O padrão de morbimortalidade tem relação direta com o perfil sociodemográfico. Ao considerar os efeitos da transição urbana na transição demográfica, e desta na transição epidemiológica, é razoável supor que os espaços distintos rurais e urbanos tenham perfis diferentes de adoecimento e morte de suas populações. Objetivo do presente artigo é apresentar as taxas de mortalidade geral, e pelos 5 maiores grupos de causas de morte no país, segundo porte municipal: municípios de pequeno, médio e grande porte. Estudo ecológico com objetivo de apresentar as taxas de mortalidade geral, e por 5 grandes grupos de causas de morte no Brasil (doenças infecciosas e parasitárias, neoplasias, doenças cardiovasculares, doenças do aparelho respiratório e causas externas), de acordo com o aspecto urbano e rural. Os dados utilizados foram obtidos do Sistema de Informação de sobre Mortalidade, obtidos para o ano de 2015 e dados censitários (IBGE), referente ao ano de 2010, com projeção da população para 2015. O estudo apontou diferenças no padrão de mortalidade quanto aos grupos de doenças. As doenças cardiovasculares apresentaram maiores taxas. Já as menores taxas são as observadas entre as doenças infecciosas e parasitárias. Contudo, não houve linearidade de crescimento ou decréscimo das taxas para todos os grupos de doenças, e há diferenças importantes quando caracterizados os locais como urbanos e rurais. Verificou-se um excesso de mortes que afetam as regiões e setores sociais mais desfavorecidos. É necessário reafirmar políticas equitativas que considerem diferenças estruturais em distintos ambientes, considerando demandas específicas de saúde.
