O crescente envelhecimento mundial é consequente ao aumento da expectativa de vida
especialmente nas pessoas de idade mais avançada, tendo expandido o número de
idosos com doenças crônicas, em especial doenças oncológicas, com risco de perda da
funcionalidade pelos eventos de crise nas trajetórias de doença, e que em algum
momento pode necessitar de cuidados que unidades de tratamento intensivo. Muitos
tornam-se doentes crônicos críticos, que demandam tratamentos e cuidados
especializados, apresentam-se como um desafio à prática médica, pela
multidimensionalidade das questões bioéticas implicadas, ressaltando-se o risco da
perda de sua autonomia, a vulnerabilidade e a suscetibilidade intrínsecas nestas
condições, e a ameaça à dignidade com comprometimento da qualidade de vida
decorrente dos efeitos do paradigma biotecnocientífico apoiados pelo binômio
biopolítica-biopoder que caracteriza a medicina contemporânea.
Nesse cenário, o presente estudo procede de uma investigação ampla descrevendo
sobre a evolução da ciência e da biomedicina, caracterizada na técnica dos cuidados
intensivos e suas ameaças de obstinação pelas possibilidades terapêuticas
contemporâneas, as demandas não atendidas de cuidados paliativos (CP) para as
pessoas idosas com doença crônica crítica – a necessidade de proteção em uma
perspectiva bioética no fim de vida – e a oportunidade para integração dos cuidados em
saúde. A equipe médica tem pouca consciência da importância das questões bioéticas
envolvidas, da necessidade de fortalecer seus conceitos e de embasar a reflexão sobre
a conflitualidade moral implícita em suas práticas assistenciais aos idosos,
especialmente no fim da vida.
O curso metodológico inicia-se pela pesquisa qualitativa, teórica-conceitual,
promovendo uma reflexão entre fatos biomédicos e referenciais bioéticos, seguido de
um estudo observacional de coorte na UTI, caracterizando a demanda por cuidados
paliativos, e uma revisão sistemática de literatura sobre cuidados paliativos integrados
em cuidados intensivos. Discutimos o exercício da autonomia pelos idosos com câncer
na UTI e as questões do paradigma biotecnocientífico e biopolítico da assistência à
saúde; a vulnerabilidade e a dignidade como princípios orientadores para a oferta de
cuidados paliativos, a demanda por estes e a necessidade de proteção dos idosos com
doenças críticas crônicas; assim como a questão da integração precoce dos CP,
especialmente nos cuidados na UTI.
Concluimos o estudo, propondo uma articulação entre Bioética de Proteção e Cuidados
Paliativos, no intuito de ampliação das possibilidades médico-sanitárias em que
profissionais bem qualificados possam assistir e orientar idosos e familiares a refletir
sobre saúde e doença, autonomia e incapacidade, vulnerabilidade e ‘vulneração’, de
acordo com seus valores e preferências para o fim de vida.
Abstract
The increasing global aging is a result of increased life expectancy, especially among the
elderly, and has expanded the number of older people with chronic diseases, particularly
oncological diseases, who are at risk of loss of functionality due to crisis events in their
pathways with diseases, and that at some point may need care in intensive care units.
Several become chronically critically ill patients, who demand specialized treatments
and care, present themselves as a challenge to medical practice, the multidimensionality
of the bioethical issues involved, highlighting the loss of their autonomy, intrinsic
vulnerability and susceptibility in these conditions, and the threat to the dignity with
compromise of quality of life due to the effects of the biotechnoscientific paradigm
supported by the biopolitical-biopower binomial that characterizes contemporary
medicine.
In this scenario, the present study comes from a broad investigation describing on the
evolution of science and biomedicine, characterized in the intensive care technique and
its threats of obstinacy by contemporary therapeutic possibilities, the demands not
addressed of palliative care (PC) for the elderly with critical chronic illness – the need for
protection in a bioethical perspective at the end of life – and an opportunity for the
integration of healthcare. The medical team is poorly aware of the importance of the
bioethical issues involved, of the need to strengthen their concepts and to support their
reflection on the moral conflict implicit in their care practices for the elderly, especially
at the end of life.
The methodological course begins with qualitative, theoretical-conceptual research,
promoting a reflection between biomedical facts and bioethical references, followed by
observational cohort research in the ICU, characterizing the demand for palliative care,
and a systematic review of literature on integrated palliative care in intensive care. We
discuss the exercise of autonomy by the elderly with cancer in the ICU and the issues of
the biotechnoscience and biopolitical paradigm of healthcare; vulnerability and dignity
as guiding principles for the provision of palliative care, the demand for them and the
need of protection of the elderly with chronic critical illness; as the issue of early
integration of PC, especially in ICU care.
We conclude the study, proposing the articulation between Bioethics of Protection and
Palliative Care, in order to expand the medical-sanitary possibilities in which wellqualified professionals can assist and guide elderly and family members to reflect on
health and diseases, autonomy and disability, vulnerability and ‘vulnerabilizing’
according to their values and preferences for the end of life.
