Abstract in Portuguese | Os antivirais de ação direta (DAA) foram incorporadas para o tratamento da hepatite C crônica pelo Protocolo de Diretrizes Terapêuticas do Ministério da Saúde (PCDT-HepC) desde 2015. Devido à natureza quasiespécies do vírus da hepatite C (VHC), tempo de infecção, genótipo, mutações de resistência aos antivirais podem emergir levando a falha no tratamento. OBJETIVO O presente estudo pretendeu rastrear e monitorar substituições associadas a resistência (RASs) aos DAAs em pacientes com hepatite C crônica. MÉTODO O estudo possui desenho descritivo de corte-transversal. Ao todo, 204 participantes foram recrutados, hemofílicos da Fundação HEMOBA (n = 23) e pacientes do Ambulatório de Hepatologia do Complexo HUPES (n = 181). RESULTADOS Após testes moleculares e análise das sequências, identificou-se prevalência total de RAS de 51% (94/184, 95% IC 44% - 59%): RAS NS3 45% (62/137, 95% IC 37% - 54%), RAS NS5A 27% (30/113, 95% IC 19% - 36%), RAS NS5B 2% (2/124, 95% IC 0% - 6%). Quando a predição da RAS incluiu apenas as drogas licenciadas e os genótipos do VHC circulantes no Brasil, 17 pacientes apresentaram variantes associadas a resistência (RAVs) para regiões NS3 e NS5A. Nenhum paciente apresentou RAV para a região NS5B. CONCLUSÕES A identificação precoce e rastreio das mutações de resistência podem auxiliar na conduta clínica prevenindo a emergência de resistência e falha terapêutica. | pt_BR |

Abstract | Direct-acting antivirals (DAAs) have been incorporated into the
treatment of chronic hepatitis C by the Ministry of Health Therapeutic Guidelines Protocol (PCDT-HepC) since 2015. Due to the quasi-species of the hepatitis C virus (HCV), infection time, genotype, antiviral resistance mutations may emerge leading to treatment failure. OBJECTIVE The present study aimed to track and monitor resistance-associated substitutions (RASs) to DAAs in patients with chronic hepatitis
C. The study has descriptive cross-sectional design. METHODS A total of 204 participants were recruited, hemophiliacs from HEMOBA Foundation (n = 23) and patients from the Hepatology Clinic of HUPES Complex (n = 181). RESULTS After molecular testing and sequence analysis, a total prevalence of RAS of 51% (94/184, 95% CI 44% - 59%) was identified: RAS NS3 45% (62/137, 95% CI 37% - 54%), RAS NS5A 27% (30/113, 95% CI 19 - 36%), RAS NS5B 2% (2/124, 95% CI 0% - 6%). When the RAS prediction included only licensed drugs and HCV genotypes circulating in Brazil, 17 patients had resistance-associated variants (RAVs) for NS3 and NS5A regions. No patient presented RAV for the NS5B region. CONCLUSION Early identification and screening of resistance mutations may aid in clinical management preventing the emergence of resistance and therapeutic failure. | pt_BR |
