Abstract in Portuguese | Introdução: Classicamente o escore Child Pugh (CP) é utilizado na prática clínica e nos grandes ensaios clínicos randomizados para avaliação da função hepática no carcinoma hepatocelular (CHC). Em 2015 foi definido pela primeira vez na literatura o escore albumina-bilirrubina (ALBI). O objetivo desse estudo é caracterizar a população dos pacientes com CHC acompanhados em um serviço baiano de Oncologia e comparar a sobrevida dos pacientes com CHC submetidos a terapia sistêmica em relação aos escores de ALBI e CP. Buscou-se ainda pesquisar entre os profissionais locais envolvidos no tratamento dos pacientes com CHC o conhecimento que eles tem do escore de ALBI bem como seu uso na prática clínica. Metodologia: Trata-se de um estudo observacional analítico, de coorte retrospetiva e prospectiva de pacientes com CHC tratados com terapia sistêmica nos últimos cinco anos (janeiro de 2016 a junho de 2021) em uma clínica privada de Oncologia na Bahia (Clínica AMO). Foram avaliados 63 pacientes restrospectivamente, entretanto apenas 34 foram incluídos, pois 29 pacientes foram excluídos pela ausência de dados para o cálculo do escore CP e ALBI. Até o momento sete pacientes estão em acompanhamento na coorte prospectiva. Todos os dados foram obtidos por meio do prontuário eletrônico padronizado utilizado em todas as unidades da clínica AMO denominado SMART. Foram coletados os dados no início do tratamento sistêmico e no momento da progressão à primeira linha de tratamento. As variáveis contempladas foram organizadas em planilhas de Excel. Foi usada a seguinte fórmula (ALBI = (log10 bilirrubina 9 0,66) + (albumina x - 0,085)) para o calculo do ALBI. Usou-se o estimador de Kaplan-Meier para calcular a função de sobrevida ao longo do tempo de seguimento. As análises foram conduzidas na plataforma R e no SPSS versão 25 Aplicou-se um questionário via google docs para avaliar o conhecimento dos de 32 profissionais locais envolvidos no tratamento dos pacientes com CHC bem como o uso desse escore na prática clínica. Resultados: Constatou-se maior número de pacientes com CP A, ALBI 2 e BCLC C com idade mediana de 68,5 anos e predomínio de pacientes do sexo masculino. Na coorte retrospetiva 80% receberam tratamento com Sorafenibe enquanto na prospectiva 4 dos 7 pacientes foram tratados com imunoterapia. A sobrevida mediana geral dos pacientes na coorte retrospectiva com hepatocarcinoma foi de 334 dias (IC 95% 148 - 1048). Os pacientes com CP A e B tiveram uma sobrevida de 412 e 305 dias respectivamente. Por outro lado, os pacientes com ALBI escore 1 sobreviveram 334 dias e com ALBI e 2 tiveram uma sobrevida de 247 dias. As diferenças na sobrevida entre os grupos não foram significativas o que poderia ser explicado pelo número total de pacientes reduzido de apenas 34 pacientes no total, mas será interessante explorar com o estudo prospectivo em pacientes tratados com imunoterapia se existe diferença entre os escores nessa população. Após avaliação dos questionários do google docs notou-se o desconhecimento do ALBI entre 31% dos profissionais questionados e por outro lado apenas 37% dos profissionais fazem uso desse escore em sua prática clínica. Conclusão: O escore ALBI é representado por variáveis clínicas objetivas e maior facilidade de aplicação na prática médica, destaca-se como uma ferramenta promissora para pacientes com CHC. Será necessário aumentar o número de pacientes para observar se pode-se confirmar que os pacientes com ALBI 1 e 2 tem uma melhor sobrevida. Abstract | Introduction: Classically, the Child Pugh (CP) score is used in clinical practice and in large randomized clinical trials to assess liver function in hepatocellular carcinoma (HCC). In 2015, the albumin-bilirubin score (ALBI) was defined for the first time in the literature. The objective of this study is to characterize the population of patients with HCC followed up at an Oncology service in Bahia and to compare the survival of patients with confirmed HCC on systemic therapy in relation to ALBI and CP scores. We also sought to investigate among local professionals involved in the treatment of patients with HCC their knowledge of the ALBI score as well as its use in clinical practice. Methodology: This is an analytical, observational, retrospective and prospective cohort study of patients with HCC treated with systemic therapy in the last five years (January 2016 to June 2021) in a private Oncology clinic in Bahia (Clínica AMO). Sixty-three patients were retrospectively evaluated; however, only 34 were included, as 29 patients were excluded due to lack of data for calculating the CP and ALBI score. To date, seven patients are being followed up in the prospective cohort. All data were obtained through the electronic medical record used in all units of the AMO clinic called SMART. Data were collected at the start of systemic treatment and at the time of progression to first-line treatment. The contemplated variables were organized in Excel spreadsheets. The following formula (ALBI = (log10 bilirubin 9 0.66) + (albumin x - 0.085)) was used to calculate the ALBI. The Kaplan-Meier estimator was used to calculate the survival function over the follow-up time. Analyzes were conducted using the R platform and SPSS version 25. An instruction was applied via google docs to assess the knowledge of the 32 local professionals involved in the treatment of patients with HCC, as well as the use of this score in clinical practice Results: There was a greater number of patients with CP A, ALBI 2 and BCLC C with a median age of 68.5 years and a predominance of male patients. In the retrospective cohort 80% received sorafenib treatment while in the prospective cohort 4 out of 7 patients were treated with immunotherapy. Median overall survival of patients in the retrospective hepatocellular carcinoma cohort was 334 days (95% CI 148 - 1048). Patients with CP A and B had a survival of 412 and 305 days, respectively. On the other hand, patients with ALBI score 1 survived 334 days and with ALBI e 2 had a survival of 247 days. Differences in survival between groups were not expressions that could be explained by the total number of patients reduced to only 34 patients in total, but it will be interesting to explore with the prospective study in patients treated with immunotherapy if there is a difference between the scores in this population. After evaluating the google docs tests, it was noted that 31% of the professionals questioned did not know the ALBI and, on the other hand, only 37% of the professionals use this score in their clinical practice. Conclusion: The ALBI score is represented by objective clinical variables and greater ease of application in medical practice, it stands out as a promising tool for patients with HCC. It will be necessary to increase the number of patients to see if it can be confirmed that patients with ALBI 1 and 2 have better survival.
