PREVALÊNCIA DA COINFECÇÃO POR HCV E ESTAGIAMENTO DA FIBROSE HEPÁTICA PELA ELASTOGRAFIA TRANSITÓRIA EM PACIENTES DE UMA COORTE CLÍNICA DE HIV DO RIO DE JANEIRO

A coinfecção por hepatites virais permanece como causa importante de complicações e mortalidade em indivíduos infectados pelo HIV. O objetivo desse estudo é avaliar a prevalência e fatores associados à coinfecção pelo vírus da hepatite C (HCV) e presença de fibrose hepática por métodos não-invasivos nos pacientes infectados pelo HIV. Pacientes infectados pelo HIV em acompanhamento no INI/FIOCRUZ eram elegíveis para este estudo retrospectivo transversal. A coinfecção pelo HCV foi definida pela detecção no plasma do RNA-HCV e a fibrose hepática foi estimada pelo resultado do primeiro exame de elastografia hepática transitória (EHT). A fibrose hepática significativa (METAVIR F \2265 2) foi definida por EHT \2265 7.2 kPa. A prevalência da fibrose hepática significativa também foi estimada utilizando-se biomarcadores sorológicos: APRI \2265 1,5 e FIB-4 \2265 3,25. Fatores de risco associados aos desfechos específicos foram identificados por modelos de regressão logística ajustados por idade e sexo. 4443 pacientes HIV positivos [67% masculino; idade=40 (IQR, 32-49) anos; 6 (3-12) anos de infecção pelo HIV] foram incluídos. A prevalência da coinfecção HIV-HCV foi de 4% (n=173). Uso de drogas injetáveis [OR=17,10 (95%IC 9,14-32,03), p<0,001] e maior tempo de infecção pelo HIV [por ano, OR=1,06 (1,04-1,09), p<0,001] foram significativamente associados com a infecção pelo HCV O total de 107 (63%) pacientes coinfectados HIV-HCV [71% masculino; idade=52 (47-57); CD4=571 (367-881) cels/mm3; 59% com carga viral do HIV indetectável; 61% genótipo 1 e 45% previamente tratado para HCV] foram submetidos à avaliação da fibrose hepática pela EHT entre Março 2012 e Novembro 2016. A EHT mediana foi 8,0 (6,6-11,9) kPa e 63% (n=67) apresentavam fibrose significativa. Ausência de resposta virológica sustentada prévia [OR=3,86 (1,22-12,23), p=0,022] e maior tempo de infecção pelo HCV [em anos, OR=1,09 (0,99-1,18]), p=0,058] foram associadas com fibrose significativa em modelo ajustado para idade e sexo. A prevalência da fibrose foi menor quando definida por biomarcadores (20%) que aquela definida pela EHT. Podemos concluir que 4% dos indivíduos infectados pelo HIV apresentam coinfecção pelo HCV e que 67% dos pacientes coinfectados HIV-HCV apresentam fibrose hepática significativa pela EHT. Implementação de estratégias de saúde pública para usuários de drogas injetáveis e maior acesso ao tratamento para erradicação do HCV são essenciais para redução da transmissão do HCV e incidência de complicações hepáticas em pacientes infectados pelo HIV.
Abstract
Liver diseases remain a main public health issue in HIV-infected patients. HCV coinfection leads to liver fibrosis progression and liver-related complications in patients living with HIV. The aims of this study was to estimate the prevalence and its risk factors for HCV co-infection in HIV-infected patients and presence of significant liver fibrosis in HIV-HCV co-infected patients. HIV-infected patients regularly followed at INI/FIOCRUZ were eligible for this cross-sectional study. HCV co-infection was defined by positive and liver fibrosis was estimated by transient elastography (TE). Significant fibrosis (METAVIR F \2265 2) was defined by TE \2265 7.2 kPa. The prevalence of liver fibrosis was also estimated by sorologic biomarkers: APRI \2265 1.5 e FIB-4 \2265 3.25. Age and gender multivariate logistic regression models were performed for identification of risk factors associated with HCV co-infection and liver fibrosis. 4443 HIV-infected patients [67% male gender; aged of 40 (IQR, 32-49) years and 6 (3-12) years of HIV infection were included. The prevalence of HCV co-infection was 4% (n=173). Injection drug use (IDU) [OR=17.10 (95%IC 9.14-32.03)] and higher duration of HIV infection [per year, OR=1.06 (1.04-1.09), p<0.001] were significantly associated with HCV infection in HIV patients. A total of 107 (63%) HIV-HCV co-infected patients [71% male gender; age of 52 (47-57) years; CD4=571 (367-881) cells/mm3; 59% with undetectable HIV viral load; 61% genotype 1 and 45% previously treated for HCV] were submitted to liver fibrosis estimation by TE between March 2012 and November 2016 Median results for TE were 8.0 (6.6-11.9) kPa and 63% (n=67) of patients had significant fibrosis. Absence of previous sustained virologic response (SVR) [OR=3.86 (1.22-12.23), p=0.022] and higher duration of HCV infection [per year,OR=1.09 (0.99-1.18]), p=0.058] were associated with significant fibrosis in multivariate analysis adjusted for age and gender. The prevalence of significant fibrosis in HIV-HCV co-infected patients was lower (20%) when defining liver fibrosis by sorologic biomarkers. The prevalence of HCV infection was 4% in people living with HIV and 63% of HIV-HCV co-infected patients had significant fibrosis by TE. Public health strategies to reduce IDU and to reach higher rates of SVR are essential to decrease HCV infection and liver-related complication in patients living with HIV.
