Abstract in Portuguese | A coinfecção leishmaniose visceral (LV)/HIV é uma doença grave e letal. O estudo dos polimorfismos genéticos, dosagem de citocinas e expressão de miRNAs são importantes na compreensão da doença, principalmente no aspecto imunogenético. Assim, o objetivo desse estudo foi investigar polimorfismos genéticos, dosagem de citocinas (Th1/Th2) e expressão dos miRNAs na coinfecção. Um estudo transversal foi desenvolvido com grupos de comparação: LV/HIV e HIV. Os indivíduos HIV foram submetidos ao diagnóstico laboratorial da LV, sendo também empregados métodos de tipagem genética na caracterização molecular da espécie Leishmania (L.) infantum chagasi. Foram analisados DNAs extraídos de sangue total (110 LV/HIV e 199 HIV): polimorfismos nos genes [SLC11A1 (rs3731865), LECT2 (rs248158, rs2526146, rs2526152, rs30740, rs4976360), CCL1 (rs159271, rs159273), CCL16 (rs2063979, rs854680) e IL4 (rs2070874)]; e dosagem sérica (65 LV/HIV e 48 HIV): citocinas (IFN-γ, TNF, IL-2, IL-4, IL-6 e IL-10) e miRNAs (mir182, mir210 e mir122). Entre todos os SNPs estudados, os modelos genéticos dominante (p=0,0206, OR 1,8126, IC 95% 1,1050-2,9727) e alélico (p=0,0347, OR 1,5645, IC 95% 1,0307-2,3747) confirmaram associação do polimorfismo do gene SLC11A1 (rs3731865) à coinfecção. Cinco SNPs de SLC11A1 (rs6734861, rs13011285, rs17228995, rs2276631, rs2290708), proximais ao rs3731865, estavam em desequilíbrio de ligação (r2>0,6). Na análise multivariada, a susceptibilidade à LV/HIV foi associada ao T CD4+ (p=0,0030, OR 0,9980, IC 95% 0,9970-0,9990) e o genótipo GG em SLC11A1 (p=0,0050, OR 3,0395, IC 95% 1,4065-6,5789), com o alelo G considerado fator de risco. Nas análises de correlação, as citocinas obtiveram correlação positiva entre si, com níveis séricos de TNF e IL-4 (p<0,0001 cada) mais elevados no LV/HIV. IL-4 foi relacionada à carga viral no grupo LV/HIV (r=0,46) e o miR-182 estava mais expresso no HIV (p=0,009). Assim, o polimorfismo do gene SLC11A1 (rs3731865) foi associado à LV/HIV, sugerindo a existência de um mecanismo genético envolvido na susceptibilidade para LV em pacientes HIV. Destaca-se o papel de TNF, IL-4 e o miR-182 como prováveis biomarcadores séricos na coinfecção. Dessa forma, os resultados apresentados são fundamentais para a compreensão do papel da imunogenética no desenvolvimento da coinfecção LV/HIV . | en_US |

Abstract | Visceral leishmaniasis (VL)/HIV co-infection is a serious and lethal disease. Genetic polymorphism studies, cytokine dosage and miRNA expression are important to understand the disease, mainly in the immunogenetic aspect. Thus, the aim of this study was to investigate genetic polymorphisms, cytokine dosage (Th1/Th2) and miRNA expression in co-infection. A cross-sectional study was conducted with comparison groups: VL/HIV and HIV. HIV individuals were submitted to laboratory diagnosis of VL, and genetic typing methods were also performed to molecular characterization of Leishmania (L.) infantum chagasi specie. Whole blood extracted DNAs (110 VL/HIV and 199 HIV) were analyzed: polymorphisms in the genes [SLC11A1 (rs3731865), LECT2 (rs248158, rs2526146, rs2526152, rs30740, rs4976360), CCL1 (rs159271, rs159273), CCL16 (rs2063979, rs854680) and IL4 (rs2070874)]; and serum dosage (65 VL/HIV and 48 HIV): cytokines (IFN-γ, TNF, IL-2, IL-4, IL-6 and IL-10) and miRNAs (mir182, mir210 and mir122). Among all SNPs studied, the dominant (p = 0.0206, OR 1.8126, 95% CI 1.1050-2.9727) and allelic (p = 0.0347, OR 1.5645, 95% CI 1.0307-2.3747) genetic models confirmed association of the SLC11A1 (rs3731865) polymorphism with co-infection. Five SLC11A1 SNPs (rs6734861, rs13011285, rs17228995, rs2276631, rs2290708), proximate to rs3731865, were in linkage disequilibrium (r2>0.6). In multivariate analysis, susceptibility to VL/HIV was associated with CD4 + T (p = 0.0030, OR 0.9980, 95% CI 0.9970-0.9999) and GG genotype in SLC11A1 (p = 0.0050, OR 3.0395, 95% CI 1.4065-6.5789), with the G allele considered as a risk factor. In correlation analyzes, cytokines were positively correlated with higher serum levels of TNF and IL-4 (p <0.0001 both) in VL/HIV. IL-4 was related to viral load in the VL/HIV group (r=0.46) and the miR-182 was over expressed in HIV (p=0.009). Therefore, the SLC11A1 polymorphism (rs3731865) was associated with VL/HIV, suggesting a possible genetic mechanism involved in susceptibility to VL in HIV patients. We highlight the role of TNF, IL-4 and miR-182 as possible serum biomarkers in co-infection. This way, the results presented herein are fundamental for understanding the role of immunogenetics in the VL/HIV co-infection development . | en_US |
