O processo de envelhecimento da população brasileira promoveu alterações nos padrões de saúde, levando
ao predomínio das doenças crônicas não transmissíveis, entre elas, o câncer. A comorbidade, definida
como a coexistência de duas ou mais doenças crônicas em um mesmo indivíduo, é uma condição
frequente entre os idosos com câncer e pode influenciar no diagnóstico, tratamento e prognóstico. Esta
dissertação teve como objetivo estimar a prevalência de comorbidade em idosos com câncer, analisar os
fatores a ela associados e avaliar a confiabilidade interobservadores de duas escalas utilizadas para essa
mensuração. A população de estudo foi constituída por homens de 60 anos ou mais que apresentavam
câncer de próstata incidente e que fazem parte da coorte do estudo “Avaliação Geriátrica Multidimensional
em idosos com câncer de próstata: viabilidade de implantação e potencial de impacto na sobrevida”. Para
mensurar comorbidade utilizou-se a escala de Charlson e a Cumulative Illness Rating Scale Geriatric
(CIRS-G). Foram analisadas variáveis sociodemográficas e de condição de saúde obtidas com base em
instrumentos que integram a Avaliação Geriátrica Multidimensional, como capacidade funcional e
polifarmácia. Estimou-se a prevalência de comorbidade segundo as duas escalas, sendo calculadas razões de
prevalência nos diferentes estratos das variáveis independentes. Utilizou-se a regressão de Poisson na
análise múltipla para avaliar associações entre o escore total da CIRS-G e as variáveis que foram
biologicamente relevantes ou apresentaram associação positiva na análise bivariada. Para a análise de
confiabilidade, os prontuários dos pacientes recrutados em um dos hospitais participantes foram analisados
por duas enfermeiras do serviço de oncologia. Utilizou-se as escalas de Charlson e a CIRS-G, com base nos
critérios padronizados de cada uma delas, para mensurar comorbidade. Foram calculadas medidas de
tendência central e de dispersão para as variáveis contínuas e distribuições de frequências para as variáveis
categóricas. A confiabilidade interobservadores foi analisada por meio de concordância simples, do
coeficiente Kappa de Cohen e pelos coeficientes de correlação intraclasse (CCI). Observou-se que a
prevalência de comorbidade moderada variou entre 7,5% e 66,4% com base, respectivamente, na escala de
Charlson e na CIRS-G. Escolaridade, dependência funcional em AVD, dependência funcional em AIVD e
uso de polifarmácia apresentaram associação com comorbidade na análise bivariada. No modelo final, uso
de polifarmácia, dependência funcional em AVD e dependência funcional em AIVD mantiveram-se
associadas à comorbidade. No estudo de confiabilidade, com base na escala de Charlson, observou-se que a
maioria dos indivíduos estudados não apresentava comorbidade no momento do diagnóstico do câncer de
próstata, enquanto com base na CIRS-G, a maior parte apresentava comorbidade moderada Observou-se alta
concordância, com coeficiente Kappa de 0,66 e CCI de 0,84 para a escala de Charlson. A concordância para
a CIRS-G foi um pouco menor, com coeficiente Kappa de 0,56 e CCI de 0,78. Com base no conjunto dos
achados, a escala CIRS-G parece ser a mais apropriada para uso em idosos brasileiros, possibilitando
identificar condições de saúde frequentes neste grupo de indivíduos. A comorbidade tem sido associada à
redução da qualidade de vida dos idosos e ao aumento dos custos com a saúde. Dessa forma, é importante a
utilização de escalas para mensurar essa condição em estudos clínicos e epidemiológicos.
Abstract
The growth of the Brazilian aged population brought about changes in health patterns, leading
to the prevalence of chronic non-communicable diseases, among them cancer. Comorbidity is
defined as the coexistence of two or more chronic diseases in a single individual and is
a common condition in older adults with cancer. As so, it may influence their diagnosis,
treatment and prognosis. This dissertation aimed to estimate the prevalence of comorbidity in
older cancer patients, to analyze associated factors and to evaluate interobserver reliability of
two scales used for this measurement. The study population consisted of men 60 years or older
with incident prostate cancer that are part of the study cohort " Multidimensional Geriatric
Assessment in elderly patients with prostate cancer: feasibility of implementation and potential
impact on survival". To measure comorbidity, we used the Charlson’s scale and the Cumulative
Illness Rating Scale Geriatric (CIRS-G). Socio-demographic and health status variables as
functional capacity and polypharmacy, obtained through instruments that comprise the
Multidimensional Geriatric Assessment, were analyzed. Prevalence of comorbidity was
estimated with these two scales and prevalence ratios were analyzed for different strata of the
independent variables. Poisson regression was applied to estimate associations among CIRS-G
total score and variables biologically relevant or that showed a positive association in bivariate
analysis. For the reliability analysis, two nurses from the oncology service analyzed the records
of patients enrolled in one of the participating hospitals. The two scales were applied to measure
comorbidity based on their standardized criteria. Interrater reliability was analyzed by simple
agreement, kappa coefficient and intraclass correlation coefficients (ICC). The prevalence of
moderate comorbidity varied between 7.5% and 66.4 % based respectively on the Charlson’s
scale and on the CIRS–G. Level of education, functional dependence in ADL and IADL and
polypharmacy were associated with comorbidity in the bivariate analysis. In the final model,
polypharmacy, functional dependence in ADL and functional dependence in IADL remained
associated to comorbidity. With the Charlson scale, it was observed that most of the subjects
had no comorbidity at diagnosis of prostate cancer. Meanwhile, most elderly people had
moderate comorbidity based on CIRS-G. Charlson’s scale showed high interrater agreement,
with kappa coefficient of 0.66 and ICC of 0.84, while for the CIRS-G interrater agreement was
a little lower, with kappa coefficient of 0.56 and ICC of 0.78. Based on scale CIRS - G, most
elderly people had moderate comorbidity. The study findings suggest that the CIRS-G could be
more suitable for use in elderly Brazilians, since it allows the identification of common health
conditions in this population. Comorbidity has been associated with reduced quality of life for
seniors and with rising health costs. Thus, the use of scales to measure comorbidity in clinical
and epidemiological studies is important.
