Este trabalho realiza uma análise das políticas públicas para o HIV/Aids no estado do
Amazonas, tendo como referencial as relações entre as três esferas da federação brasileira no
âmbito do Sistema Único de Saúde. As políticas públicas brasileiras para enfrentamento do
HIV/Aids, em geral, são consideradas exitosas em diversos estudos. No entanto, restam ainda
alguns desafios, especialmente ao se olhar para os diferentes níveis da epidemia em algumas
regiões do país. Sob essa lógica, o estado do Amazonas é um dos que apresentam alguns dos
piores indicadores relativos ao HIV/Aids nos últimos anos em relação às médias nacionais. Este
estudo apresenta histórico das políticas para o HIV/Aids primeiramente no Brasil e, em seguida,
no estado do Amazonas, enfocando nas questões da descentralização e regionalização. Em
seguida, foram reunidos documentos das instâncias formais do SUS, a fim de verificar como se
dão as relações entre gestores e a participação da sociedade civil nesses foros. Assim, foi feito
levantamento de registros de reuniões e de resoluções do Conselho Estadual de Saúde (CES) e
da Comissão Intergestores Bipartite CIB) do estado Amazonas no período de 2010 a 2016, que
nos mostrou a força das políticas de indução de ações do Ministério da Saúde por meio do
chamado instrumento da Programação de Ações e Metas (PAM), em contraste com a
fragilidade, tanto das instâncias intergestores do SUS no Amazonas, como é o caso da CIB
local, quanto da própria Secretaria de Estado da Saúde do Amazonas. Tal fato parece levar à
consecução de políticas para o HIV/Aids no Amazonas insuficientes para suprir as
peculiaridades locais e regionais, tal como nos parecem apontar os atuais índices
epidemiológicos para esse agravo naquele estado.
Abstract
This work analyzes the public policies for HIV / AIDS in the state of Amazonas, using as
reference the relations between the three spheres of the Brazilian federation within the scope of
the Unified Health System. Brazilian public policies to face HIV / AIDS, in general, are
considered successful in several studies. However, there are still some challenges, especially
when looking at the different levels of the epidemic in some regions of the country. Under this
logic, the state of Amazonas is one of those that present some of the worst indicators related to
HIV / AIDS in recent years in relation to national averages. This study presents a history of
policies for HIV / AIDS primarily in Brazil and then in the state of Amazonas, focusing on the
issues of decentralization and regionalization. Next, documents were gathered from the formal
instances of the SUS, in order to verify how the relations between managers and the
participation of civil society in these forums are. Thus, a record of meetings and resolutions of
the State Health Council (CES) and of the Bipartite Intergovernmental Commission (CIB) of
the State of Amazonas was carried out considering the period from 2010 to 2016, which showed
us the strength of the policies of induction of actions of the Ministry through the so-called
Program of Actions and Goals (PAM), opposed to the fragility of both the SUS
intergovernmental bodies in Amazonas, as is the case of the local CIB, and of the State of
Amazonas State Health Department itself. This fact seems to lead to the achievement of HIV /
AIDS policies in Amazonas that are insufficient to meet the local and regional peculiarities, as
the current epidemiological indexes for this disease in that state seem to point.
