Abstract in Portuguese | O objetivo desta tese é contribuir com o conhecimento sobre a mortalidade e a assistência ao infarto agudo do miocárdio (IAM) no Município do Rio de Janeiro de forma a possibilitar o aprimoramento das políticas de saúde voltadas para a melhoria das condições gerais de vida e saúde. Os resultados são apresentados em três artigos. O primeiro trata da qualidade dos dados sobre mortalidade hospitalar por infarto agudo do miocárdio, em dois hospitais do Sistema Único de Saúde no Município do Rio de Janeiro, através da comparação entre prontuários, declarações de óbitos e formulários de autorização de internação hospitalar. O segundo artigo aborda a sobrevida de pacientes com diagnóstico de IAM internados em hospitais públicos e contratados pelo Sistema Único de Saúde. O terceiro artigo estuda a distribuição espacial do IAM, tendo os bairros da cidade do Rio de Janeiro como unidade de análise. Os homens concentram mais da metade dos óbitos e as maiores taxas de mortalidade por IAM, no entanto apresentam uma sobrevida mais elevada do que as mulheres. Verificou-se uma sobrevida mais elevada após o IAM entre as unidades hospitalares que dispunham de unidade de terapia intensiva e um pior prognóstico entre aquelas que apresentaram um baixo volume de internações por IAM (inferior a 25 ao ano). A natureza jurídica dos hospitais mostrou-se relacionada com a condição social dos residentes da área onde se localizam e com a qualidade da assistência prestada. A análise do tempo de sobrevida de pacientes com IAM evidenciou um efeito protetor dos hospitais federais e universitários em relação aos hospitais municipais. O pior prognóstico para o infarto foi observado nos pacientes internados nos hospitais estaduais. Os padrões de mortalidade por IAM são marcados por contrastes geográficos, que reproduzem as desigualdades sociais observadas entre as áreas (bairros e APs) no Município do Rio de Janeiro. A grande vantagem do uso das bases de mortalidade e de internação hospitalar está na sua rápida disponibilidade, economia de tempo e recursos. A qualidade da informação, entretanto, é um problema a ser enfrentado. | pt_BR |
