Esta dissertação objetiva discutir as interpretações feitas por usuários hipertensos
quanto à experiência de ser cuidado, vivenciada em uma Unidade de Atenção Primária à
Saúde (UAPS), que tem como linha de cuidado a Estratégia Saúde da Família; no
município de Juiz de Fora, Minas Gerais. Para alcançar a dimensão da experiência
lançamos mão de uma abordagem qualitativa de cunho sócio-antropológico; entrevistas
abertas, observação participante e uso do diário de campo foram os instrumentos
empregados na coleta de dados. A revisão bibliográfica se pautou em trabalhos das
Ciências Sociais e da Saúde Pública. Na oportunidade visitamos autores como Marshall
Shalins, Paulo Freire e De Certeau, que nos emprestam argumentos na construção do
referencial teórico em que se discute: a Estratégia Saúde da Família, enquanto política
de cuidado; o cotidiano de cuidado em uma UAPS; o cuidado ao hipertenso; a relação
entre experiência e cultura; e o conhecimento das classes populares. O presente trabalho
de dissertação estrutura-se sobre três principais eixos: A etnografia do espaço de
cuidado, onde temos por proposta caracterizar a dinâmica de funcionamento da UAPS;
O contexto, caracterização das localidades em que vivem o usuário hipertenso;
Experiência e conhecimento, momento no qual desenvolvemos discussão quanto à
relação entre a experiência de ser cuidado e a construção do conhecimento. A
experiência do cuidado se constitui da relação dialética entre convenção e invenção,
entre ordem cultural e experiência individual. A biografia do usuário hipertenso, bem
como sua capacidade de invenção e criatividade são elementos que respondem pela
experiência de ser cuidado. O cuidado ao hipertenso implica na construção de diálogo
entre saberes, portanto, entre culturas. A mudança de hábitos, sugerida pelos
profissionais de saúde, relativos à alimentação, práticas de atividades físicas, bem como
ao tratamento médico, cria um processo de rediscussão das categorias culturais que
emprestam sentido à compreensão dos hipertensos quanto ao processo saúde e doença.
Como particularidade da sociedade brasileira, o SUS reproduz no espaço de cuidado,
compreendido aqui como a UAPS, seu sistema de relação social, reproduzindo, portanto
suas desigualdades, harmonias, conflitos e resistências. No espaço de cuidado, aspectos
relativos à cultura brasileira figuram em seu cotidiano, como a emocionalidade, a
caridade e a lei personalizada.
Abstract
This essay discusses the interpretations made by hypertensive patients about their
experience of being cared, lived in a Primary Health Care Unit (UAPS in Portuguese),
whose line of care is the Family Health Strategy, in the city of Juiz
de Fora, Minas Gerais. To achieve the proportion of the experiment we used socioanthropological qualitative approach; open interviews, participant observation and daily
records were the instruments used in data collection. The literature review was
based on the Social Sciences and Public Health Works. On the occasion we
visited authors such as Marshall Shalins, Paulo Freire and De Certeau, who gave us
arguments to build the theoretical framework, which discusses: the Family Health
Strategy as a policy of care, the daily care in a UAPS, care to hypertensive patients, the
relationship between experience and culture, and knowledge of the popular classes.
This dissertation is structured on three main areas: The ethnography of the area of
care, where we propose to feature the operation system of UAPS; The context,
characterization of the locations where hypertensive patients live ,experience and
knowledge, when we develop a discussion about the relationship between the
experience of being cared for and construction of knowledge. The experience of
care constitutes the dialectical relationship between convention
and invention, between cultural and individual experience. The biography of the
hypertensive patient, as well as its capacity for invention and creativity are key
elements that account for the experience of being cared. The care of hypertensive
involves building dialogue among knowledge, therefore, between cultures. The change
of habits, as suggested by health professional related to feeding practices, physical
activities, as well the medical treatment creates a process of renewed discussion of
the cultural categories that give meaning to the understanding
of hypertensive concerning the health and disease process. As a peculiarity of Brazilian
society, the Unified Health System (SUS) reproduces in the space
of care, understood here as the UAPS, its system of social relations, playing, so its
inequalities, harmony, conflict and resistance. In the area of care, aspects of Brazilian
culture appear in their daily lives, such as emotionality, charity and custom law.
