO DAS LESÕES CARDÍACAS

A doença de Chagas ou tripanossomíase americana, causada pelo protozoário Trypanosoma cruzi, é uma doença de caráter multifatorial que envolve basicamente a persistência do parasita, o processo inflamatório e a fibrose tecidual. O objetivo deste trabalho foi o de avaliar a presença tecidual de citocinas pró e anti-inflamatórias e da expressão da iNOS através da imunohistoquímica e associar os resultados com as análises dos perfis clínicos desenvolvidos por nossa coorte de macacos rhesus em 25 anos de infecção experimental, além das análises histopatológicas e morfométricas. A avaliação clínica foi realizada através do levantamento dos dados dos exames radiográficos, eletrocardiográficos e ecocardiográficos realizados neste período. Para categorizar os animais, adotamos os parâmetros de classificação clínica indicados pelo Consenso Brasileiro em Doença de Chagas. Todos os animais apresentaram IgG específica para T. cruzi, com exceção do animal 99 que acreditamos ter apresentado cura espontânea. Foi possível verificar uma dissociação entre a presença do processo inflamatório, de caráter multifocal com predominância de células mononucleares e as alterações cardíacas na coorte avaliada A análise histopatológica revelou uma hipertrofia miocárdica de caráter multifocal e degeneração vacuolar em todos os animais infectados sob variados graus de acometimento com exceção do animal 99. Somente o animal 95 apresentou foco de necrose de liquefação em átrio esquerdo, desprovido de atividade inflamatória local. Nenhum elemento parasitário típico foi encontrado nas amostras cardíacas avaliadas. O padrão de distribuição da fibrose foi avaliado através da coloração de Tricomio de Masson e foi caracterizada como sendo multifocal nos ventrículos e difuso no átrio esquerdo dos animais infectados. Evidenciamos um aumento significativo da densidade de volume dos vasos em ventrículo direito dos animais infectados em relação aos animais controles sadios (3,7 \F0B1 0,1%; 1,6 \F0B1 0,5%, p=0,05). A avaliação da presença de citocinas e da iNOS em ventrículo esquerdo mostrou uma forte tendência à marcação de células em meio a infiltrados inflamatórios e em regiões perivasculares dos animais infectados avaliados. Concluímos que este modelo é satisfatório para o estudo clínico da doença de Chagas aguda e crônica.
Abstract
Chagas\2019 disease or american tripanosomiasis, caused by the protozoan Trypanosoma cruzi is a multifactorial disease that involves basically parasite persistence, inflammatory process and fibrosis. The aim of this work were to evaluate the tissue presence of inflammatory and anti inflammatory cytokines and the iNOS expression by immunohistochemistry analysis and associate the results with the clinical follow up presented by our coort of rhesus monkeys in 25 years of experimental infection, and also the histopathological and morphometrical analysis. The clinical evaluation was performed analyzing the features found in the X-ray, eletrocardiogram and ecocardiogram exams in the period of infection. For the clinical characterization, we adopted the Clinical Classification proposed by the Brazilian Consensus on Chagas Disease. All the subjects presented specific anti- T. cruzi IgG, except animal 99, which we believe to develop an spontaneous cure. Microscopically, the findings suggested a mutifocal hypertrophy and vacuolar degeneration in all infected subjects The inflammatory process was composed mainly by mononuclear cells in variable degrees of damage. It was possible to verify an dissociation between the presence of inflammation and the cardiac alterations, however the presence of iNOS and IFN\F067 was proportional to the inflammatory damage found in the studied slices. The distribution of fibrosis was characterized by a difuse process in the atrium and multifocal in ventricles of the infected animals. A significant increase of the vascular density of volume in right ventricles was found in the infected animals in relation with the healthy control ones (3,7 \F0B1 0,1%; 1,6 \F0B1 0,5%, p=0,05). The evaluation of the presence of cytokines and iNOS in left ventricle showed a strong tendency of marked cells in inflammatory infiltrates and in perivascular regions of the infected animals. We concluded our present work indicating that the model is satisfactory for clinical studies of acute and chronic Chagas disease.
