Abstract | Introduction: Infective endocarditis (IE) is a systemic infection most caused by bacteria, with high lethality in absence of treatment. The genus Bartonella is one of the causative agents associated with blood culture-negative infective endocarditis (BCNIE), in which no pathogen can be detected by laboratory methods, mainly blood culture. Of the 14 species of Bartonella associated with BCNIE in humans and domestic animals, B.henselae is the most prevalent in both groups. This species is also known as the main cause of cat scratch disease (CSD), a neglected zoonotic disease in Brazil. BCNIE cases are a classic example of zoonosis that One Health approach must be applied, considering domestic cats and dogs and their ectoparasites can be hosts of B.henselae. The risk of infection is not restricted to the human population, as dogs and cats can also develop cardiac complications related to infection by Bartonella. Methods: After two patients with IE were admitted to Pedro Ernesto University Hospital (UERJ/RJ), a multidisciplinary team of physicians, biologists, and veterinarians performed fieldwork in patient’s homes to evaluate healthy living conditions with the collection of ectoparasites and blood samples from domestic animals for laboratory analysis. Serum samples from patients and their pets were evaluated by IFA for IgGantibodies against Bartonella spp. (cut-off:>64). Fluids and tissue samples were submitted to DNA extraction and PCR with specific primers (gltA,groEL, and htrA) to identify Bartonella spp. DNA. Results: Serum samples from patients (>2048~32000) and their pets (>128) were reactive with high titers to Bartonella IgG-antibodies. DNA sequences generated from human samples were similar to B.henselae (97~100%). Both patients showed unhealthy conditions, cats and dogs frequently were found in dirty spaces or sharing living spaces like beds. Conclusions: Professionals of human health, animal health, and environmental health should be alert for IE caused by Bartonella, an invisible and neglected pathogen in Brazil. | pt_BR |
